Diabetics Need Comprehensive Annual Foot Exam: NY Podiatrist
 
Diabetic neuropathy and peripheral arterial disease (PAD) are the two complications people with diabetes should be aware of, says Bruce Pinker, DPM, a podiatrist and board-certified foot and ankle surgeon in Nanuet, New York. Both are complications of diabetes that affect the lower legs and feet. "Diabetic neuropathy, or nerve damage in people with diabetes, can be painful and sometimes chronic," he explains. "It usually involves tingling, numbness, or burning in the legs and feet."

Inspect your feet for cuts, scrapes or bruises — pretty much any abnormalities — says Dr. Pinker. That includes checking between your toes for things like corns, ulcers, or athlete's foot. At minimum, Dr. Pinker says anyone with diabetes should visit their podiatrist for a comprehensive annual foot exam. Those with severe cases of diabetic neuropathy or PAD may need to be seen every few months, though.

Volunteer podiatrist John Coleman, DPM treats mostly diabetics with foot problems at MASS Clinic, helping fill the follow-up care void between urgent care and hospitalization. Dr. Coleman recalls a grateful patient whose toes were removed at an emergency room.

“By the time I saw him, the staples were in his foot three months later. They were buried in his skin. No one would see him and he had no insurance,” he says. Coleman removed the staples and provided two months of care until the foot healed. “I still get thank-you cards and thank you emails to this day, probably six years later.”

Query: Achilles Tendon Repair
 
I have a patient who has had 6 months of pain at her Achilles tendon. We obtained an MRI that identified a partial rupture of the left Achilles tendon with a tendinosis noted area with pain. The surgical plan is debridement, application of a synthetic graft to augment the repair, and use of platelet-rich plasma. Any suggestions of coding this would be appreciated?

Response: There are three CPT codes for the repair of a ruptured or torn Achilles tendon:
CPT 27650 Repair, primary, open or percutaneous, ruptured Achilles tendon
CPT 27652 Repair, primary, open or percutaneous, ruptured Achilles tendon; with graft (includes obtaining graft)
CPT 27654 Repair, secondary, Achilles tendon, with or without graft
 
CPT 27650 and CPT 27652 are used for the surgical correction of an acute injury. That is, one that is treated procedurally within the first 2 weeks of the injury. CPT 27654 is used for the surgical correction of a chronic injury. That is, one that is treated procedurally 2 weeks or later following the injury.
 
Since the partial rupture of the Achilles tendon was identified by an MRI and was corrected procedurally following 6 months of pain within the Achilles tendon, the most appropriate CPT code to bill would be CPT 27654. Since the use of a graft is included in the reimbursement of this CPT code, the application of the synthetic graft is not separately reimbursable. Based upon the fact that the use of platelet-rich plasma is not covered by most health insurance carriers including Medicare, the patient would have to pay out-of-pocket.
