HOSPITAL PODIATRISTS IN THE NEWS
GA Podiatrist is First to Use Hospital Timeshare Space 
 
Polk Medical Center announced the first – and hopefully not the last – physician to take on a contract for time-shared space in a renovated portion of the medical office building designed for just that use. Dr. Stephen Lapointe, a podiatrist out of Rome, GA is now working out of the timeshare space on Tuesday afternoons and Friday mornings, and according to hospital administrator Matt Gorman, is also performing surgeries.

“We’re excited to have Dr. Lapointe working down here, and we look forward to having more physicians take advantage of the space and keep patients down here in Polk,” Gorman said.

PODIATRISTS IN HISTORY
Early 1900s MN Podiatrist Profiled in Museum Exhibit
 
The Winona County Historical Society will present a history of the many people who call Winona their home with an exhibit entitled “We Are Winona: The Cultures of Our County.” The exhibit profiles community members from diverse backgrounds, past and present. While some of the profiles reveal cultural tensions, others tell the tale of success. For example, Dr. Emma Shelton, an African-American woman, came to Winona in 1909, opened her own parlor and went on to study podiatry in Chicago.

“She was pretty successful in the early 1900s in Winona,” said Jennifer Weaver, Assistant Director of the Winona County Historical Society. “A lot of the stories surround her helping the poor. She wouldn’t necessarily give them money, but she would send them out to a cafe or a restaurant and she would actually pick up the tab for their meal.”

ICD-10 UPDATE

New ICD-10 Guidance - Part 9

Question 10: If a Medicare paid claim is crossed over to Medicaid for a dual-eligible beneficiary, is Medicaid required to pay the claim?
 
Answer 10: State Medicaid programs are required to process submitted claims that include ICD-10 codes for services furnished on or after October 1 in a timely manner. Claims processing verifies that the individual is eligible, the claimed service is covered, and that all administrative requirements for a Medicaid claim have been met. If these tests are met, payment can be made, taking into account the amount paid or payable by Medicare. Consistent with those processes, Medicaid can deny claims based on system edits that indicate that a diagnosis code is not valid.

5. Patient balance "trio pack"
 
Historically, patient revenue has been fairly minor compared to insurance revenue. So if collections slipped, it was not going to put you out of business. Now that patient financial responsibilities are in the stratosphere, they represent real money and need renewed focus. Assess the status of patient receivables using a trio of reports:
 
Patient A/R. Generate this by patient account in descending balance order, not alphabetically, so that the biggest balances will be on page one. Ask for a status update on the top 10 accounts, every month. Train staff to call patients, not just send statements and letters, and establish payment plans using automated, recurring, payment-plan software. Be sure front-desk staff ask for past-due balances when patients come in for their appointment.
 
Payment plan status. Be aware of those patients who are current and who are not. Train staff how to speak with and assist the non-compliant patients.
 
Accounts recommended for collection or bad debt. Some patients simply can't or won't pay. Keeping them on the books for years is foolish financial management and frustrating for staff. Ask staff to prepare their recommendations on which patient accounts should be sent to collections, and ask them to provide back-up details. Approve the accounts for bad debt or send them to collections, but get them out of the A/R.

Response: Since the question is, what will Medicare cover for routine foot care, the answer is found in your MAC's "Future Local Coverage Determination" for routine foot care. The required ICD-10 codes can be found there. They will give you probably several listings of ICD-10 codes based on the various CPT codes.

When Dr. Steinberg concludes, "Why would any podiatrist need the services of someone who could be only a high school graduate with 2-3 weeks of formal pedorthic education, unless the DPM does not feel competent?" it reminds me of our own history of latent discrimination.  
 
It's like deja vu, as we were judged by allopathic physicians as" lesser than" because our education and credentials were misunderstood. I remember when we had to perform surgery in our offices because hospitals would only "admit us if we were sick." I remember limitation of hospital privileges with supervision of a general surgeon observing our procedures. Thankfully, today we are the chiefs of staff of hospitals, equal partners in orthopedic groups, and have admitting privileges in all of our major hospitals. 
 
We need to know when to collaborate and refer to others that have better skills and expertise than ourselves, whether it be a pedorthist, physical therapist, endocrinologist, physiatrist or chiropractor. Most of us know our limitations and skill sets and cannot be an expert in all aspects of podiatric medicine. 
 
So, as Dr. Steinberg posits in 'the best interest of professional podiatric patient care' we need to "get over" being omniscient (and remember our history of unfair discrimination) and collaborate, collaborate, collaborate...and your patients will appreciate you for your referral and come back to you for what you do best.

Reconstructive Foot and Ankle Surgery - This fellowship is a one-year experience during which the Fellow will develop a rational approach to revisional and reconstructive foot and ankle surgery. The Fellow will function as a Specialist and participate in the teaching of Podiatric medical students and residents, and have ample opportunity to be involved in clinical and basic research projects. The fellowship will provide the Podiatric Surgeon, further expertise in Charcot reconstruction, trauma and deformity correction.
