MEETING NEWS
Podiatric Medicine & Surgery Section of NMA Holds Annual Convention

This year's The National Medical Association (NMA) Annual Convention was held in Manhattan, NYC, August 3-7. NMA is both the nation's oldest and largest non-profit organization, representing African American physicians and health professionals in the United States. The Podiatric Medicine & Surgery section has been the fastest growing section of NMA for over ten years! The Podiatric Medicine & Surgery section's robust program included hands-on surgical skills workshops, a live-streamed surgery, a visit and a roundtable discussion with students from both the New York College of Podiatric Medicine and Temple University School of Podiatric Medicine engaging tomorrow's podiatric physicians with a glimpse into their future.

(L-R) Drs. William Long, DPM, (Trustee, APMA), Adrienne Atkinson-Sneed, (Co-Scientific Chair) and Lawrence Santi, (APMA President)

The hallmark of the program was the great success of the second Annual Limb Preservation Symposium & Town Hall; a multi-disciplinary, forum-based platform, focusing on medical, surgical, and legislative approaches of tackling the disparities of lower extremity amputations in marginalized urban and rural communities nationwide. Experts representing podiatric medicine & surgery, general surgery, vascular surgery, interventional cardiology, emergency medicine, community health & preventive medicine, infectious disease, radiation oncology, endocrinology, and Capitol Hill engagement presented to an engaged and packed audience.

PODIATRISTS AND SPORTS
IN Podiatrist to Swim in the Chicago River Swim For ALS

Paul Sommer, DPM of Valparaiso, IN will be one of only 500 swimmers from around the country and the world on September 22nd to participate in the inaugural racing of the Chicago River Swim. It is a 2 mile race in downtown Chicago to raise money for ALS "Lou Gehrig's Disease" research to find the cause and cure for this terrible neuromuscular disease. Dr. Sommer will represent Indiana and podiatry in this exciting and important swimming event.

Dr. Paul Sommer

The last races to be held on the Chicago River were won by the likes of Johnny Weissmueller, Olympic gold medal swimmer in the 1920s, who went on to play Tarzan in the movies. This race in September will revive the great Chicago River legacy!

PRACTICE MANAGEMENT TIP OF THE DAY
5 Commonly Overlooked Cybersecurity Practices in Fast-Growth Specialty Groups

While there are more than five best practices that specialty provider groups should have in place, the following are commonly overlooked or completed haphazardly.

3. Leverage a recognized cybersecurity framework

Healthcare providers should leverage recognized cybersecurity frameworks like the National Institute of Standards and Technology’s (NIST) Cybersecurity Framework (CSF) and HHS’s 405(d) Health Industry Cybersecurity Practices (HICP), which offer best practices to create strong and resilient cybersecurity practices that are reasonable and appropriate for their organizations, are known to safeguard organizations from the rapidly changing threat landscape, and beneficial for demonstrating conformance with cybersecurity practices with regulators or if subject to litigation.

The Cybersecurity Framework (CSF) 2.0 is designed to help organizations of all sizes and sectors to manage and reduce their cybersecurity risks. The NIST CSF describes desired outcomes intended to be understood by a broad audience, regardless of their cybersecurity expertise.

QUERIES (CODING & BILLING) - PART 1A
From: Michael G. Warshaw, DPM, CPC

In order to appropriately address the posted scenario, it is important to access the 2024 CPT Manual. Under Surgery Guidelines, you need to access the “Foreign Body/Implant Definition.” Foreign Body/Implant Definition:
•  “An object intentionally placed by a physician or other qualified health care professional for any purpose (e.g. diagnostic or therapeutic) is considered an implant. An object that is unintentionally placed (e.g. trauma or ingestion) is considered a foreign body. If an implant (or part thereof) has moved from its original position or is structurally broken and no longer serves its intended purpose or presents a hazard to the patient, it qualifies as a foreign body for coding purposes, unless CPT coding instructions direct otherwise or a specific CPT code exists to describe the removal of that broken/moved implant.”

If Implant Needs to be Removed (i.e. Internal Fixation):
• 20680 Removal of implant; deep (e.g., buried wire, pin, screw, metal band, nail, rod or plate)
If Foreign Body is Removed:
• 28190 Removal of foreign body, foot; subcutaneous
• 28192 Removal of foreign body, foot; deep
• 28193 Removal of foreign body, foot; complicated

Based upon the location and position of the broken K-wire described within the post, the most appropriate CPT code to bill is CPT code 28190 appended by the T2 or the T7 Modifier depending upon whether it was the left 3rd toe, or the right 3rd toe that was involved.

Michael G. Warshaw, DPM, CPC, Mount Dora, FL

QUERIES (CODING & BILLING) - PART 1B
From: Steven Berkey, DPM

According to my understanding of the guidelines, a K-wire that's broken and not performing the function for which it was intended now becomes a foreign body. The appropriate way to bill would be removal of a foreign body (deep) with a -78 modifier.

RESPONSES/COMMENTS (NON-CLINICAL)
From: Vincent Gramuglia, DPM

I am happy that Dr. Rubin brought up this false advertising regarding orthotics. If you ever listen to The PGA station with Michael Breed in the morning on Sirius radio you will have heard the Pine Valley orthotic commercial. I cannot express more emphatically how this ad ruins my day. Owner and “Inventor” Stu Sachowitz even features Michael Breed, who is usually a very measured and thoughtful broadcaster/golf instructor, on the Pine Valley bandwagon. These $99 over-the-counter inserts have the magical power to cure everything from plantar fasciitis to scoliosis and herniated discs. And by the way, they will Instantly cut your handicap In half. Stu says that if he can’t help you, he will gladly refund your $99.

He doesn’t care what size your feet are, whether they are flexible or rigid, pes planus, cavus, vertical talus, club foot... his “orthotics will correct the problem and make you a better golfer!” I don’t understand how companies like this are able to get away with these claims. Is this practicing medicine without a license? Did anyone in Pine Valley wonder if the lower extremity pain could be due to peripheral vascular disease or if the back pain was due to a spinal deformity or neoplasm? These miraculous claims are made without any evidence-based reasoning and are an insult to those of us who base our treatments on a sound history and physical and a knowledge of podiatric biomechanics. I’m sure that if a licensed healthcare provider made such claims, they might at minimum receive a phone call from their state society, or even visit from their medical board.

RESPONSES/COMMENTS (OBITUARIES)
From: Tilden Sokoloff, DPM

Frank was the essence of the Jewish word "mensch." In 1967, I received a telephone call that literally changed my life and Francine's life forever. Frank (Known as Don) along with 3 other stellar East Bay Doctors of Podiatric Medicine called to tell me that I was chosen to be their 1967-68 resident...Wow life-changing for a Philadelphia boy in a world where residency programs were scarce.

They started and literally ran the residency program at Highland General Hospital in Oakland California, a large County Hospital and Trauma Center. Henri L. DuVries was Chief of Foot Surgery. Roger Mann was a 3rd year orthopedic resident. It was no easy feat to...
