NY Podiatrist Discusses How to Choose Shoes for Flat Feet
If you’re one of the 20–30% of Americans with flat feet, meaning that when you stand straight, your arches touch the floor, you might need some more thoughtfully designed shoes. Whether or not your flat feet cause you pain, we can all agree that wearing the right shoes is vital. Slipping your feet into the wrong pair can lead to ankle, knee, hip, back, shoulder, and even neck problems, says Edgard Nau, DPM, a podiatrist at Manhattan Home Podiatric in New York City.
|
Dr. Edgard Nau |
Flat feet also tend to be longer and wider, Dr. Nau says. Nau says to look for a running sneaker with good medial arch support and insoles.

PRACTICE MANAGEMENT TIP OF THE DAY |
Tips for Collecting Patient Balances
Practices have been creative during COVID-19 to ensure safe patient access: Telephone visits. Curbside immunizations. Drive-up virus testing. Even checking patients in for their in-office visits while they wait in the car. However, each of these scenarios poses one significant challenge: Collecting copayments and coinsurance. Experts provide these tips to help practices successfully collect patient balances during COVID-19 and beyond:
7. Keep a credit card on file
This enables practices to simply charge the card once a patient responsibility is assigned, says Denny Flint, chief commercial officer at Millennia, a tech-enabled patient engagement company specializing in the recovery of end-to-end patient financial obligation. However, there are caveats. “If your practice intends to accept card payment as well as store, process, and transmit cardholder data, you need to host your data securely with a Payment Card Industry (PCI)-compliant hosting provider that ensures the data is encrypted, password protected, and compliant with PCI Data Security Standards,” he adds. These standards cover technical aspects of handling and managing cardholder data.

CLINICAL CASE OF THE DAY |
RE; The Case of the Screwy 2nd Toe
This is the toe of an 80-year old lady who basically came to see me for her nails at the insistence of her son. I did routine history and physical and turned her over to my assistant to debride her nails. Next thing I know, my assistant comes and gets me and says, “you gotta see this”.
|
Screw in 2nd toe |
She had no memory of any previous toe surgery-she was somewhat senile. Her son had no knowledge of any previous surgery or any problems with infection or pain—just thick nails. We were all surprised to see this screw in the nail bed. Still don’t know who or when it was put there!

RESPONSES/COMMENTS (NON-CLINICAL) |
From: Jeffrey Kass, DPM
I respectfully disagree with Dr. Kravitz’s definition of board certification. How does certification “help to protect the public”? Does this mean that board certified doctors don’t or can’t commit malpractice? “It informs the public about physicians who have specific training and education in a specific subject area” – what does this mean? Is the board the doctors a P.R. firm? If a doctor wants to inform the public about certain areas of expertise, then let the doctor advertise it. Does being board certified in podiatric surgery indicate to the public that the doctor specializes in flatfoot reconstruction? I think not. That is the doctor’s responsibility of sharing with his colleagues or letting referral sources know what he is capable of.
Podiatry residency has become standardized to a 3-year program. All this training should be enough to “protect the public”. If it is not, then the resident should not graduate. These tests do not test a doctor’s integrity or dexterity in an O.R. They do not test how quickly a doctor can think while a tourniquet is on and a procedure is not going as planned. Board certification unfortunately does not measure what is really important. It has been an unnecessary cost to the doctor. With the advent of a 3-year residency training, we should do away with the boards, or make it an exit exam for podiatry residency programs. If you want to do a 5-year refresher exam of some sort, fine, but don’t tell me the current way these certifications are done do anything to protect the public.
Jeffrey Kass, DPM, Forest Hills, NY
