MI Podiatrist Discusses the Confusing State of Foot Supports - Part 1
According to Conway McLean, DPM, "Retail footcare products are big business, available in every convenience store and pharmacy in the land. Twelve billion dollars is spent annually on these items and that’s a lot of aching feet. But how often is an over-the-counter approach truly effective? When it comes to arch supports, the answer is both surprising and very complicated."

"Studies have been performed. We’ve examined the effects of custom orthotics as compared to generic supports, but the results are unclear and inconsistent. The difficulty inherent in research into this topic is the nature of prescription foot orthotics: each pair is unique, as is each physician. The benefits of a pair of custom foot supports are largely dependent on the training, experience, education, mindset, etc. of the prescribing healthcare provider," says Dr. McLean.

Barefoot Shoe Can Help Strengthen and Stabilize Feet: IL Podiatrist
“When used appropriately, barefoot shoes can help improve foot stability, activate intrinsic muscles, and enhance balance,” says Peter Lovato, DPM, a podiatrist at Northern Illinois Foot and Ankle Specialists. These shoes can also help strengthen the small stabilizing muscles of your foot and ankle, creating better balance, he adds. Plus, they add a layer between your feet and those of everyone else at the gym—a win for borderline germophobes like me. Just know this: Podiatrists overall prefer running shoes over barefoot shoes for walking or running, given they offer more support and protection for your feet. But that doesn’t mean they don’t appreciate the benefits of barefoot shoes under certain circumstances.

Flexibility is important in a barefoot shoe because it allows the foot to move and adapt to your gait, Dr. Lovato says. However, he warns that flexibility can also mean less structure and support, while Lockwood says that wearing barefoot shoes exclusively could put you at risk for a stress fracture. “Those transitioning into barefoot shoes must do so gradually to allow the foot to adapt,” Lovato adds.

Converting at least some of an old 401(k) to a Roth IRA can offer long-term tax benefits and retirement flexibility, especially if you anticipate being in a higher tax bracket later or wish to leave a tax-free legacy.
4. Roth IRAs offer more than just tax-free growth
Aside from the obvious benefit of tax-free growth and withdrawals in retirement, Roth IRAs come with unique advantages that appeal to both retirees and those focused on legacy planning.
For early retirees or those considering a career break, Roth IRAs offer flexibility. While earnings are subject to a five-year rule and the age 59½ threshold, contributions can be withdrawn at any time, tax- and penalty-free. Roth IRAs can also help reduce taxable income in retirement, which could lower the amount of Social Security benefits subject to tax and help keep Medicare premiums in check.

Query: Surgical Dressings
I provide patients with surgical dressings at the time I perform the procedure. Do I need to see them again in order to refill their orders? Can someone pick them up or can I ship them to the patient on a subsequent date? Also, are there quantity limits and deadlines on what I can ship?
Response from thedoctorline.com: Surgical dressings can only be provided sufficient to care for the patient for 28 days. For liability purposes, my suggestion is to have an in-person exam, for the initial order. As for the exact quantity amount to be dispensed, Medicare expects you to determine from a prospective and not retrospective basis how much is to be dispensed. Thus, if you see patient where 7 units are allowed in week one, then in week two, they expect your documentation to at the least support using the same or lessor amount(s) and certainly not more than the initial week.
Each surgical dressing family has a different maximum number of units allowed per 28-day cycle based on the dressings properties and how it is supplied (oz, cm, yards, etc.). As for when you can order it, you must either have a written note from the patient that they are soon running out or have run out of supplies; an in-person evaluation of the patient (less likely if they were recently seen); or you have had a contemporaneous note based on a phone conversation with the patient concerning their supply limits. Contact with the patient regarding refills cannot occur no sooner than 30 days before the delivery or shipping date.

Daryl J. Saferstein, a devoted family man, respected podiatrist, and cherished friend to many, passed away peacefully on July 31, 2025 at the age of 80.

I was a member of ABLES for almost 10 years. When I started re-certification, I was unable to get in touch with anyone at ABLES. A colleague informed me that it was being absorbed by ABSMP and found that I would have to complete another exam and submit 50 surgical cases as well. The ABSMP requirements were too burdensome. I made the decision to sit for the ABPM exam and was able to achieve Diplomate status in 2024.
This summer, one of my hospitals notified me they would suspend my surgical privileges until I completed board certification through ABFAS. This decision was made by the surgical board based on a letter and position statement from American College of Foot and Ankle Surgeons.

RE: Beware of Insurance Phishing Scam

i wanted to alert PM News to an issue I found in my email. I had it verified that it was a phishing scam. The letter statied that they need verification of a check from an insurance company with a stated amount and a bonafide insurance company that I have submitted claims to in the past.
It had the date of service, my TIN number, but it was lacking the patient's name. They wanted me to fill out the questionnaire to get a check sent to me. After searching through my EMR for 3 years of service, I discovered there was no such claim with that insurance company. The thought process was that this very well could have been from the Change healthcare fiasco. Let this serve as a warning to those receiving similar emails. It looked too good and it was.
