FL Podiatrist Discusses Grades of Ankle Sprains
Dr. Bryan C. Swift is a podiatrist with the Orthopedic Center of Florida in Fort Myers and Bonita Springs. "An x-ray of the foot and ankle can determine the severity of the injury (ankle sprains) and rule out a fracture. Sometimes an MRI is required for higher grade ankle sprains to determine if cartilage, ligaments or tendons are damaged," says Swift. Grade 1 is a mild injury and may require an ankle brace or air cast. This injury may require R.I.C.E. (rest, ice, compression and elevation), early physical therapy, and two to three weeks healing time.

Grades 2 and 3 are partial to severe tearing or rupture of multiple ligaments that include swelling and bruising on the injured area. This level of injury may require a boot or splint, R.I.C.E., physical therapy when tolerable, four to 12 weeks healing time, and possibly arthroscopic surgery if the pain persists more than two to six months.

MI Podiatrist Provides Tips for Buying Back-to-School Shoes
Do they glow in the dark? Would a princess wear them? When shopping for back-to-school shoes, these are the factors that weigh in with children. “However, while it’s important for them to have a shoe that they like and that they feel good in, there needs to be a compromise for support,” says Dr. Jodie Sengstock of West Bloomfield Township, past president of the Michigan Podiatric Medical Association.

“Children spend a lot of time at school and they need a shoe that is comfortable all day — while they’re sitting in a classroom, but also when they’re outside for recess,” she says. Oh, and forget the old saying the longer you wear the shoes the better they’ll feel? It’s a myth. “If it’s not comfortable when you leave the store, it’s not going to be comfortable later,” Sengstock says.

Global Onychomycosis Market Estimated to Account for 3 Billion Dollars
Persistence Market Research (PMR) delivers key insights on the global dermatophytic onychomycosis therapeutics market in its latest report titled “Global Market Study on Dermatophytic Onychomycosis Therapeutics (DOT): North America Expected to Account for Maximum Market Share by 2021”. Market growth is primarily driven by factors such as increase in the prevalence of fungal toenail infections in the U.S. and Canada, and rising per capita healthcare expenditure across the globe.

Aetna Deals Blow to ACA Exchanges with Mass Retreat
Aetna will drastically cut back its 2017 participation on the individual markets, the for-profit insurer said Monday evening, just months after expressing optimism in the future of the Affordable Care Act's exchanges.
Aetna's about-face builds on its pessimistic second-quarter earnings as well as the actions of two other large publicly traded insurers. UnitedHealth Group and Humana similarly decided to scale back their ACA plans after suffering larger-than-expected losses.

8 Patient Collection Mistakes to Avoid - Part 3
Medical practices are caught in the crosshairs when it comes to collecting patient co-pays and deductibles. On one hand, they are obligated by their contracts with payers to collect patient cost-sharing amounts; on the other hand, patients are being faced with unprecedented medical bills. If you are caught in this collection free-for-all, here are the eight worst things you can do.

Query: Routine Nail Care Coding
If a patient has neuropathy from chemotherapy, but is not a diabetic and does not meet class findings (Q7, Q8 or Q9), is the patient covered under Medicare for routine nail care? I looked in the routine foot care LCD, but the language is unclear.

Response: The Ohio LCD says the patient must have class findings to be covered for routine foot care. However, directly under that it says, "Benefits for routine foot care are also available for patients with peripheral neuropathy involving the feet, but without the vascular impairment outlined in Class B findings. The neuropathy should be of such severity that care by a non-professional person would put the patient at risk. If the patient has evidence of neuropathy but no vascular impairment, the use of CLASS FINDINGS MODIFIERS IS NOT NECESSARY. This condition would be represented by the ICD-10-CM codes in the 3rd table of “ICD-10 Codes that Support Medical Necessity” listed below.”
In the codes that are listed below, one of the options is G62.0, drug-induced polyneuropathy, which is what I consider the appropriate code for neuropathy from chemotherapy. This tells me that the patient you are asking about should be covered for routine foot care with a qualifying diagnosis of G62.0 even in the absence of class findings. I suggest you document the nature of their neuropathy and that having this done by someone other than a podiatrist would place this patient at risk.
Finally, be aware that G62.0 has the instruction to "Use additional code for adverse effect, if applicable, to identify drug (T36-T50 with fifth or sixth character 5)”. You should include that if you know the drug.

With all due respect, I have to disagree with Ms. Thompson that the “9%” reduction in reimbursement under MACRA is recoupable. When the policy is fully implemented, as it has already begun, physicians are already targeted based on their utilization cost and outcome generated by the eQCMs and under MIPS.

Beginning THIS year, CMS and HHS have begun tying and targeting 30% of all reimbursement to quality initiatives. This includes PQRS. By 2018, the number will be 50%. By 2021, 90%. It is not a matter of whether as a provider you WANT to participate by reporting quality measures, it is a question of whether or not...
