Abandon Heel Strike: OH Podiatrist

ACFAP Appoints VP, Receives $5,000 Grant from Tetra Corporation

Saturday's pediatric track at the 2012 APMA national meeting featured two very special events for the American College of Foot and Ankle Pediatrics (ACFAP). First, ACFAP appointed Louis J. DeCaro, DPM of West Hatfield, MA Vice President.

NC Podiatrist Joins Wound Care Center

Dr. Patrick Ricotta, a podiatrist, has joined Southeastern Wound Healing Center as a panel physician. He will periodically treat patients at the center, which specializes in the treatment of chronic wounds and non-responsive conditions and offers hospital-based outpatient wound care and hyperbaric oxygen therapy.

Ricotta earned his doctorate degree from Ohio College of Podiatric Medicine in Cleveland and completed residency training in foot and ankle surgery at the Podiatry Hospital of Pittsburgh in 1996.

TX Podiatrist Holds Shoes for Orphan Souls Drive

Dr. David Andreone is working to collect at least 120 pairs of shoes at his Tyler office for Shoes for Orphan Souls through early September. Shoes for Orphan Souls offers hands-on opportunities to individuals, groups, and organizations wanting to transform lives. Since 1999, more than 2.3 million pairs of new shoes and socks have been distributed to children in 74 countries.

Andreone had his first shoe drive several years ago when he was practicing in Plano and Frisco. Since moving to Tyler, he wanted to continue helping those in need and figured that since he is a podiatrist, shoes just made sense, he said.

CT Podiatrist Opens Spa

The opportunity to become the best you can possibly be is offered by Dr. Joan Tekula and Michelle Morgan, owners of Revèe spa, which opened in May on Bullet Hill Road in Southbury. The spa offers three different non-invasive, non-medical cosmetic procedures. One that Dr. Tekula specializes in is called the TC3000 Vein Erase, which uses a device that thermo coagulates the small veins in the skin. It is only for spider veins in the face and legs. She also does Lipo-Light, which is spot fat reduction.

Dr. Tekula has been practicing medicine for 24 years and, as a podiatrist, she worked with the geriatric population. “Even though I was trained to give drugs and perform surgeries, my heart, my philosophy was just to be as conservative as possible and to exhaust all those other means before moving on to things with greater consequences,” said Dr. Tekula, who added that this was right up her alley in terms of helping people.

2012 Centennial APMA Scientific Meeting One of the Best Ever

Amid the background of our nation's capital, over 1,400 podiatrists and 280 exhibitors enjoyed a well-organized and designed meeting. APMA Annual Meeting Chair Frank Spinosa, DPM and APMA Director of Scientific Affairs James Christina, DPM, along with APMA's hard-working staff again put together a varied program which offered lectures, breakfast and lunch symposia, and hands-on workshops to cover the gamut of clinically relevant topics.

I have a very simple approach to Charcot neuroarthropathy, assuming that this is in a quiescent phase and not infected. I look at them as STABLE or UNSTABLE. The stable Charcot foot is fairly easily accommodated, even if it is significantly deformed. The unstable Charcot foot is the really tough one. The surgical goal that one should keep in mind is changing an unstable to a stable foot and avoiding the obvious sequelae. If reconstructive surgery is not a reasonable option (risks >>benefit), then ablative surgery is certainly reasonable.

I do not think that removal of the entire cuboid would be necessary. In fact, it might add to the instability and those associated problems. I would simply aggressively remove the prominent bone—and then remove a little more—making sure that the peroneus longus was either grafted to the brevis or re-attached to the lateral foot. A TAL is a MUST DO. Forget the gastroc on this one. I would also bet that this patient is obese and would make sure I talked with the “team” member that could make sure that this patient was metabolically maxed out. Using a wheelchair post-op is not a bad idea.

I am not an attorney; however, I do work at a large multispecialty clinic. I do not see any problem with mentioning that there is a problem with a patient’s BMI. In fact, it needs to be brought up. I think discussing that weight can affect the feet is well within our scope of practice. Since you work in a multispecialty clinic, it should be “easy” for you to refer a patient to another provider, dietician, or other nutritional specialist for proper counseling and management.  Discussing specific diets and weight loss programs in a professional setting would, in my humble opinion, be beyond the scope of our practice at this time in Illinois. If I’m wrong, I will be happy to be enlightened.

Dr. Theresa Hughes’ recent inquiry about the legality of podiatrists providing counseling to a patient with an elevated BMI was timely. A just released report from the U.S. Federal Inter-agency Forum on Aging-Related Statistics found that obesity is becoming increasingly prevalent among the elderly. The report states that in 2009-2010, 38 percent of people age 65 and older were obese, whereas only 22 percent were obese between 1988 and 1994. Among those age 65-74, the 2009-2010 data showed that 44 percent were obese, as were 29 percent of those age 75 and older.

Dr. Hughes’ question implicitly asks whether we should provide some form of BMI counseling. The APMA has addressed the subject of childhood obesity. Does anyone know if the APMA has formally spoken to adult obesity? The podiatric profession has more of a public health role to play in America’s obesity epidemic than advising as to which is the best walking shoe.

As a contemplative man, and a reflective podiatrist, I always like to ask the question, "WHY?" Why are we being presented with patients who we call "The Challenging Patient."
 
Here's my take. Our focus, as a profession values, trains and offers residencies in surgery and perhaps looks at "The Surgical Residency" as more favorable than another type. The jury is still out on that. But, nevertheless, when our focus is on "surgical expertise", we may be creating a VOID in understanding the "mechanisms" of a disease pathology -- necessary and...
