Symptoms in Foot Often Indicate Circulatory or Neurological Problem: MA Podiatrist 
 
After a routine ankle sprain, Jessica Seiden went to her doctor, concerned about her symptoms. "I was feeling numbness and coldness starting in my toes and slowly moving up my leg," said Seiden. Just a foot problem, right? Not so fast says Dr. Jordana Szpiro of Boston Common Podiatry. "There can be other disease processes in the body that can manifest or show up first in the foot," said Szpiro.

Together, the left and right feet hold a quarter of the body's bones and countless nerves and blood vessels that link directly to the heart, spine and brain. "If blood vessels are going to die off, they start first in the foot," said Szpiro. "If they're very, very cold, that could be a circulatory problem or a nerve problem."

MEDICAL EDUCATION

Nearly One in Five Medical Students Reports Being Mistreatred

About 17 to 20 percent of medical students report that they have experienced mistreatment, according to the Graduation Questionnaire (GQ) of the Association of American Medical Colleges. The AMA is taking action to address this problem at its roots and improve the medical education learning environment.

At its Annual Meeting in Chicago in June, the AMA reviewed an informational report on the medical education learning environment that calls for developing a working definition of medical student and resident mistreatment; identifying tools that can ascertain whether mistreatment is occurring, and developing strategies to prevent and address mistreatment.

Query: Recalcitrant Wound

A 42 year old man in good health fell off a ladder in February 2009 and broke his left os calcis. Under the care of an orthopedic surgeon, an open reduction internal fixation was done in April 2009. There were wound healing issues, hence he was brought to the OR in October 2010 for debridement, sequestrectomy, and bone grafting to his left calcaneus. Subsequently, he ended up at a plastic surgeon for a persistent wound that failed to heal.

A gallium scan was negative. An arterial Doppler examination was normal. VAC therapy failed to...

QUERIES (MEDICAL/LEGAL)
Query: Prescription Data and HIPAA  
 
This morning my pharmaceutical rep told me that the reason why she stopped detailing me is that the company policy is that they will only come to the top prescribers of their pharmaceuticals. She told me that the pharmacies let them know exactly how many prescriptions each doctor is writing for their products. Isn't this a violation of confidentiality? Shouldn't this be a violation of the HIPAA laws?

RE: Hallux Extensus in a 2 Year Old (Arthur Gudeon, DPM)

If a 2 year old has hallux extensus, and so does the mother, a possible anatomic, genetic correlation is screaming out to be found. The child (and possibly the mother) should be examined and imaged for spina bifida, cauda equina, or spondylolysthesis, etc. with nerve compression involvement in the S1,2, tibial nerve distribution of the flexor hallucis (which is apparently weak) allowing for the extensus,and secondary to EHL overpowering.  After the correct diagnois is made, then you can consider treatment options.
 
If the podiatrist is uncomfortable with this exam, a referral to a pediatric neurologist or pediatric orthopedist would be a good idea.

RE: Medial Malleolus Screw Removal (Gary Bjarnason, DPM)

Make a stab incision near the head of the screw with a #11 blade (no more than 1/4 inch).  Clean any bone debris out of the screw head with a  #64 blade or micro-curette, if necessary.  Use an appropriate hex head screwdriver  -  counter clockwise twist (rightie - tightie; lefty - loosie).  One stitch should take less than five minutes. Done!

RE: 93% Error Rate for Therapeutic Shoes (Jeffrey A. Szczepanski, DPM)

There is no doubt that the current documentation process is flawed. Orthotists have, for the most part, abandoned this program, and many pedorthists are doing the same. Podiatrists do have a one-up on these two groups because we are able to provide the lower extremity examination which is required. The single most difficult process we have is to obtain some ”acceptable” medical record from the MD to prove that the patient is diabetic. Certainly, a consult note from the MD limited to  the patient’s diabetes status should be sufficient to resolve the issues which the DME MAC still feels is insufficient.
 
The Thompson Reuters study is correct, thus podiatrists should not abandon this worthwhile program. Instead, we should be pressuring the DME MAC’s and CMS to make the documentation process easier. They must clearly understand that if this program is abandoned by the physicians most intimately associated with foot care, it's going to cost the taxpayers far more than any expenditure on shoes/inserts would ever cost.

I would be remiss if I did not comment on the retirement of Carol Jensen. I have had the opportunity and pleasure of working with Carol since I became a residency director 21 years ago. Carol has been a steadying force in the evolution of residency education, from having no centralized interview locations or application matching process to the mammoth system that we have today. Carol has been the glue that has held this component of resident education together.
 
I am sure that other program directors and former residents alike would echo my sentiments in wishing Carol all the best in retirement and a warm thank you for "being there" for all of these years.
