Feet Can Show Early Signs of Systemic Health Problems: NY Podiatrist
Your feet can show early signs of systemic health problems. Poor circulation and hardening of the arteries can affect the feet before other parts of the body, says James R. Hanna, DPM, a podiatrist in Lockport, NY, and president of the New York State Podiatric Medical Association. For example, hardening of the arteries can cause pain in the feet or legs when exercising.

Maintaining good overall health can help your feet stay healthy. You'll also stay aware of health conditions that frequently cause foot problems, such as diabetes. Plus, better foot care can help improve health problems related to diabetes, obesity, and falls, Dr. Hanna says.

Diabetic Amputations Soared Amid Italian Pandemic Lockdown
Italian patients with diabetic foot ulcerations (DFUs) suffered mightily during the height of the coronavirus pandemic in that country, according to a newly published study. Amid a mandatory national lockdown, the rates of amputations skyrocketed at a hospital far from the hardest-hit region as many patients developed gangrene. The study compared 25 patients who were admitted from March 9 to May 18, 2020, with 38 patients who were admitted from a longer period between January and May 2019.
The demographics of the groups are similar, with average ages in the early 60s and more men than women (21:4, respectively, in 2020 and 23:15, respectively, in 2019.) The results reveal high numbers of emergent and serious cases in 2020. Compared with 2019, fewer were outpatients (16% vs. 45%, P = .028) and more were emergency patients (76% vs. 26%, P < .001). Clinically, gangrene was much more common in the 2020 group, compared with the 2019 group (64% vs. 29%, P = .009), as was amputation (60% vs. 18%, P = .001).

Query: Nurse Practitioner and Podiatrist
Our practice has a nurse practitioner (NP) as a member in the group. She functions as an independent practitioner in our podiatry group. This is the scenario that we are trying to figure out: The NP sees a new patient for a bunion deformity and does the initial evaluation and management. Later, the patient decides that they would like to proceed with surgery. They return to the practice but then see a podiatrist for a pre-operative consult.
My question is regarding what E/M series does the podiatrist choose from. For the second visit (with the podiatrist), is this patient established because they are established with the practice? Or is this considered a new patient visit because the NP is not a doctor of the same specialty?

Response: In this scenario, the NP is independent and I assume is billing under a separate tax ID number. Even if this is the scenario, this patient has been established within the practice and should be billed as an established patient within the practice. If this patient was seen by another physician within the group that is not a DPM, this patient is still considered an established patient within the practice.
