PODIATRIC PODCASTS
VA Podiatrist Interviews Footwear Guru

Ben Pearl,  DPM interviewed  Pedorthic Footcare Association past president Bob Schwartz, CPed regarding the concept that shoes tell a story regarding pathology of the foot. Schwartz emphasized that not only should you be looking at the outsole of the shoe but the insides of the shoe and any wear patterns on the orthotics or insoles. Here is a video clip of the podcast.

Robert Schwartz

Dr. Pearl reviewed some shortcuts for adding padding to orthotics that are seamless such as 1 mm Poron. Schwartz schedules follow-ups for orthotics and shoes to make sure the wear pattern does not show that the modifications are compromising the clinical outcomes.

PODIATRISTS AND THE LAW
WY Podiatrist Pleads Guilty to One Additional Count of Sexual Abuse

A former podiatrist who was sentenced to prison earlier this year for sexual abuse of a minor pleaded guilty last month to one more count of sexual abuse. In January, Craig Capron, 60, was sentenced to six to 12 years in prison after pleading guilty to four of 10 counts of second-degree sexual abuse of a minor in Campbell County. A sentencing date and a plea agreement were not available on the courts’ electronic docket system as of Thursday afternoon.

PRACTICE MANAGEMENT TIP OF THE DAY
Preventing a Malpractice Claim Before the Patient Arrives

By the time a patient sets foot in your waiting room, many malpractice risks have already been set in motion or avoided. From your online presence to your appointment scheduling process, preventive steps taken before the first visit can help minimize legal exposure and protect your reputation. Here are nine ways to get ahead of potential trouble.

4. Screen for high-risk patients before scheduling

Establish intake protocols to flag patients who may require specialized care outside your scope. This can include complex cases, frequent malpractice filers, or individuals with unrealistic expectations. A structured screening process can help determine if your practice is the right fit and reduce the likelihood of disputes.

FINANCIAL TIP OF THE DAY
The 8 Financial Documents You Should Always Shred

Many financial documents include most or even all the information needed to steal your identity, like your name and mailing address, email address, bank account numbers, insurance policy numbers, and more. Shredding these financial documents once you no longer need them can help keep your information safe:

1. Tax returns. Tax returns contain personal and financial information, including your Social Security number. While it’s important to keep your tax returns for at least seven years in case of an audit, you can shred tax returns older than seven years.

CODING & BILLING Q&As FROM THE DOCTORLINE.COM
Query:  When UHC asks for records what does that mean?

I billed the following CPT codes for a surgery: CPT 28306, CPT 28310 and CPT 28313. They are asking to submit records, in this case an operative report, but what triggered a request? This is the first time I am seeing this and do not know why.

Response from thedoctorline.com: I believe it was the NCCI edits as CPT 28310-Osteotomy, shortening, angular or rotational correction; proximal phalanx, first toe (separate procedure) is column 2 to CPT 28306-Osteotomy, with or without lengthening, shortening, or angular correction, metatarsal; first metatarsal. This would make sense if there was really a great toe osteotomy bunionectomy because the correct code is CPT 28298-Correction, hallux valgus with bunionectomy, with sesamoidectomy, when performed; with proximal phalanx osteotomy, any method.

If NO bunionectomy was performed, then you must demonstrate how the "separate procedure" is anatomically distinct. In your operative report did you use the word "separate and anatomically distinct"? Rule: The codes designated as "separate procedure" should not be reported in addition to the code for the total procedure or service of which it is considered an integral component.

However, when a procedure or service that is designated as a "separate procedure" is carried out independently or considered to be unrelated or distinct from other procedures/services provided at that time, it may be reported by itself, or in addition to other procedures/services by appending modifier -59 to the specific "separate procedure" code to indicate that the procedure is not considered to be a component of another procedure, but is a distinct, independent procedure. This may represent a different session, different procedure or surgery, different site or organ system, separate incision/excision, separate lesion, or separate injury (or area of injury in extensive injuries).

David Freedman, DPM, Silver Spring, MD. Additional coding and billing questions and responses can be found at thedoctorline.com

RESPONSES/COMMENTS (OBITUARIES)
RE: The Passing of Robert M. Ritchey, DPM

Rob did not want an obituary. He did not want a memorial. However, I have come to the realization that I need to share the extraordinary impact of my husband's presence on this earth. His artwork was on par with even the most skilled masters.

Having a life long fascination with human behavior, Rob wrote novels reflecting this genre. In his words, "From Purple Mountain Majesty to the Krebs cycle, it is all a beautiful dance. The universe must have danced with joy when reuniting with your indomitable energy, my darling. You are loved and will be deeply missed."

RESPONSES/COMMENTS (CLINICAL)
From: Mark Spier, DPM

If your ID consultant hasn't done so yet, try to obtain a fungal culture from the wound.  S. hominis may not be the causative organism responsible for this wound infection. Sometimes a mycological etiology cannot be definitively identified, so empirical antifungal therapy may provide a resolution in this instant case as it did for a similar patient of mine many years ago. Best of luck!

RESPONSES/COMMENTS (MEDICAL EDUCATION)
RE: Validity of Caribbean Medical Schools
From: H. David Gottlieb, DPM

It has been mentioned here that Caribbean medical schools are as legitimate as American medical schools. I do not question that some might be. I am wondering though why a legitimate school of any type would find the need be advertising on Reddit? I have recently seen many ads for a medical school named “American University of the Caribbean”. Personally, I would be leery of any medical school advertising on such an unlikely place. I would also be leery of anyone with a diploma from said college.
