AT THE COLLEGES
WesternU Names Interim President
 
Gary Gugelchuk, PhD, has agreed to serve as interim president of Western University of Health Sciences following the retirement of Founding President Philip Pumerantz, PhD, WesternU’s Board of Trustees announced. Gugelchuk, a 29-year employee of WesternU who has served as the university’s Provost and Chief Operating Officer since February 2012, will begin serving as interim president following Dr. Pumerantz’s official retirement on September 8, 2015.

Dr. Pumerantz announced his retirement in April of this year, and will serve as President Emeritus after September 8. A national search for his permanent successor is underway. The search is expected to last into 2016.

INTERNATIONAL PODIATRY IN THE NEWS

Nigerian Diabetologist Urges Expansion of Podiatric Services

Currently, there is no national or localized foot care training institute in Nigeria. However, to help bridge the gap and raise awareness on  the subspecialty of podiatry, diabetologist and medical director, Rainbow Specialist Medical Centre, Lekki Phase1, Lagos, Dr. Afokoghene Isiavwe urges government to pay more attention to diabetic foot care.

As part of strategies to increase awareness, the Podiatry Initiative, Rainbow Specialist Medical Centre, Lagos, World Diabetes Foundation, and Podiatry Institute, USA, are organizing  an international workshop on foot care with the theme: “Building Local Capacity in Diabetes Foot Care”. The  conference, August 17-21, 2015, is designed to build local capacity in foot care in Nigeria and improve management and prevention of diabetic foot complications by raising awareness on proper foot care.

UK Podiatrist Discusses Vicks Vaporub and Feet
 
Vicks contains camphor, eucalyptus and menthol in a turpentine oil base. "This type of ointment is very emollient so will trap in any moisture, while the menthol and eucalyptus will be cooling," explains Dr. Tariq Khan. And while there's no evidence that it's particularly good for hard skin, some podiatrists do regularly use Vicks on feet.

"Around seven years ago, someone found that Vicks was effective at tackling fungal nail infections, which are notoriously difficult to clear up. We're not exactly sure why, but the theory is that it contains an ingredient that works on types of fungi that cause these infections," says Dr. Khan.

ICD-10 UPDATE
New ICD-10 Guidance - Part 11
 
Question 12: Will CMS permit state Medicaid agencies to issue interim payments to providers unable to submit a claim using valid, billable ICD-10 codes?
 
Answer 12: Federal matching funding will not be available for provider payments that are not processed through a compliant MMIS and supported by valid, billable ICD-10 codes.
 
Question 13: Will the commercial payers observe the one-year period of claims payment review leniency for ICD-10 codes that are from the appropriate family of codes?
 
Answer 13: The official Guidance only applies to Medicare fee-for-service claims from physician or other practitioner claims billed under the Medicare Fee-for-Service Part B physician fee schedule. Each commercial payer will have to determine whether it will offer similar audit flexibilities.

I believe your letter is based on many assumptions. I have had great success as an out-of-network provider, as many physicians and surgeons of many specialties have. I believe this is one isolated example of a practice failure that could easily be blamed on many potential factors. More information is needed beyond former patient gossip and hearsay to place blame on a poorly executed out-of-network model.  
 
That being said, it is not easy to maintain a successful out-of-network practice. It is a model of practice that requires a great deal of staff training and understanding, exquisite record-keeping, and attention to detail. I'm not sure what the best model is. However, a volume-driven practice, dictated by insurance companies and the federal government, seeing patients on a treadmill is not what I describe as quality care. It's certainly not the care I want as a consumer.

Dr. Kass cites one example of an out-of-network dermatologist who, as a result, had to close her office. For his one example, I can cite a dozen who have done very well being out-of-network. Yes, it is a significant change to your practice style (you work smarter, not harder). Yes, it does require courage (testicular fortitude) to make the change. Do I have any regrets dropping out? Absolutely not. Remember you can see as many patients as you want if you want to see them for nothing. Volume does not equal income generation.  
 
Some in our profession don't believe we have worth and continue to accept what the insurance companies dole out to us. Then they whine and whine about how unfair they are being treated. I'm simply saying you are worth something and you should take a stand to prove it.

Dr. Kass posts a lesson in futility. It is NOT testicular fortitude that makes a cash practice successful (although it is an important ingredient). It is a business plan. The one that he mentions is about as anemic as anything I have ever heard. You cannot decide to go cash and then expect patients to pay you for what they can get elsewhere for less money. That is simply professional suicide. 
 
On the other hand, the way to thrive is to lower the water (your expenses) and raise the bridge (your income). A savvy businessman knows that to succeed, you need to ...

Few podiatrists are going to relinquish the orthotics fee to a certified pedorthist voluntarily. That is what we're talking about, isn't it? With reduced reimbursements from insurers, those fees may make the difference between subsidizing other services or even keeping your podiatry office open. The fight with orthopods was over which professional was the most qualified to render better quality foot care. 
 
I'm not sure what the discussion with the certified pedorthist is even about. After 4 years of podiatry school and a 3 year residency, it seems to me to be a moot point. If a podiatrist finds his or her practice too busy to 'bother' with orthotics, refer this task to a newly-minted podiatrist just starting out. At least they'll keep it in the family.

2. How would you treat a juvenile bunion that displays early pedal expansion, mainly in the forefoot?
