Anesthetic Shortage Impacting NV Podiatry Office
A national trend is reaching medical offices in the Biggest Little City. In the last few months, Reno Foot and Ankle has been dealing with a shortage of local anesthetics. “We’ve experienced some shortages in the past but nothing as significant as what we’re experiencing right now,” said Lacey Loveland, DPM. The office sees about 30 to 40 patients a day and is getting close to its last bottles of Lidocaine.

If they don’t get more soon, Dr. Loveland worries they won’t be able to do certain procedures. “Like ingrown toenails or even excisions of potentially cancerous lesions, anesthetics for wound care,” she said. Loveland told KOLO-8 News Now that she has called multiple companies. One of them explained they’re on backorder until next month, whereas the others are more uncertain. “One of them has nothing and doesn’t know when they’re going to get it in, one is thinking that maybe next year,” she said. According to a local hospital pharmacy, right now Marcaine and Carbocaine are being used for surgery only, while Lidocaine is in low supply.

FL Podiatrist Completes Prestigious Gravel Bike Race
Matt Werd, DPM just returned from completing the Steamboat Gravel Race (STB GRVL) which is considered as one the top five most prestigious gravel bike races in the country by VeloNews elite gravel racers, and race promoters. It is a 100-mile off-road gravel bike race taking place at high elevation in the Rocky Mountains in Steamboat Springs, Colorado.

3,000 racers from all 50 states and around the world participated in SBT GRVL, in which entries are capped and sell out within minutes of registration opening. This year’s field of racers endured the full spectrum of weather extremes which included heat, high winds, torrential cool rains, lightning strikes, flooding, and even livestock on the trails.

Query: Billing Company and Balance Billing
We have recently switched to a billing company that insists on sending patients statements even when their insurance company pays for the service. For example, for a CPT 11721, we bill Medicare and collect around $41. They insist on billing the remainder of what our fee schedule says to the patient and said they have a legal obligation to the carrier to do so? Is that true?

Response: There is absolutely no requirement to balance bill a patient for a balance they do not owe. Your example is not completely clear, but if I understand the amount billed, it sounds like it is above the allowed amount? If so, billing the patient risks the patient paying that amount which would violate all participation agreements for billing above the amount allowed by the insurer. I have heard of providers and billing companies billing the patient prior to insurance paying to let them know there was a balance, but never billing after the balance was paid up to the allowed amount.
A less serious issue is that it costs them, or you, to send unnecessary statements, and likely confuses some patients receiving a “bill” they do not owe and should not pay. If they aren’t passing that cost on directly, then the extra cost is in their fee that you are paying. You may want to challenge the billing company to show you where it is written (if it isn’t written, then it isn’t true!) and send you the document to review.

Thank you, Tim Ford, DPM, for the most sensible response yet to the ABPM CAQ in the podiatric surgery issue and the most balanced and useful opinion on the entire podiatric training issue. Of course, not all podiatric residency programs are equal…there is considerable diversity among them. Of course, a CAQ is only a CAQ and is not board certification. Of course, we ultimately need to consolidate our surgery and medical boards into one board certification in podiatry.
I would add that CAQs by ABPM would recognize additional competence in surgery, wound care, sports medicine, etc. Our board certification process needs to recognize what all competent podiatrists have in common, but also that there is diversity among us and that some of us have additional competencies that deserve to be recognized. This evolution will make us stronger as a profession and be less confusing to the medical establishment. We need to evolve to be more easily understood.

Dr. Jacobs - I never stated in my post that all residencies were equal. I wish they were. My problem is the weaponizing of board certification as proof of competency falls short. The graduate of a program who performs all the fancy procedures you mention should be able to use his logs to demonstrate he has been educated. Protecting the public is the responsibility of the State education department and office of professional discipline.
Doctors took the Hippocratic Oath and should not be performing procedures they haven’t learned or are not competent to perform. I don’t believe it’s the boards’ responsibility to play podiatry police.

I think the answer to Dr. Rosenblatt's question is actually very simple... compounding interest and fees over 30 plus years. Compounding interest can be very dangerous with loans repayment (likewise very beneficial with investments). If loans are unpaid and payments are missed, the debt adds up and interest drives it up quick. It continues to add even more when there are fees added to the original principal for missed payments, etc.
It is heart wrenching to read this but should also be a wake-up call for all those who are borrowing money to get an education that could ultimately lead to an insurmountable repayment amount.

It was very sad to read about the passing of Scott Becker. Our paths first crossed almost 30 years ago when I won a prize for getting closest to the pin on a par three hole while playing golf at a summer tournament during an FPMA convention in Florida. The prize was TEN free pairs of custom orthotics from Scott's Lab, JSB Orthotics. I've been using JSB ever since and enjoyed a wonderful relationship with Scott. He was a great person and an asset to the podiatric profession. My condolences to his family and all the gang at JSB orthotics.
