PODIATRISTS IN THE NEWS
Fungal Infection is Most Common Cause of Peeling Feet: NY Podiatrist

With warm weather finally on the horizon, it's almost time to take your feet out of hiding and let them bask in the open-toe-sandal glory of spring. But now that they're out and on display, it might be time to address a problem a lot of us suffer from: peeling skin. "Peeling feet is definitely common, and there are various reasons why," says Rebecca Pruthi, DPM, a doctor of podiatric medicine and owner of Foot Care of Manhattan. "We're on our feet all the time and the constant pounding and walking can cause cracking and scaling of the skin," she says.

Dr. Rebecca Pruthi

According to Dr. Pruthi, the most common reason patients have peeling feet is a fungal infection (like athlete's foot) — although they might not always realize it. "A lot of times it just presents itself as peeling skin and patients don't have the itchiness, so they don't know it's a fungal infection," Pruthi says. So if your skin starts mysteriously peeling all of a sudden, get it checked out sooner rather than later. Once you have a fungal infection on your skin, it can easily infect your toenail, which Pruthi warns can be very difficult to get rid of. "What I find happens is that the skin infects the nails, the nails re-infect the skin, back and forth, back and forth," she says.

PODIATRISTS AND FOOTWEAR
MN Podiatrist Provides Tips on Buying Ankle Boots

Fall isn't quite here yet, but as we inch our way toward September, we'll be suiting up in cozy sweaters, leather jackets, and stylish boots. Look for ankle boots with a low heel. "The higher the heel, the more stress is placed on the knees, hips, back, and forefoot," says Paul Langer, DPM, a sports medicine podiatrist based in Minneapolis, Minnesota.

Dr. Paul Langer

Dr. Langer recommends looking for ankle boots designed with high-quality leather, which is typically softer and more flexible. Boots with zippers, straps, and other closure systems are also recommended for people who have difficult-to-fit feet, Dr. Langer says.

OUTSIDE INTERESTS
NY Podiatrist is Co-Owner of Bar

At Doc Hennigans, it’s perfectly acceptable to take shots and sing along to Don McLean’s “American Pie” on a weekday. Owners Craig Campbell, DPM and Michael DeForest say they are “so grateful” for the support so far. The bar’s name is a nod to both Campbell and DeForest. The Hennigans were DeForest’s grandparents, who kept the bar’s building in the family for generations. “Doc” comes from Dr. Campbell, a podiatrist.

“I have partners who know what they’re doing in terms of the bar end,” Campbell said when asked how he balances his medical practice and the bar. “I love and trust them; it’s been a great run so far.”

RESPONSES/COMMENTS (CLINICAL)
From: Martin M Pressman, DPM, Stephen Musser, DPM, Todd Lamster, DPM

When in doubt, think of gout!

Martin M Pressman, DPM,  Milford, CT

Order labs: CBC/Diff., Uric acid, and CRP.

Stephen Musser, DPM, Cleveland, OH

What is this patient's vascular status? Is he in critical limb ischemia? I would send him for non-invasive vascular examination right away, or just send him for a vascular consultation ASAP. Also, does he have a DVT? Venous U.S. should also be performed to rule this out as well.

He may have  gout. Was any blood work performed to evaluate his uric acid level, ESR, and CRP? Unfortunately, the inflammatory markers may be elevated due to his concurrent UC. Along those lines, he may be suffering with inflammatory arthropathy due to his UC. Is he experiencing a current flare?

What about Charcot arthropathy? You didn't mention it specifically, but was there any non-specific bone marrow edema?  "Arthritic" changes were noted, but the morphology of the foot was not discussed.

Todd Lamster, DPM, Scottsdale, AZ

RESPONSES/COMMENTS (NON-CLINICAL)
From: Adam Siegel, DPM

Seeing as how physicians currently, for the most part, cannot collectively bargain (see US Supreme Court cases Goldfarb v. Va. State Bar as well as Arizona v. Maricopa County Medical Society), how will answering the question you proposed be of any use? Physicians are already hamstrung. We can’t collectively bargain now and we likely won’t later. I’m not sure I get Dr. Rotwein’s point.

RESPONSES/COMMENTS (OBITUARIES)

Dr. Carl Purvis will be greatly missed, by the North Carolina Podiatry Association, and certainly by his family and especially his daughter Jennifer who had been in practice with him. Carl has been a longtime friend and colleague and a past member of the Civic and Kern Hospital Alumni Association. Podiatry needs more doctors like Carl representing this profession. He will be greatly missed.

RESPONSES/COMMENTS (NEWS STORIES) - PART 1A
From: Scott L. Schulman, DPM

An 82 year old unhealthy patient presented with a horribly painful bunion, with an infected ulcer, and asked me to fix the bunion and end his pain.  I found him not to be a surgical candidate and could offer little help other than standard ulcer care and off-loading. He responded, "Why the heck didn't that other guy tell me this could happen and why didn't he fix it 10 years ago?

Bunions are hereditary, PROGRESSIVE deformities that WILL in most cases get worse. I do not think it is "Just cosmetic." Doctors don't just treat pain, they treat conditions and potential problems. High blood pressure does not hurt. Neither does melanoma or even dental cavities, but we treat these conditions to PREVENT bigger problems down the road. How many bunions end up causing bigger problems later?  I think we owe it to our patients to at least educate them on the progression and likelihood of bigger issues, increased pain, and more extensive surgery down the road if left untreated.

RESPONSES/COMMENTS (NEWS STORIES) - PART 1B
From: Elliot Udell, DPM

When I entered practice, over 35 years ago, I had two offices. One was in a suburban area in Long Island and the other was in a specific Chassidic community in Brooklyn. I found the patients’ reaction to bunions, hammertoes, and other foot deformities to be radically different in each community. I found that on Long Island, if a forefoot deformity was slightly painful or deemed "ugly", most but not all of my patients were more inclined to seek surgical corrections. In Brooklyn, where religious life and raising large families consumed family members from dawn to dusk, unless the deformity was extremely painful to the point of being incapacitating, or interfered with other aspects of their life, most but not all of my patients optioned for conservative management of their deformities.
