NJ Podiatrist Uses Bio-Engineered Skin Graft to Heal Wound

“A year ago, I cut my leg bumping into some furniture after I tripped,” says Mr. Walter Sugalski. “The scratch became infected and grew to the size of a quarter in three months." Sugalski was initially given antibiotics and compression socks to encourage healing, but was then referred to Marshall Feldman, DPM, Chair of the Department of Podiatric Medicine and Surgery, President of the New Jersey Podiatric Medical Society, and a wound care specialist at the Wound Care Center® at Robert Wood Johnson University Hospital Rahway.

Dr. Marshall Feldman

"After examining Mr. Sugalski, I found he had inadequate vein function, which led to the infection,” says Dr. Feldman. “His wound had been causing him a great deal of pain because it was exposing nerve endings to the open air. After performing wound debridement [removal of unhealthy tissue], we found the tissue underneath would provide an appropriate base for a bio-engineered skin graft.”

AT THE COLLEGES

Barry Professor Co-Authors Article in The Lancet

Dr. Robert Synder, professor and director of Clinical Research for the Paul & Margaret Brand Research Center in Barry’s School of Podiatric Medicine, has co-authored an article detailing the results of a [mid-stage] phase 2b trial for a unique cell-based therapy in venous leg ulcers. The article and findings were recently published in the prestigious medical journal, The Lancet.

Dr. Robert Snyder

The study evaluated HP802-247, an investigational allogeneic living cell bioformulation containing keratinocytes and fibroblasts currently being developed by Healthpoint Biotherapeutics for the treatment of venous leg ulcers. The trial results showed that approximately 70 percent of subjects in the active treatment group receiving the optimal observed dose of HP802-247 achieved complete wound closure at 12 weeks compared with 46 percent in the vehicle control group.

PODIATRISTS AND THE OLYMPICS

UK Podiatrist Part of Olympic Heptathlon Gold Medalist's Team

The lone figure of Jessica Ennis stood on the track at the Olympic Stadium last week as the world’s adulation rang in her ears. Beyond the 80,000 people cheering themselves hoarse in the stadium were the massed ranks of Team Jennis: the coaches, physiotherapists, physiologists, nutritionists, podiatrists, and biomechanists who together built the greatest female athletic body Britain has ever seen.

Karen Duff

Ennis’s physiotherapist, Alison Rose, taught her to walk again following the three fractures in her right foot which forced her to miss the Beijing Olympics in 2008. A podiatrist, Karen Duff, is called in from time to time to specifically examine her feet. Duff graduated as a podiatrist in 1990, and since 1991 has been working specifically with national and international level athletes at sports injury clinics.

FL Podiatrist Admits Hiding Patient Files, Obstructing Investigation

A Delray Beach podiatrist pleaded guilty on Monday in federal court in West Palm Beach to a charge of obstructing a criminal investigation of a healthcare offense. Kenneth D. Poss, 62, admitted that after receiving a subpoena for records as part of a healthcare fraud investigation of his Foot & Ankle Health Center in late July 2010, he told an office manager to remove relevant records concerning patients' comments and complaints that should have been turned over to the FBI and the federal grand jury.

Benjamin, MD, MBA, will help kick off APMA’s 2012 Annual Scientific Meeting in exciting fashion as APMA's guest of honor.

Dr. Regina Benjamin

Ryan: SGR Formula is 'Ridiculous', What's Ahead Could be Worse

Wisconsin Rep. Paul Ryan's selection as the Republican vice presidential nominee has again put Medicare reform in the national spotlight. Ryan has predicted that Congress would approve another temporary postponement of a physician Medicare payment cut in January of around 30% under the sustainable growth-rate formula used to calculate reimbursement.

"I think the SGR is ridiculous and should have been replaced long ago," Ryan said, adding that he hopes to replace the SGR next year with a formula that furnishes doctors with something predictable and accountable so that "we don't have this can-kicking exercise every six months." While the SGR has not worked, Ryan said something worse lurks ahead. “I always say to the doctors, 'If you don't like the SGR, just wait until you see what the IPAB has in store,'” Ryan said.

QUERIES (CLINCAL)

Query: Chronic Charot Foot

I have a patient with Charcot feet who has had a chronic ulcer under the cuboid area plantarly for a couple of years. I have tried most wound care treatment modalities, including off-loading, total contact cast, Dermagraft, Apligraft, bi-lobed rotational flaps, many wound care dressings, wound VAC, and the list goes on. I am considering simply taking the cuboid out of an already collapsed foot, and performing a gastoc/TAL

Chronic Charcot Foot

The patients don't want reconstructions. Does anyone have any experience with simply removing the cuboid to allow the ulcer to heal? In the x-ray, the screws, etc. were put there previously by another surgeon before the Charcot happened. The round metal washer is the lesion marker showing where the ulcer is. Any thoughts would be appreciated.

Query: Source for Hospital Podiatry Forms

Can anybody suggest a source for podiatry forms such as H&P, pre-op orders, recovery room orders, and post-op orders. I am planning on making these as easy to use as possible using the check the box method.

RE: Verruca Treatment in an Immunocompromised Patient
From: Jim Ricketti, DPM

For a compromized patient with verrucae, try debridement of the lesion to pinpoint bleeding and then apply Monocyte EZ Swabs (Pedinol)l and then keep it dry for 24 hours. Then at home, have the patient apply 17% Sal acid to the wart daily for 2 weeks. Repeat the procedure every 2 weeks until the lesion disappears.

Disclaimer:  I invented the Monocyte Swabs, and I am on the clinical advisory committee for Pedinol Pharm.

RE: Verruca Treatment in an Immunocompromised Patient (Charles Morelli, DPM)
From: Elliot Udell, DPM

Dr. Morelli advises that patients should be told that the verruca present, if left untreated for many years, may undergo cancerous changes. Is there literature to support the fact that the kind of verrucae that we treat, if left untreated, will become malignant?
