KS Podiatrist Discusses Bunions
 
"A bunion is a prominence on the first joint on the base of your big toe on your foot," Asim Sayed, DPM, a podiatrist and foot/ankle surgeon from Overland Park, Kansas, explained. In other words, it's a sizable bump that forms when the metatarsal bone begins to shift outwards. Bunions are often caused by genetics, but can be the result of several other factors including tight shoes, overpronation (caused by the way you walk) and ligament laxity of the foot. According to Dr. Sayed, those with flat feet are also more prone to developing bunions than folks with high arches.

Once you have a bunion, you're usually stuck with it — unless you want to undergo surgery. "You can manage them to feel more comfortable, but the only way to eliminate or reverse them is surgical intervention," Sayed said, adding that untreated bunions can lead to arthritis pain.

Return of Ballet Pumps is Bad News for Feet: UK Podiatrist
 
Ballet pumps are making a comeback. Emma McConnachie, who is a practicing podiatrist and Royal College of Podiatry spokesperson, tells HuffPost UK that wearing such shoes repeatedly can cause “calluses, corns, and in some cases, fractures.” Although the latter, she notes, is something she’s only seen happen “a handful of times” in her practice. Ballet pumps tend to have very thin, flat soles, which offer that trademark chic appearance but aren’t necessarily great news for the feet.

The flat soles offer no support and because there’s not usually a strap across the top of the foot, they don’t stay on easily – meaning you alter the shape of your foot to try to keep them from falling off. The base of the shoe also doesn’t cover the entire sole of the foot, resulting in the width of your foot when standing becoming “much wider” than the sole of the pumps, says McConnachie. “These all can cause various issues for the feet both in the short-term and the long-term.”

Practices have been creative during COVID-19 to ensure safe patient access: Telephone visits. Curbside immunizations. Drive-up virus testing. Even checking patients in for their in-office visits while they wait in the car. However, each of these scenarios poses one significant challenge: Collecting copayments and coinsurance. Experts provide these tips to help practices successfully collect patient balances during COVID-19 and beyond:
 
8. Consider settlement rights
 
If the practice makes a reasonable effort to collect from a patient who is experiencing financial hardship (e.g., job loss due to COVID-19), providers may be able to offer a discount (e.g., settle for 70% of the amount owed) without violating Stark Law, says Tinsley. “But remember that just because even if someone doesn’t have a job, they could still have money,” he adds. “There are a lot of people out there with big savings accounts.”

Query: New Rules For Retained Hardware
 
I know things have changed a little for removal of retained hardware, and I am seeking clarification for a scenario. A patient has previously had an Austin Akin bunionectomy. It healed uneventfully. Years later, she has developed pain at the retained screw in the metatarsal. There is a k-wire noted in the proximal phalanx. X-rays showed complete healing at the osteotomies. The patient would like to have both implants removed despite only one of them hurting today. How would the changes in CPT coding apply to this situation?

Response: Removing the painful screw is fairly straightforward from a billing and medical necessity perspective. The question is what is the medical necessity of removing a K-wire which I presume is imbedded in bone? There was no mention of any pain or problem being caused by this wire. Patient preference is not necessarily a medical necessity criterion. Also, removing a buried wire is not always a simple procedure and not without risk. Murphy’s law is what it is.

Kerry Antorveza Becker, 49, beloved mother, wife, sister, and daughter passed peacefully on August 3, 2022, surrounded by her family after fighting a long and courageous battle with breast cancer. Dr. Becker attended The Scholl College of Podiatric Medicine in Chicago, IL, earning her Doctor of Podiatric Medicine.

Dr. Becker then completed a two-year combined podiatric medicine and surgical residency at St. Clare’s Hospital, in New York City. The following year she completed an additional subspecialized 2nd year podiatric surgical residency at the University of Maryland, in 2001. Most recently, she has been a practicing podiatrist for Foot and Ankle Specialists of the Mid Atlantic in Columbia and Clarksville, Maryland.

Dr. Kass makes some good, interesting points regarding certification. However, that does not negate any of my comments made previously which were simply made to provide historical background as to why lifetime certification is no longer accepted in allopathic or podiatric medicine. Limited timed certificates that require recertification are the standard. Indicated in my original article, recertification was established as a method to demonstrate the physician has kept up-to-date (since being originally certified) with the current standards of practice. This is a response to the medical knowledge base which is said to now double every seven years.
 
Lifetime certificates do not provide a method to re-assess the “current” knowledge of the practitioner. Recertification was designed to do that. This has nothing to do with ethics and does not evaluate the actual quality of care but...
