PODIATRISTS IN THE NEWS
Tarsal Tunnel Syndrome Can Cause Tingling Feet: IL Podiatrist

Tingling feet isn’t a medical term, but doctors definitely know what it means. It can present itself in a number of ways, says Melissa Lockwood, DPM, a podiatrist at Heartland Foot & Ankle Associates in Bloomington, Illinois. “It can feel like your foot fell asleep and you’re trying to wake it up, or it can feel like your foot is completely numb,” she adds. “It can sometimes be quite painful and burning.”

Dr. Melissa Lockwood

Tarsal tunnel syndrome is basically like carpal tunnel syndrome but for your feet, Dr. Lockwood says. The condition is caused by compression of the posterior tibial nerve (which is found in your foot) and can cause symptoms like pain, tingling, or numbness in your foot, according to Johns Hopkins Medicine. Treatment involves taking anti-inflammatory medications or getting steroid injections into the tarsal tunnel to relieve pressure and swelling. In more severe cases, surgery may be needed.

PODIATRISTS AND STUDENT DEBT
NC Podiatrist Owes $895K in Student Debt

Kathleen LaRose, DPM, a podiatrist in North Carolina, graduated from the California College of Podiatric Medicine in 1991, having borrowed $277,000 in federal loans. Thirty one years later, she owes $895,512.97 in student debt, which Insider verified — all while on an income-based repayment plan that should have granted her loan forgiveness three years ago.

Dr. Kathleen LaRose

While she was in her doctoral and residency program, Dr. LaRose’s loans were placed on in-school deferment, which lets borrowers postpone loan payments until after graduation. But since her student loans were unsubsidized, interest continued to accrue during that period, adding to her principal balance and causing the debt to surge. “I thought, ‘I’m going to be working with a doctorate in this country, so I’ll make a lot of money and be able to pay this,'” Dr. LaRose, now 55, told Insider. “But I never ended up making a ton of money, and I’ve maintained the monthly income-contingent payments, and they’ve never been in arrears. But they’re just wildly out of control now.

PRACTICE MANAGEMENT TIP OF THE DAY
Four Ways Physicians Can Minimize Interruptions

Interruptions are a chief impediment for doctors and medical office staff when it comes to getting things done. Here's how to deal with them.

1. Monitor, but do respond.

Doctors and medical office staff keep their communication technology at hand and periodically check to see who has left a message, and then immediately turn back to the task at hand. By briefly monitoring the calls and text messages that come in, but not responding to them, they are able to turn back quickly to their work and make good progress. You may have already gravitated towards this procedure, but now let’s up the ante.

RESPONSES/COMMENTS (CLINICAL)
RE: How Long to Apply Phenol in a Chemical Matrixectomy
From: Howard E. Friedman, DPM

I recently came across an article about the well-established chemical matrixectomy procedure (P&A) which challenged the conventional technique of 3 one-minute applications of phenol to kill the nail matrix. The authors tested various time applications and concluded that one 1-minute application had the same outcomes as longer applications with less side-effects and quicker healing.

The article is titled, “Comparison of phenol applications of different durations for the cauterization of the germinal matrix: an efficacy and safety study" Acta Orthop Traumatol Turc 2009;43(4):298-302 doi:10.3944/AOTT.2009.298

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1A
RE: ABFAS vs. ABPM
From: Allen Jacobs, DPM

The ABPM surgical CAQ question is emblematic, not unique, to the “surgical certification” issue in podiatry. For many years, this profession has offered alternatives to ABPS now ABFAS for “certification” in foot and ankle surgery. Whether such alternative "certifications" have been accepted for credentialing at the local level is another matter. Additionally, our profession has accepted 2 levels of “recognized” certification, foot and rear foot ankle. Therefore, the concept of “party qualifying” a podiatrist for surgical privileging is neither a unique nor recent concept.

The issue is to what extent should an ABPM member be trusted to provide surgical services, and who should determine the extent of those privileges. A gastroenterologist may perform an endoscopic examination but not a bowel resection. A general dentist may perform extractions or uncomplicated root canals but not a...

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1B
From: Lawrence Oloff, DPM

The last published statement concerning the CAQ by the board ABPM is incredulous. To put in writing that a test that is designed to measure competency is too difficult for its constituency, so they are going to create a test that accepts a lower level of competency is pretty disturbing. And to use gender reasons as one of the reasons to popularize that stance is beyond disturbing. I believe one of the purposes of board credentialing is to protect the public, yet this seems to have been discarded.

The gripe here seems to be that a lot of the members seem to be concerned that the bar to pass the surgery boards is too high. I can appreciate that stance, but not one that takes to lower standards by another board whose members want a less intense examination. Tackle the problem of...

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1C
From: Ivar E Roth DPM, MPH

Concerning Allen Jacobs’ comments, I could not agree more with Dr. Jacobs. At my local hospital, you must be board certified to apply for surgical privileges. Most of the younger doctors here are using the ABPM as their board to qualify. The hospital thinks a board is a board and has been accepting ABPM as an equal board to ABFAS certification. This is because the doctors applying are saying it is an equal. In my opinion, these doctors are disingenuous. I know for a fact that one of them could not pass or qualify for the ABFAS, but yet thinks he can pull the wool over the eyes of the hospital administration and so far this has worked. Until it is firmly determined that ABPM is not officially recognized as a surgical equivalent, this charade will continue. It is my opinion that ABFAS needs to go on the record and send out letters to all the hospitals in the U.S. and clear up this matter.

RESPONSES/COMMENTS (OBITUARIES) - PART 1A

I'm very sad to hear that Scott Becker has passed away. We enjoyed the camaraderie of fellow orthotic laboratory owners over the past decades. Although we only spent time together at the many podiatry conferences and trade shows we attended, we were friends and I will miss Scott. Sending love and condolences to Scott's family and friends.

RESPONSES/COMMENTS (OBITUARIES) - PART 1B

RE: The Passing of Jon "Scott" Becker

I am so saddened to hear of the passing of my friend Jon Scott Becker. Most of us knew him as Scott, the owner of JSB Orthotics. But I think most will agree that Scott had an incredible personality and very much like me, a passion for podiatry. He displayed at almost every podiatry conference and because of his innovation and devotion to his craft, he became quite successful. Scott was one of the many people I looked forward to seeing at the podiatry conferences, but one of the few that had I had the pleasure to spend time with. My condolences go out to his family. He will be greatly missed.
