AZ Podiatrist Recommends Foot Strengthening Exercises

How your feet function directly influences everything above them. “You could really generalize it as causing postural fatigue,” said Emily Splichal, DPM, a functional podiatrist in Chandler, Arizona. “When your feet aren’t working optimally, people just can’t stand as long as they used to or move as they would like to. Their foot fatigue limits their ability to do certain activities, like hikes or even walking the dog.”

While she acknowledged research showing that minimalist footwear can strengthen foot muscles and improve toe flexor strength, she doesn’t view shoes as “the only or best way to strengthen the foot.” Instead, she advocates for intentional foot exercises combined with consistent recovery practices.

VA Podiatrist Discusses Diabetic Limb Salvage Inequities

Ben Pearl, DPM on his podcast, Fit Foot U, led a discussion of diabetic limb salvage inequities for patients of color. On the panel were Dr. Pearls's former classmate Earnest Simms Mawusi, DPM along with former Buffalo Bill, Bernard Ford. Ford is currently working in the wound care space. Drs. Mawusi and Pearl have both developed products for podiatry. Dr. Mawusi developed an all natural pain cream known by his patients as “the bomb” and Pearl developed a turf toe splint with a bracing company. Here is a clip from the podcast:

The discussion then turned to compassion in healthcare. Drs. Mawusi and Pearl both agreed that the top 20% academically in their class did not always become the most effective and compassionate physicians. Emotional intelligence is not measured on a test. Ford and Dr. Mawusi then cited examples of patients of color who were not offered the same advanced healing options for their wounds despite having the same insurance coverage. The hope is that forums like those will bring enlightenment to the plight of some patients who are overlooked.

Every Step Counts: Are 10,000 Necessary?
 
A comprehensive review and meta-analysis published in The Lancet Public Health reveals that walking 7,000 steps daily provides significant health benefits, comparable with the traditionally recommended 10,000 paces. Led by Katherine Owen, PhD, the study analyzed 57 studies, focusing on nine health outcomes: including all-cause mortality, the incidence of deaths from cardiovascular disease, dementia, cancer incidence and mortality, the incidence of type 2 diabetes, depressive symptoms, and falls. 
 
Results showed that 7,000 steps per day reduced all-cause mortality by 47% and decreased risks for cardiovascular disease, cancer, type 2 diabetes, dementia, depression, and falls. The study emphasizes that while any physical activity is beneficial, 7,000 steps daily is optimal for health improvements. Current guidelines for physical activity recommend 150 minutes of moderate-to-vigorous physical activity weekly but lack step targets. Researchers suggest incorporating step goals into future guidelines.
 
The Takeaway: Achieving 7,000 steps a day optimizes health by reducing both mortality and chronic disease risk.

Converting at least some of an old 401(k) to a Roth IRA can offer long-term tax benefits and retirement flexibility, especially if you anticipate being in a higher tax bracket later or wish to leave a tax-free legacy.

3. Watch for timing issues with RMDs

If you're age 73 or older and already subject to required minimum distributions (RMDs), there's an important rule to follow: You must take your RMD before converting a traditional IRA or rolling a traditional 401(k) into a Roth IRA.
 
RMDs themselves cannot be converted to a Roth, and failing to take one first could lead to IRS penalties.
 
This rule doesn't apply to Roth 401(k)s, which aren't subject to RMDs during the owner's lifetime.

About seven years ago, I received a series of 5 Synvisc injections in my knee. My knee pain was reduced by 80%. This allowed me to ski. The following year, I received Synvisc One. It reduced my pain by 99%. I then received Synvisc One twice a year for three more years, until it no longer helped. I went on to have a Stryker Triathlon knee replacement, eliminating 100% of the pain. 
 
Perhaps Synvisc One would work better in the ankle.

Just a short note to correct Dr. Gottlieb and his erroneous conception of what Caribbean medical schools are and their standards. Caribbean schools are not driven by academic institutions that offer an MD degree once tuition is paid. There is no wink-wink, slap on the back, and transfer of cash under the table to get an MD degree. A student goes to classes for two years and then sits for step 1 USMLE boards. Even podiatry school uses that format. In order to sit for USMLE step 1, a DPM graduate would have to repeat the first two years of medical school or the school could forfeit accreditation which allows Caribbean students to sit for USMLE, all parts. I don’t know how many times it has to be repeated but just because some DPMs think our basic science curriculum is equal to an MD curriculum, doesn’t  make it so. The Caribbean schools must publish statistics concerning their pass rates for students taking Step 1 who attend the first two years of classes in the Caribbean.

Congratulations to Dr. Tomczak for his excellent article in the current Podiatry Management Issue. This is an eye-opener for sure, and it is one to elicit lots of questions beyond what I present here today. It does answer a great deal of questions I have long had, especially on the issue of GPA and MCAT scores, highlighting the not so difference in scores. And really does a 3.4 vs 3.6 or even 3.2 vs 3.6 really dictate the ability for the lower GPA students to grasp the basic science materials within the first two years of MD/DO/DPM school?
 
How does one differentiate those scores from within a diverse population of students at easier vs. more difficult undergraduate institutions? How does one differentiate those who took the basic undergrad science and math courses only and then majored in less demanding disciplines vs those who were math, physics, chemistry, etc. majors? Are those math science majors with 3.2 really smarter and better able to handle the work than those with other degrees with a 4.0, or just the opposite?
 
If their course work in the essential disciplines were the same, what does...
