NY Podiatrist Discusses Cause and Treatment of Bromhidrosis
Podiatrist Emily Splichal, DPM, author of Barefoot Strong, answers the often asked question, "why do feet smell and how can I remedy this condition?" The noxious scent comes when sweat mixes with bacteria on your feet. After your daily wash, sprinkle on a foot powder to absorb excess moisture, leaving bacteria little to feast on. Or, if you've got time, brew some black tea and, once it cools, do a 10-minute foot soak every day for a week, which will shrink the sweat glands.
|
|
Dr. Emily Splichal |
If your feet sweat excessively (aka hyperhidrosis, meaning you're perspiring through socks), ask your podiatrist for a prescription foot antiperspirant. Botox injections can also help, if you can stand them. "They require literally 100 injections per foot, which is not fun," says Dr. Splichal. It can cost up to $1,000 and isn't covered by insurance, but it will keep you dry for three to six months.

| MACRA UPDATE | |
Few Docs Ready for Risk Under MACRA
Under MACRA, providers will use either the Merit-based Incentive Payment System, known as MIPS, or an alternative payment model. Under MIPS, physician payments will be based on a compilation of quality measures and the use of EHRs. HHS will announce the eligible measures, and providers will have some options on which to report. More on how this will work is expected in the final rule, which is likely to be published in November. About 90% of physicians are expected to take this path.
Most providers will choose MIPS because they are not ready to take on the other option, a qualifying alternative payment model that requires a hefty amount of risk. Many don't have the capital to set one up or to risk losing money with subpar performance. Although MIPS requires putting some profits on the line, it is much less of a gamble than heading into an APM without experience and confidence that quality measures are high.

8 Patient Collection Mistakes to Avoid - Part 1
Medical practices are caught in the crosshairs when it comes to collecting patient copays and deductibles. On one hand they are obligated by their contracts with payers to collect patient cost-sharing amounts, on the other, patients are being faced with unprecedented medical bills. If you are caught in this collection free-for-all, here are the eight worst things you can do.

Query: Cyrostar Repair
I purchased a Cryostar unit from a company called Cryotech about 10 years ago. My hand pieces are no longer functioning. I have tried to contact the manufacturer but have not been able to locate them. Does anyone have an address, phone number, or email of someone who could repair the hand pieces?

From: Richard Rees, DPM
By Jove, I think Dr. Hecht has got it! So far, if we follow the government's formula, we've been asked to provide more and more minutia each year which, by and large, has little to do with what we actually do to help patients. We are now assured that the majority of us will be doing our best for our patients for less. Surprise!

RE: ABFAS Fees
From: James J DiResta, DPM, MPH
If members are concerned about the expenditures of an organization, i.e. ABFAS or any other tax exempt organization, you can simply Google their financials and, in this case, ABPS, Inc. financials and CitizenAudit.org will allow you to review the organization’s (501(c) (6) financials and their actual filed IRS tax returns including salaries of the directors, officers, expenditures, etc… this is public information.
The basic filing information remains transparent, but of course the specific line items and as to "why” and the “way” that an organization would chose to spend its membership monies is a board's decision; a board that the membership has chosen to elect and to represent their collective interest. The board has a fiduciary responsibility to spend your money wisely. This is an important reminder to take the time to review just who is running for office and make the effort to vote.

| RESPONSES/COMMENTS (NON-CLINICAL) - PART 3 | |
From: Roody Samimi, DPM
When I decided on this profession, I was told it was a requirement to shadow a podiatrist. And going back to earlier postings, yes -- having patients walk out feeling better is that motivation and satisfaction in this profession. As I told my classmates as a podiatry student, don't get involved in the politics.
But, you may say, you have to make money. I didn't go into podiatry strictly to make money, though I can see how frustrating it is for currently practicing podiatrists, especially those in practice since 1980, who have seen the historical downfall of all medical professions (via insurance companies, from a money-making standpoint) which hit podiatry pretty hard as well, as I cannot deny there are parity issues.
In return, there are podiatrists, like me, who...
