NY Podiatrist Discusses Foot Cramps
Foot cramps are an involuntary contraction of the muscles in the feet. They occur most often in the arches and toes, and can last for seconds or up to a minute, says Ilya Makarovskiy, DPM, a podiatrist at Staten Island University Hospital.

They have a variety of causes, most of which are not serious. However, if you frequently get foot cramps you should speak with a doctor to rule out serious underlying conditions like liver disease or nerve damage that can make foot cramps worse. "If you're experiencing foot cramps, it's very likely from muscle tightness, fatigue, improper shoes, or dehydration," Dr. Makarovskiy says.

FL Podiatrist Sues Defendants for Misappropriating Funds, Intellectual Property
Jaws Podiatry, Inc., has filed suit in the Southern District of Florida against individual defendants and A&D Creative Marketing LLC. The complaint for injunctive and monetary relief alleges that the defendants unfairly took funds and trade secrets from the plaintiff.
Plaintiff Abraham Wagner, who the complaint says owns Jaws Podiatry, has a unique method for performing minimally invasive and aesthetic foot surgical procedures under local anesthesia, which they have worked to maintain the secrecy of through protected surgeries, photographs, videos, and more. Luxe Foot Surgery, who is a direct competitor of the plaintiff, allegedly worked with the other defendants to steal the medical procedure videos and begin using the plaintiff’s techniques.

Tips for Collecting Patient Balances
Practices have been creative during COVID-19 to ensure safe patient access: Telephone visits. Curbside immunizations. Drive-up virus testing. Even checking patients in for their in-office visits while they wait in the car. However, each of these scenarios poses one significant challenge: Collecting copayments and coinsurance. Experts provide these tips to help practices successfully collect patient balances during COVID-19 and beyond:
6. Employ enough billers or consider outsourcing
Many practices may have laid off staff when patient volumes decreased. However, once volumes resume, they need to make sure they have enough staff for timely billing and collections, says Denny Flint, chief commercial officer at Millennia, a tech-enabled patient engagement company specializing in the recovery of end-to-end patient financial obligation. In some cases, it may make more sense to outsource revenue cycle management (RCM) to a company that is a branded extension of the practice and staff. Working with an external vendor also helps practices avoid internal staffing challenges as patient volumes continue to ebb and flow during a potential second wave of the virus.
Flint provides these tips for negotiating a favorable contract with an RCM vendor:

Require the vendor to charge only a percentage of what they collect.

Ask the vendor to waive its implementation fee.

Demand an easy-out contract. “If the vendor doesn’t perform within six months, you should be able to walk away without cause or penalty.”

Query: Coding a Deep Debridement
A patient has a left distal fibular pressure ulcer of varying depth, measuring 8 cm x 12 cm down to and exposing around 4 cm squared of the fibula and 4 centimeter squared of peroneal tendon. We went to the operating room for formal debridement. The entire ulcer was debrided, including bone, tendon, and subcutaneous tissue.

How do you bill this? The entire location was grafted with Stravix Graft and I applied a negative pressure, wound closure device on top of the debrided area.

Response: With debridement of wounds, there are different tissue depths (codes) to select based upon the type of tissue debrided. Each deeper layer includes debridement of the more superficial layers. So in this case, the proper code to use is CPT 11044 alone due to 4 square centimeters debrided.
The use of Stravix could be an issue as well. Most skin subs (depending on carrier) will allow skin substitutes only after 30 days of failed local conservative care. This needs to be documented in your chart if this occurred, or why it was not. Check with the involved carrier prior to the use of this or any skin sub product. Many carriers also only allow certain products for payment. So they could pay for product A but not for product B.

I am writing to urge your support as well as the PM News readership to help increase the availability of naloxone in the nation’s pharmacies and in the community at-large. Hence acting of the fourth strategy proposed by the National Academy of Science and Engineering in 2017 to “Prevent Harm”. Fentanyl and its analogs are being used as contaminates in imported illicit drugs to include marijuana as well as counterfeit legend medications putting many citizens living in the United States at risk of opioid overdose from this weapon of mass destruction. Marijuana should no longer be referred as a “Gateway Drug but a Pearly Gate Drug”.
For this reason and many more, I believe that additional steps are necessary to ensure that Naloxone, a lifesaving medication, is more widely available. I would like to strongly encourage action to...

Certification is a method to help protect and inform the public about physicians who have specific training and education in a specific subject areas. This requires that recertification be completed on a regular basis to demonstrate the physician is up-to-date on current standards. As an example, the Academy of Physicians in Wound Healing/Council for Medical Education and Testing does not offer a lifetime certificate.
Medicine is a consistently evolving science and what may have been acceptable in previous years changes as the science advances. ABPO had announced for a number years that the cut off year for lifetime certification was going to occur in 1995. Not moving in that direction would have invalidated the entire certification process for the ABPO. These are standards accepted by medicine across-the-board including all aspects of allopathic medicine.
