IA Podiatrist Named DMU CPMS Alumna of the Year
Mindi Feilmeier, DPM has been named the 2016 Alumnus of the Year by the Des Moines University College of Podiatric Medicine and Surgery (CPMS). Feilmeier completed her residency at Trinity Regional Medical Center in Fort Dodge, IA. She went on to become an attending staff at Trinity, where she played an active role in the clinical and surgical education of podiatric residents.

In 2012, she brought her experience to DMU’s College of Podiatric Medicine and Surgery, where she serves as assistant professor and a member of the clinical faculty. Feilmeier was named the CPMS Clinical Faculty of the Year for 2015-2016 and 2013-2014. She received the DMU President’s Recognition Award for Outstanding Service in 2014.

OH Podiatrist Provides Valuable Fellowship Opportunities
As she walks through the halls of the Boardman office of Ankle & Foot Care Centers, Dr. Danielle Butto knows that her time at the practice is almost over. A year ago, she joined the podiatry center as a fellow to co-founder Dr. Lawrence DiDomenico, working with patients, assisting with surgeries, and helping with research. “This has been the quickest and best year of my training,” she says. “I’m building on what I learned in residency and taking on more advanced cases to learn how to approach those.”

Later this year, after her fellowship ends, Butto will move on to another job out-of-state, but leaves the door open to return once she feels established in her career. What gives her confidence in returning, should the opportunity and her career path allow, is that earlier fellows such as Dr. Ramy Fahim and Dr. Frank Luckino III returned to the practice as associates. “There’s not a lot of people who can do this work, so being able to learn from [DiDomenico] and then take it outside of Youngstown will be great,” she says.

Wearing High Heels Provides Shaky Foundation: NY Podiatrist
Think of your feet like the foundation of a house, says Dr. Adam Keslonsky, a podiatrist at the NYU Langone Medical Center. "If two houses are built side by side, one with a level foundation and the other with uneven foundation, they'll look identical at first," he says. "As time goes by, the house on the uneven foundation will start to get some cracks and warping. Eventually the doors may not close properly." Yikes.

"At the most simple level, heels don't allow your foot to function properly," Keslonsky says. Wearing heels can lead to joint disease in your foot bones, hammertoe deformities, calluses, bunions, ingrown toenails, neuroma—just about any foot issue you can imagine, he says.

WA Podiatrist Speaks at Summit on Viruses - Liverpool, UK
Over 350 leading research scientists from 45 countries around the world met in Liverpool, England to discuss advances in the study of microbes inhabiting earth’s environments, including human populations. The global antibiotic resistance crisis is increasing with few answers. Randolph C. Fish, DPM of PhageBiotics, Inc. in Olympia, WA gave an invited talk on human use of bacteriophage, viruses against bacteria. “While much research work needs to be done, bacteriophages are not affected by resistance and are, therefore, an excellent tool in the fight against infections and resistance.” His latest paper on human use of bacteriophage is found in the July issue of The Journal of Wound Care.

The initial presentation by Dr. Fish on this topic was presented at the 2015 National Meeting of the Academy of Physicians in Wound Healing (APWH). Dr. Fish will present his follow-up research at the 2016 APWH National Conference in November.

Query: Getting Paid for an E/M and Strapping
When billing for an office E/M visit and strapping (CPT 29540), we are finding that payers are frequently bundling the two together, and paying for the strapping (which is of course the lesser charge). How do we bill correctly to get paid for both?

Response: The established E/M service is a component service included in the "comprehensive" strapping application according to the CCI edits [Medicare]. If, however, you use modifier "-25", you override the CCI edit. You can only used the "-25" modifier when the E/M is "a significant, separately identifiable evaluation and management service." In those cases, you should get paid for both the strapping and the E/M visit. Non-Medicare payers have similar payments, but their own versions of bundling edits.
For example, if you are treating a patient with an established plantar fasciitis diagnosis by the application of a subsequent strapping, the E/M service would be included in the strapping allowance.

It is about time someone spoke up and mentioned the ridiculous antics of the "credential you can trust." The simple fact is that we can pretend to be part of the general medical community all we want, but go to any major hospital/medical system and you will find that no one had heard of the ABFAS/ABPS, and its certification doesn't matter at all for privileges. In fact, they will mention the hundreds of other boards we have diluting our already fragile profession.
Residency places a false sense of identity to our young practitioners as they go from a pseudo sense of equality in the medical world to a reality of isolation and discrimination. Meanwhile, those in charge of the credentialing continue to...

Perhaps instead of changing professions or going to a cash-only office, consider changing your collection policy. We verify insurance on every single patient and know approximately how much their portion will be. Before treatment is rendered, we tell them how much everything will cost and that we expect full payment that day. The vast majority of my patients pay without any issues. Have I had irate patients who refuse? Yes, and I am happy to let them leave. If they will not pay you when they are most wanting treatment, they will not pay you three months later. Additionally, I do not accept payment arrangements.
I practice in a small town in Missouri, I have a non-surgical practice and gross just short of $1M. My monthly average of waiting to be paid is always less than 20 days, and my AR >30 days is always below $10K. I have a traditional podiatry office that I run like the internist's clothing store. Payment truly is expected at time of service and I couldn't be happier.
