VA Podiatrist Volunteers and Educates Nurses in Turks and Caicos
Rodney A. Kander, DPM recently returned from providing free podiatry clinics throughout the Turks and Caicos Islands. The Ministry of Health and the Turks and Caicos Diabetic Association were instrumental in facilitating these well attended clinics on the various islands of Grand Turk, Salt Cay, Providenciales, North Caicos, and Middle Caicos.

In addition, Dr. Kander provided Nursing CME training sessions to staff members on each of the islands. He emphasized that they are the first line of healthcare professionals, and it is critical that they utilize the podiatric information provided to educate the patients and continue to observe for any condition that warrant further intervention. The sessions were enthusiastically received by all clinic staff.

While the rise of chunky, arguably "un-cool" shoes might have some dedicated fashion fans up in arms, we're pretty big fans of the trend. Because the thing is, instead of focusing solely on design, many of these so-called "ugly" shoes, incorporate elements like heavily cushioned soles, shock-absorbing features, and elements of arch support that put your foot health first.

Take the dad sneaker trend, for example. One podiatrist told us that dad-style shoes tend to be pretty good for your feet. "They are wide and allow our toes to naturally bend and move. Plus, they are especially ideal if you have bunions or hammertoes,” Casey Ann Pidich, DPM, a New York City-based podiatrist and owner of DoctorGlassSlipper.com said.

Local Marketing Tips for Small Practices
Facebook? Radio ads? Local SEO? Billboards? Email? There are so many options when it comes to marketing your small medical practice that it can be hard to know where to begin. With research confirming that 98% of consumers went online to research local businesses in 2022, focusing on activities that make it easier for potential patients to find you on the Internet is the most logical place to start. Here are some tips for small practices.
Tip 7: Stay Social - As a small local practice, you don’t need to build an enormous following for a social presence to be worthwhile. Networks like Facebook, Instagram, and TikTok offer a vast range of features that allow you to connect specifically with people in your immediate local area and build an engaged community.
Keeping your social channels updated is surprisingly easy. That’s because you can repurpose much of the effort you’ve invested elsewhere to keep your social profiles updated. You could harvest the most compelling Google reviews to use as status updates, crop snippets of text from your website content, and share questions and answers from your Google Business Profile.

My friend and classmate Allen Jacobs, DPM is rarely wrong and has contributed positively to my 47 year career in numerous ways. His pleas to the profession to be more comprehensive with our evaluation and management of our diabetic patients are a series of benign micro aggressions that I believe moved the bar higher and will prove to be clinically important modalities to reduce amputation rates in diabetic patients.
I would add to his neurological recommendations that every podiatrist learn to utilize an 8 megahertz Doppler to LISTEN to the 3 runoff vessels to the foot plus the popliteal artery. This screening exam will find evidence of PAD with the finding of monophasic sounds in these arteries. We should use the Doppler like a cardiologist uses the stethoscope. Sending these patients for LE arterial ultrasounds will give the referring podiatrist pertinent information with respect to stenosis and oftentimes belies the notion that pulse palpability equals adequate blood flow.
If you document a “plus 1” palpable pulse, you are documenting an abnormality that requires further work-up, you are not documenting adequate flow. Thank you, Dr. Jacobs for your 5 decade efforts to educate our beloved profession.

Of course, I acknowledge and agree with Dr. Jacobs regarding the significant limits of the LOPS exam. From my initial response to his letter, I had stated that the LOPS program is a flop simply because it was ineffectual because most diabetic patients would not qualify and the reimbursement is pitiful. What it includes is I also agreed that an E/M code was far more appropriate. The description I am advocating is to use an appropriate E/M. After all, is this exam not an evaluation and management examination?
As I discussed and Dr. Jacobs and the IWGDF have provided, a clear message that all patients with diabetes should be seen by a healthcare professional based on a tiered structure. It is inconceivable that third-party payers and MCR don't see the effectiveness in this message given the significant amount of money paid out for treatments related to diabetic foot pathologies, which eclipse those of many...
