MN Podiatrist Elected President of AAPSM

Paul R. Langer, DPM, a sports medicine podiatrist from Minneapolis, MN, was elected President of the American Academy of Podiatric Sports Medicine at their Annual Meeting held in Las Vegas, NV. Dr. Langer received his Doctor of Podiatric Medicine Degree from the Des Moines University in Des Moines, Iowa. In addition to being a Fellow and President of the AAPSM, Dr. Langer serves on the Clinical Advisory Board for the American Running Association and is Clinical Director for Special Olympics Fit Feet in Minnesota. He is also Board Certified with the American Board of Podiatric Orthopaedics and Primary Podiatric Medicine.

QUERIES (CLINICAL)

Query: Staged Procedures for Digital Deformities

This is a 59 year old male who presented to my office about 2 years ago complaining of a painful foot deformity. He suffered a CVA years ago and wears an AFO, but complains of pain predominantly in the digits. He has pre-ulcerative hematomas and lesions in two of the interspaces. He wishes to have the foot corrected. Initially, he will be sent for an arteriogram with runoff to ascertain vascular status coupled with a Doppler evaluation. Assuming he is a candidate, my plan is to stage the procedures to protect vascularity and minimize potential compromise.

Weight-bearing X-Ray

Initially, I plan hammertoe fusions 2-4, most likely using a fusion device such as the Metasurg Digifuse. Next would be decompression osteotomies 2-3-4 on the metatarsals, and finally the fusion of the first MTP with appropriate shortening to maintain the parabola. Obviously, a Hoffman-Clayton type procedure was considered, but I would still stage that as well for safety. Suggestions?

I have had a Hill chair for about 20 years. It is a very basic chair at a reasonable price. I had my concerns originally about some ergonomic minor issues (switch placements and footrest length) that were changed by the company for my chair at my request, and I felt they should have  adopted these changes for all their chairs, and stated it publicly in the past.

Recently a +500lb patient (above the chair-rated capacity) damaged the frame of my chair which,  in turn, ruined a motor, too. This was well beyond any warrantee. The company picked up my chair (I am within 50 miles of their plant) and fixed it in-house and returned it, re-welding the frame and replacing the motor. They refused payment for this. I need to now publicly commend them for their service.

Generally speaking, getting a 90% success rate with an injection, as Dr. Forman writes, would be considered excellent, however to ultrasound everyone and bill for it under these circumstances is an example of overuse. If you feel that you need to get 100% accuracy with every injection and use ultrasound, why make 90% of your patients (and thus the insurance companies) pay for unnecessary services?

If you feel you need to use an ultrasound to give an injection, do so but do not charge the patient or the insurance company, or bill for those where it actually helped, i.e., the 10%. In summary, if you choose to give a shot and need assistance, that is your choice in providing better care, not a requirement, and so ultrasound should be included in the shot charge. As podiatrists, we cannot think, "Oh, I saved the insurance company $2,000 for an MRI" to justify using ultrasound on every patient who requires an injection."

RE: Treating Homeless Patients (Elliot Udell, DPM)

Why not have homeless patients sign a statement that you are giving them treatment for free, and in return, they must promise not to sue for any substandard care or substandard record-keeping? I understand that courts, in general, do not allow patients to waive their right to sue, but if a monetary consideration is given for this, such as free care, then this agreement can be shown to a jury.

The bottom line is if the judge even tells the jury to disregard such a contract, the contract is still in the subconscious minds of the jurors. Perhaps most jurors will see a podiatrist who made such a contract to treat for free as a caring person and tend not to grant a large settlement, even if found negligent.

I was very happy to see the post regarding the attendance of 3rd year students at practice management seminars. Most of them (except the ones sacrificed to the residency crisis) will have FOUR MORE YEARS before they can put the things they heard into practice. I don’t ever remember experiencing a practice management seminar when I was a student, resident, or fellow. With all the negative comments regarding the residency crisis and the plight of the student, at least this is one thing we are doing positively!
