CA Podiatrist Discusses Plantar Fasciitis
“We typically treat plantar fasciitis conservatively, at first,” said Michael J. Cornelison, DPM, a foot and ankle surgeon and president of the American College of Foot and Ankle Surgeons. “This can include a combination of remedies, including rest, exercises that stretch the calf muscles, orthotics, icing the heel, over-the-counter medications like ibuprofen, supportive footwear, and physical therapy.”

Most patients respond well to conservative treatments. However, depending on the severity of the plantar fasciitis, additional therapies may be required, including: Injection therapy, shockwave therapy, ultrasonic treatment, and surgery. No matter what kind of treatment a patient undergoes, the underlying causes that led to the condition may remain. Preventive measures, such as wearing supportive shoes, stretching, and using custom orthotic devices, are the mainstay of long-term treatment for plantar fasciitis.

Virginia Beach Podiatry Meeting Delivers Diverse Presentations
The Virginia Beach Podiatry Conference this past weekend had a variety of topics that were well received by the attendees. The topics ranged from non-unions by Michael Downey, DPM, plastic surgery techniques by Dock Dockery, DPM, coding for a level 4 visit by David Freedman, DPM, to new sports medicine technologies by Ben Pearl, DPM. Takeaways included Dr. Downey's assertion that although orthobiologics have a role in non-unions, podiatrists forget about the value of using morselized bone autograft. Dr. Dockery discussed how the bowtie technique can relax tension on a circular wound.

Four Ways Physicians Can Minimize Interruptions
Interruptions are a chief impediment for doctors and medical office staff when it comes to getting things done. Here's how to deal with them.
2. Increase your message monitoring interval.
If you monitor your messages every five minutes to 10 minutes, increase the interval to 15 or 20. In the grand scope of things, that’s not a big deal in terms of staying informed, while at the same time it affords you the ability to stay focused and accomplish great things. In time, increase the interval to 25 minutes to 30 minutes. You’ll accomplish even more. On those rare occasions when you check your messages and something requires your immediate response, by all means, get in touch with that party. To not do so would create more anxiety than you need to endure.

The original studies performed at Lehigh University by Dr. Nabolah showed that only one application of the Phenol EZ Swab was needed to destroy the matrix. We have marketed that fact for over 31 years.

For chemical matrixectomies, I was taught by my program manager, and used, 3 x 30 seconds phenol (88%) applied with a debulked Q-tip. We also "neutralized" it with saline. I would consider a 2 x application, but sometimes the first application would reveal blood on the Q-tip, suggesting that the initial application was effectively diluted.
Also note that Phenol-EZ Swabs, while an acceptable quality product, was too generous and could cause spill-over chemical burns. We ordered phenol in bulk from a medical distributor, with the usual shipping hazard fee, and poured it into a 1 oz. brown drug jar. Gloves and a running sink were on the standby for safety.

I have used the one minute PA procedure for over 40 years and it works fine. I apply a snug Band-Aid and have the patient remove it the next day. I then have them apply isopropyl 70% alcohol bid x 2 weeks. Do not apply ointment, creams, etc. Showers are okay, but no baths or swimming are allowed until healing has occurred, which is about 4 weeks.
I apply phenol, then apply a dry pledget of cotton, then repeat one more time. I give the patient a post-op care sheet advising of a phenol flare at about 5 days and of drainage in about 3 weeks. I have the patient keep the toe open to the air as much as possible starting the second day post-op.

Finally, Pennsylvania will allow 100% of CME credits to be taken online. Some groups spread across the country may have to rethink their live conferences. Of course, there is nothing like hands-on for perfecting skills. I only feel sorry for the small companies dedicated to podiatry. They will have to remodel their sales approach.

With all of this arguing about which board should certify podiatric surgery, we are ignoring an elephant in the room and it’s a big one. At a dinner this past week, a gastroenterologist was complaining to me that at his university-based hospital, PAs and nurse practitioners are fighting for the right to do endoscopies and colonoscopies and will probably win. Why? Because the hospitals make more money that way. At my local hospital, some of the PAs working in the OR told me that many of the surgeons will allow them to do belly surgery, skin-to-skin, with or without their physical supervision. When my mom fractured her pelvis, a PA or nurse practitioner took care of her from the moment we entered the ER to the moment we left, and we saw no medical doctor.
As Dylan wrote, "The times they are a changin." and only time will tell if it will be better or worse for patients.

My colleague and friend Dr. Jacobs is spot-on regarding podiatric medicine and surgery residency (PMSR) training. Having the opportunity to evaluate many programs in the past 20 years, I can tell you there is a vast difference between programs. Although CPME 320/330 provides common requirements, institutionally and program-wise, there is a clear difference in residency training across the country. This is particularly true when it comes to academic medical centers and community hospitals, not just in training but in overall institutional monitoring of residency programs.
Academic institutions often have 100+ residency and fellowship programs they oversee, whereas community hospitals may have only a PMSR at their facility. A critical component of medical education involves interaction with other residents and fellows in various specialties which enables valuable interdisciplinary learning to be...

Regarding the podiatrist who has an almost $900,000 college loan burden: This shocking article requires more data. I am certain that I am not the only podiatrist who wonders how this could occur. We need a detailed financial analysis of this debt. I realize there may be personal issues involved. But if it is true that she had a form of loan forgiveness which has not been actuated, we need to find out why not. Perhaps one has to inquire and follow it up. Maybe you just can't leave it alone and expect someone else to do it for you.
Otherwise, it is just incendiary publicity that the enemies of more people entering podiatry will certainly capitalize upon. She started out with a $227,000 debt. This "morphed" into a situation where it appears that only part of the interest, if any, was paid. Yet, she insists she was and is not in arrears. This makes no economic sense.
One cannot retire a debt like this without paying the interest completely on a regular basis, which would maintain the base loan amount. Reducing the original loan balance requires payment against the principal as well. It is possible to do calculations on the amount of time and accumulating interest required to balloon $227,000 into 895,512.97 over a 31-year period. Borrowing money requires financial expertise. One cannot simply divorce yourself from the mechanics of any loan. There is an object lesson here. I am certain that I am not the only DPM who wants more data.
