NY Podiatrist Uses Botox to Treat Hyperhydrosis

When Jennifer Pyron, a 27-year-old from New York City, had to stop wearing her favorite summer shoes because they were being ruined by sweat from her toes, she decided to have a cosmetic procedure. “A lot of people have the problem, especially women that want to wear great shoes,” said Dr. Suzanne Levine, a podiatrist at the Institute Beaute in New York City who treated Pyron.

“They [women] don’t want their shoes ruined. It really is quite a problem.” Dr. Levine injected Pyron’s feet with Botox to lessen the sweating. Pyron also found relief in her feet after her Botox injection. “Once it kicked in, I totally noticed a difference,” she said.

AT THE COLLEGES

NYCPM Holds 1st Annual 5K Walk-a-Thon

Irina Abramova, class of 2014, along with other members of the 2014 class, organized a walk-­a-thon to raise money for the Save the Children Foundation. Save the Children Foundation is there to save lives with food, medical care, and education.

The Steps 5K walk-­a-­thon was held on July 15th, 2012 at Flushing Meadow Park in Corona Queens, New York.  The walk-­a-­thon was a huge success. The students raised $2,000, all of which went to the Save the Children Foundation.

At recent ceremonies for the graduating residents from Akron General Medical Center’s post-graduate training programs, orthopaedic surgery chief residents Ryan Gasser, MD and Matthew Stonestreet, MD presented Jordan P. Grossman, DPM, FACFAS with the Golden Apple award, which signifies the “Teacher of the Year” for the Orthopaedic surgery residency program. Dr. Grossman is the President-elect for the American College of Foot and Ankle Surgeons.

New CMS Rules Cut 'Red Tape' - Will Save Money

The CMS issued new financial transaction rules for hospitals and other HIPAA-covered providers Tuesday that federal officials expect will cost up to $2.7 billion over 10 years but save as much as $4.5 billion. The interim final rule, with which providers must comply by Jan. 1, 2014, will establish new operating standards both for electronic fund transfers in healthcare and for describing adjustments to claim payments.

The rule requires insurers to offer a standardized, online enrollment for EFT and electronic remittance advice, or ERA, to encourage more providers to enroll and receive electronic payments from multiple health plans. The rule also requires health plans to send the EFT within a certain number of days of the ERA, to help providers reconcile their accounts more quickly.

The Patient Protection and Affordable Care Act and Centers for Medicare & Medicaid guidelines and rules resulted in a record number of investigations and enforcement actions in 2011.  ACE Medical Risk, a division of ACE USA, has responded through an endorsement that increases limits on Medicare/Medicaid legal expense reimbursements, raising coverage limits from $30,000 to $50,000 per action, with an annual aggregate of $50,000. This product is underwritten by companies within the ACE Group, an A.M. Query: Painful Bunion and Hammertoe

This 69 year old,active female presented with complaints of a painful bunion right foot, and hammertoe 2nd digit, right foot. She related that the pain is pretty constant and affects her ability to wear 'regular' shoes. Changing footgear hasn't helped, so she's wondering what her options are at this point.

Her neurovascular status is within normal limits. Painful crepitus with ROM 1st MTPJ right foot was noted. I am considering a 1st MTPJ hemi-implant based on her x-rays, age, and symptoms. Other perspectives are appreciated.

RE: Oral Steroids and Infection -- How Big Is the Risk?

The following was abstracted from a study "Immediate and Delayed Impact of Oral Glucocorticoid Therapy on Risk of Serious Infection in Older Patients With Rheumatoid Arthritis: A Nested Case-Control Analysis Dixon WG, Abrahamowicz M, Beauchamp ME, et al. via Ann Rheum Dis. 2012;71:1128-1133 via Medcape.

"Although it is common knowledge that glucocorticoid use increases risk for serious infection, this large database study has helped to quantify that risk. Of importance, it has demonstrated that although reducing the dose or stopping therapy does not reduce the risk for infection to that of patients who have not taken glucocorticoids, it does eventually reduce overall risk for infection. Whether the risks are similar in other rheumatic diseases, and what the risks may be for less serious infections (such as upper respiratory tract and lower urinary tract infections), is unknown. Moreover, it is not clear how a reduced risk for infection balances with the control of underlying disease and quality of life that glucocorticoids can offer many patients.

These issues notwithstanding, a take-home point from this study is that even modest doses of glucocorticoids carry real risk for infection that may be ameliorated by reducing the dose and duration of therapy, although it may take a while to see an effective risk reduction."

RE: Walking: Saving Lives: Protecting People, Saving Money Through Prevention

Walking is the most commonly reported physical activity by U.S. adults concluded the Centers for Disease Control and Prevention (CDC) in an early release August 7, 2012. To sustain increases in the prevalence of walking, communities can implement evidence-based strategies such as creating or enhancing access to places for walking or using design and land use policies and practices that emphasize mixed-use communities and pedestrian-friendly streets.

The impact of these strategies on walking should be monitored systematically at the national, state, and local levels. Public health efforts to promote walking as a way to meet physical activity guidelines can help improve the health of U.S. residents. Noting this, the role of podiatric physicians to encourage walking by preventing or treating conditions causing disorders of the pedal extremities cannot be overemphasized.
