PODIATRISTS IN THE COMMUNITY - PART 1
Changing of the Guard at OH Podiatry Office
 
Chase R. Ranker, DPM, has purchased the podiatry practice of Dr. Mark S. DeSilvio. DeSilvio will continue to see patients at the office to assist Ranker in the practice as well as to stay connected to his patients. “I just knew it was the right time,” DeSilvio said. By selling the practice, he said it will allow a younger doctor to come in and hopefully serve the community for many years to come. DeSilvio also indicated that Dr. Sandra Witt has retired from the practice.

Dr. Ranker is a graduate of the Ohio College of Podiatric Medicine in Cleveland. He recently finished a three-year residency at St. Vincent Mercy Medical Center in Toledo, where he was trained in both forefoot and rearfoot surgery as well as diabetic wound care and limb salvage. In addition to working at his office, he will also spend time at the Wound Care Center at Wood County Hospital.

IL Podiatrist Joins Danville Polyclinic
 
New and exciting changes continue at the Danville Polyclinic, including new doctors and a convenient online Patient Portal to engage patients more in their healthcare services. Those hired include Dr. Kirk Bott, a podiatrist, who had been in private practice for many years.

Dr. Bott graduated from the Dr. William Scholl College of Podiatric Medicine in 1985. He is past president, Zone 3, Illinois Podiatric Medical Association and is a staff physician with surgical privileges at Hoopeston Community Memorial Hospital.

Court Forbids FDA From Blocking Truthful "Off-Label" Promotion of Drug
 
A federal judge said on Friday that the Food and Drug Administration (FDA) could not prohibit the truthful promotion of a drug for unapproved uses because doing so would violate the protection of free speech. The decision, by a district judge in Manhattan, could inhibit the ability of the FDA to regulate one aspect of pharmaceutical marketing. Pharmaceutical companies have collectively paid billions of dollars in fines in recent years after being accused of marketing drugs for unapproved uses.
 
Lawyers who represent pharmaceutical companies said it was too soon to say how far-reaching the ruling would be, in part because technically it applies only to the district in which it was handed down. Also, while the F.D.A. declined to comment, the lawyers said they expected the agency to appeal. For now, some said, pharmaceutical manufacturers would probably remain cautious about openly promoting their products for unapproved uses.

Walk-in Clinics Force Big Medicine to Rethink
 
When Christine Ryan’s ear was aching one recent afternoon, she didn’t head to the doctor’s office or emergency room; she went to her local CVS store in Cambridge. Within 20 minutes, Ryan had been diagnosed with an ear infection and was picking up medicine and heading back to work. “This was the quickest visit I’ve ever had in my life,” the 24-year-old human resources professional said. Consumers like Ryan increasingly are looking for faster and more convenient options to get basic medical care, and retailers like CVS are filling the gap with walk-in clinics and other services. That’s forcing traditional healthcare providers, from small doctors offices to big hospitals, to react.
 
At Atrius Health, a large medical group, more doctors are leaving their doors open until 8 p.m. Tufts Medical Center is taking online appointments for its emergency room. Several hospital networks are building walk-in clinics for urgent care. Doctors have started seeing patients through video chats. And apps are being built that will let consumers make appointments and view medical information from their phones, the way consumers already access so many other services. “This represents a huge paradigm shift in healthcare,” said Normand E. Deschene, chief executive of Wellforce, the parent company of Tufts Medical Center and Lowell General Hospital. “The systems that are going to succeed are those that are going to embrace it because this is what the consumers want. Most industries follow what their consumers want. Healthcare should be no different.”

Query: Treatment for Unusual Fungi
 
I have had some unusual results with routine KOH and fungal cultures: Yeast  Rhodotorula rubra, mold Geotrichum candidum, yeast Trichomonascus ciferrii, Aspergillus niger. With the population I treat, I don’t think these are just contaminants. Does anyone have any experience with these particular organisms? If so, what medications did you find effective against them.

Query: Unna Boot Coverage Policy
 
Does anyone know where I can find the Medicare guidelines for Unna boot (CPT 29581) coverage. Does anyone have any experience on this subject? I have searched both the CMS and Noridian sites, and only found a couple of paragraphs on the subject that really don't apply. I have a claim that is being denied as "too many services". This patient has had quite a few in the past, but this is the first time they have been denied. I am trying to find out what the limitations are that are warranting this denial.

"You cannot decide to go cash and then expect patients to pay you for what they can get elsewhere for less money. That is simply professional suicide." - Dr. Kornfeld.  Dr. Kornfeld, I would argue that your practice is very different than that of most podiatrists. Your practice involves functional integrative medicine of which many podiatrists know nothing about. Some wonder if you are practicing podiatry (I think you are). 
 
The point is you have a niche practice, do something different, and hence you could expect patients to pay you. I think most of us practicing for the most part do the same things (Perhaps, I am wrong) and this is why I would absolutely agree with the statement you make above. As to Dr. Graziano - I congratulate you for your bold move and hope you continue to prosper. 
 
I still do not believe that in regions where there is a podiatrist on every block or every few blocks that the one who goes out-of-network will prosper. No, unless it's a niche practice. As for having a business plan, I agree that would be a smart idea. I did not take any business courses in college and don't recall them giving any in podiatry school. I agree that private practice is a business, and perhaps we all should have a business plan. And for all who emailed me asking, the dermatologist is in her fifties and she now works on a family farm.
