MI Podiatrist Discusses Mueller Weiss Syndrome
Another top athlete felled in his prime, succumbing to the rigors of professional sports. So goes the more sensational headlines when the news leaked of Rafael Nadal’s condition. Apparently, the best tennis player in the history of the sport, if judged by the number of Grand Slam tennis tournaments won, had suffered a potentially career-ending malady of his left foot. According to Conway McLean, DPM, "Mueller Weiss syndrome was first described over a century ago. Yet, the specific mechanism remains unknown. Much more common in women than men, it develops generally in middle age, although of course, these are only tendencies. It can cause severe pain but typically does not hurt all the time."

"In Mueller-Weiss, the navicular bone loses its blood supply. When blood flow to the navicular is cut off, it loses its structural integrity and collapses under the compression force exerted by the adjacent bone nearer to the ankle (the talus). The navicular bone flattens or distorts, slowly, over time, altering the articulation between the two bones. This is an important and very functional joint. And these destructive changes generally lead to inflammation and pain over time. Depending on the degree of deformity prior to any modification, the architecture of the foot may be altered. These changes in foot anatomy are always one of succumbing to gravity, a collapsing of the arch. Mueller-Weiss seems to occur more often to a flatter foot type," says Dr. McLean.

Bahamas Podiatrist Discusses Hyperhydrosis
According to Monique Mitchell, DPM, MPH, "Hyperhidrosis is excessive sweating of the feet. It’s slightly more common in men than in women, and more common in young adults than older adults. People whose feet sweat excessively often also have problems with excessive sweating of the palms. Hyperhidrosis can cause great emotional distress and occupational disability for many people. It is estimated to affect 0.6 to one percent of teenagers and as high as 2.8 percent of the population of the United States. Hyperhidrosis most commonly occurs among people aged 25 to 64 years of age – but some may develop it from early childhood or as teenagers.

"Both topical and systemic medications have been used in the treatment of hyperhidrosis. Prescription antiperspirants may be applied directly to the feet and are more effective than over-the-counter versions. Botox injections can temporarily control excessive sweating of the feet with treatment generally lasting for about six to nine months. Severe cases of sweaty feet may be treated with a surgical procedure called a sympathectomy, which interrupts the nerve signals that tell the feet to sweat excessively. It has been reserved as the final treatment option and is not often done."

Minority Shareholders can “Follow the Money” Linked To Alleged Diversions by Majority for Phantom Salaries and Benefits
The minority shareholders of a podiatry practice felt like they had been kicked around by the alleged financial misadventures of two colleagues who together controlled an 80 percent interest. A four-way split of practice expenses was particularly galling to the minority, based on their allegations that practice “expenses” included robust salaries and benefits afforded to family members of the majority shareholders who didn’t work for the practice, and Carolina Hurricanes season tickets reserved for personal use. Plaintiffs Joel Kelly and Elizabeth Bass Daughtry each held ten percent interests in Piedmont Foot Clinic, P.A., and defendants Jason Nolan and Richard Hauser each held 40 percent shares. Piedmont was sold in 2020 to a group of entities that operate a multi-state business.
While the Court acknowledged the general rule that shareholders don’t owe a fiduciary duty to one another, Judge Davis found that Nolan and Hauser owed the minority shareholders a “special duty” that North Carolina requires majority shareholders in a close corporation to afford to minority holders. The Court noted that North Carolina law also allows minority shareholders to pursue asset diversion claims in settings, like here, where more than one shareholder is alleged to have acted together to exercise control to the detriment of minority interests.

Tips for Collecting Patient Balances
Practices have been creative during COVID-19 to ensure safe patient access: Telephone visits. Curbside immunizations. Drive-up virus testing. Even checking patients in for their in-office visits while they wait in the car. However, each of these scenarios poses one significant challenge: Collecting copayments and coinsurance. Experts provide these tips to help practices successfully collect patient balances during COVID-19 and beyond:
5. Continue post-visit collections calls … but with sensitivity
Denny Flint, chief commercial officer at Millennia, a tech-enabled patient engagement company specializing in the recovery of end-to-end patient financial obligation suggests calling patients after the same invoice remains unpaid after two billing cycles to make sure they received it, answer any questions they have and try to connect in some way.
To minimize questions, be sure to send a clear and concise invoice. This includes the service date, location, provider, description of the service rendered, amount insurance paid (including primary and any secondary insurance), and the amount the patient owes, says Flint. Differentiate between copayment and co-insurance amounts, he adds.
However, don’t just call and ask for money, says Sergio Quiej, Texas-based independent revenue cycle consultant. Inquire about the patient’s family and job — even if it takes a few extra minutes. “The way you treat the patient directly impacts how they’ll react to the debt,” he says.

Query: Pre-operative Nerve Block
We are having a debate in our group about billing for a nerve block. The scenario that we are debating is when a patient has a Lapidus bunionectomy. The patient has general anesthesia provided by an anesthesiologist. At the start of the case, the surgeon performs an ankle block for the patient in order to provide them with at least 12 hours of pain relief. In this scenario, can the surgeon bill for a Lapidus bunion and the nerve block or is that bundled into the procedure code?

Response: If the surgeon is performing a nerve block for the procedure prior to surgery, it is considered as part of the operation and is not billable. Now, if the block is performed by the anesthesiologist, they are entitled to bill for the nerve block. Additionally, if the surgeon performs a nerve block at the conclusion of the case to prolong the pain-free period, this is not billable by the surgeon either.

This note is excellent. although distressing to read. Targeted audits are not to be taken lightly and their advice about seeking counsel is spot on. PICA has excellent ADC (administrative defense coverage) in all their policies. Don't go this alone. Get expert assistance. This situation of the targeted audits is ramping up substantially as of late. It is critical to review one's LCD/LCA for all services from the local MAC and make sure the EMR and macros within are complete, comprehensive, and consistent with both the service rendered and the details needed from the LCD.

I can’t blame Dr. Blanken for being sour with ABPM based on the story he shares. However, the whole idea of a lifetime certificate is a farce. If the idea of board certification is that one is being held to some grandiose higher standard, then shouldn’t they have to demonstrate such said level regularly? How can they pass an exam and then be given a “free pass” for the rest of their career. Lifetime certificates should be annulled as void and worthless.
