NY Podiatrist Volunteers to Treat Horse Track Workers
According to Nancy Underwood, Backstretch Employee Service Team (B.E.S.T.) Saratoga program director, "We’re currently working on a B.E.S.T. Steps Forward campaign. The goal is to give new work shoes to all backstretch workers who have diabetes-related or chronic foot problems. The program was launched this spring at Belmont Park and in Saratoga... So far more than $10,000 has been given to the program, which will provide workers with high-quality, water-resistant shoes and socks. Backstretch workers quite often have to work in wet conditions, from bathing and cleaning horses."

Dr. Mark Buthorn |
"We treat more than 300 diabetics at our two primary care clinics. Most suffer from severe foot problems as a direct result of their illness. These “diabetes-approved” shoes were selected by Dr. Mark Buthorn, B.E.S.T.’s Belmont-based podiatrist to provide workers with a shoe that will keep their feet dry and avoid irritation from previously existing foot sores," says Underwood.

NJ Podiatrist and All-American Runner Returns Home
Dr. Katy Trotter, one of the most accomplished high school runners to ever come out of New Jersey, has returned to the East Coast to join the practice of podiatrist Dr. John Connors in Little Silver and New York City. While attending Red Bank Regional High School, Trotter was the 2003 Footlocker National Cross Country runner-up. She also finished third at the Footlocker Northeast Regional, ran the 1,600 meters on the distance medley relay team that finished first at the 2003 Penn Relays, and set a national record at the Adidas Outdoor High School Nationals.

HEALTHCARE NEWS |
Uninsured Rate Dips Below 10%
The percentage of uninsured people in the U.S. dropped below 10% this year for the first time, according to a Centers for Disease Control and Prevention survey released Wednesday. The national uninsured rate decreased to 9.2% in the first three months of 2015 from 11.5% a year earlier as 7 million people gained insurance, according to the survey, conducted by the CDC's National Center for Health Statistics.
The reduction is not as big as the drop from 2013 to 2014, when 8.8 million people gained coverage. The total decrease since major parts of the Affordable Care Act went into effect in 2013 is 15.8 million. More people have gotten covered in states that expanded Medicaid under the law and in states that set up their own exchanges instead of relying on an exchange created for them by the federal government, according to the report.

QUERIES (MEDICAL-LEGAL) |
Query: Paying a Nail Tech or Cosmetologist
If you have a pedicure spa in your office, how do you compensate your nail tech or cosmetologist? Do you work on a contract basis or do you pay them as an employee?
Name Withheld
Editor's comment: PM News does not provide legal advice. In general, since nail techs or cosmetologists do not provide medically reimburseable services, the practice is free to pay these individuals as either employees or on a contract basis. Before hiring such individuals, it is wise to check with a healthcare attorney in your state to rule out the possibility that a state law applies in your specific case.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 2 |
From: Daniel Grapel, DPM
I am in full agreement with Dr. Kass that trying to be completely independent of insurance participation is a recipe for disaster. The great majority of podiatrists practice in a very similar way. We all know that our services are elective in nature, and unless someone is clearly superior to all of us, the average patient will go to the provider which costs him the least.
Of course there are always exceptions, but I don't think that we can show the public that it is better to pay the full charge and not a $30 co-pay for coverage that their employer pays huge premiums for. In Queens and the Long Island area, I have seen numerous large high-end specialists and PCPs prefer to be bought by the large local healthcare systems, and they become EMPLOYEES. The paradigm has changed drastically .
Try to talk to any newly-minted physician. They are looking to become the employee and not the boss. The trend is irreversible since the cost of maintaining a practice is astronomical and the mindset of most people is to get the service and pay only the co-pay. Socialized medicine is the clear future, even though I personally detest it. But if the public continues to vote for the political leaders that we have at the present time, we can only blame ourselves for the destruction of the once great medical system.

RESPONSES/COMMENTS (NEW STORIES) - PART 1A |
From: Jeff Root
The role and relationship between podiatrists and pedorthists is anything but clear and simple. First, the difference in education, training, and licensing between podiatrists and pedorthists is significant. A podiatrist cannot practice without a degree in podiatry and a license. To the best of my knowledge, both certified and non-certified pedorthists are able to practice without a license. With no licensing requirement and authority, who oversees their conduct to provide a layer of protection for the public? The difference between certification and licensure is an important point of distinction between podiatrists and pedorthists.
The national, state, and local podiatry associations should not just look at the role of CPeds working under the direction of podiatrists, but should consider...

RESPONSES/COMMENTS (NEWS STORIES) - PART 1B |
From: Stephen Peslar, BSc, DCh
Mr. Kraus wrote, "highly experienced pedorthists are available to podiatric practices on an on-demand basis, either as an "extra set of hands" or to add to the skill-set of the practice. The CPeds work at the direction of the DPM. Positive impact to these hundreds of practices has been considerable." On paper and in an ideal world, this may work, but not in day-to-day practice these days. In the same edition of PM News, Elliot Udell wrote that he received a check for $51 and that was 100% coverage for orthotics. If he utilized a CPed from OHI, would the CPed volunteer his services to Dr. Udell? Perhaps Mr. Kraus could also be a friendly greeter at Eliot's office? At $51 & 100% coverage, not only is Dr Udell volunteering his services, he's also subsidizing the orthotics for his patient's therapeutic benefit.
I've had patients pay for orthotics with a check, take the orthotics with them and stop payment on the check within a few hours. I personally lost money on those orthotics. I've had patients with "100% coverage for orthotics" written in their insurance benefits booklet receive $100 because that was the annual amount of coverage for orthotics in the master contract. Again, if the CPed did the casting would this service be done on a voluntary basis? Does a notarized letter that "neutral plaster casting was performed" from a CPed increase the 100% insurance payment of $51 that Dr. Udell received?
