FL Podiatrist Gives Commencement Speech at UCF
 
Gideon J. Lewis, DPM was the keynote speaker at the University of Central Florida (UCF) for the Summer Commencement Program. As a decorated UCF alumnus, College of Medicine faculty member, and UCF Foundation Board member, he stays very active in academics. Dr. Lewis is an assistant professor with the UCF College of Medicine, an interviewer for the college’s admissions committee and serves as the chief sports medicine faculty advisor for the college’s orthopedic and sports medicine interest group.

In 2010, he received the College of Medicine’s inaugural Rising Star Award and in 2018 received the College’s Dean’s Award, which is the highest honor awarded to a faculty member in the college. Dr. Lewis shared with the audience that the most valuable thing in life is relationships. MD Podiatrists Published in Anti-Cancer Research Journal
 
Three Foot & Ankle Specialists of the Mid-Atlantic (FASMA), Drs. Caitlin Madden. Adam Spector, and Gene Mirkin, and three pathologists, Drs. Sarwat Siddiqui, Joon Yim, and Xingpei Hao collaborated on the research paper entitled, Dermatofibrosarcoma Protuberans on Adult Toes: A Case Report and Review of the Literature.

The paper was published in this month's Anticancer Research Journal. The case report involved a patient who was treated in the FASMA Wheaton division office, with surgical excision performed in its in-office Joint Commission-accredited ambulatory surgical center. FASMA employs its own pathologists, enabling a prompt diagnosis of the tumor. The article includes a literature review of dermatofibromas, which rarely occur in the foot, and emphasizes the importance of obtaining an accurate histopathological diagnosis.

VA Podiatrist Promotes Podiatry as a Career at Running Camp
 
Ben Pearl, DPM  spent last week providing medical coverage education and coaching at a running camp at SUNY New Paltz, NY. Dr. Pearl discussed how to prevent the most common running injuries. He also discussed podiatry as a career.

High on the students’ minds was the cost of education in a medical career. Pearl highlighted the high match ratios for podiatry residencies as opposed to the more challenging numbers for allopathic medicine. He also emphasized that a career track in podiatry typically finishes after seven years post-college compared to a longer time frame for other medical specialties.

Off-Label Use: Patient Safety Implications - Part 2

The patient safety implications for physicians in prescribing a medication or using a medical device in an off-label manner include the following:

Educate the patient about the medication or product, the proposed off-label use, and the use for which the medication or product received FDA approval.

Obtain the patient’s written consent for off-label use. Use consent forms for off-label use for prescription drugs and medical devices. Note that the key points in patient education and consent include the following:
  • A statement that the off-label use is an “experimental use,” so all the possible side effects and complications are not known.
  • A list of the common and/or known risks and complications with use of the medication.
  • A statement that the details of the treatment have been fully explained in terms that are easily understood by the patient.
  • A statement that no guarantees about the results of the medication are given.
  • A statement that the patient is not required to take the medication.
Contact the Underwriting Department of your malpractice company to verify coverage of the proposed use of the medication prior to the planned use. Follow up with the patient on a regular basis to monitor for any adverse or unknown reactions to the medication.

RE: The Passing of Carlton G. Purvis, DPM
 
Carlton G. Purvis, DPM, age 71, passed away peacefully at his home Saturday morning, August 3, 2019. Dr. Purvis received his Doctorate of Podiatric Medicine from the Ohio College of Podiatric Medicine. He completed his externship and residency and moved to Rocky Mount, NC., and opened his podiatric practice in 1976.

Purvis-Moyer Foot & Ankle Center continues his tradition of excellence with his business partner and friend Dr. Peter Moyer, his daughter Dr. Jennifer Purvis, and his exceptional office staff.

QUERIES (CLINICAL)
Query: Unilateral Edema, Redness, and Pain 
 
A 65 year male with a PMH of neuropathy, Stage 3 chronic renal failure, thyroid goiter, and ulcerative colitis presented with 2-3 months of swelling, redness, and pain to the entire left foot from the toes to the ankle. There is no allodynia or hyperesthesia. Calor, edema, and redness are worse at the 3rd met head, spreading to the 2nd and 4th met heads. His pain is concentrated in those areas.  
 
Yet, x-rays and MRI only show severe arthritic changes to the 1st MTPJ, while the areas of pain only revealed edema to the dorsum soft tissue. There are no soft tissue lesions or bony lesions. I thought it was unusual to have worse pain, warmth, redness, and swelling away from the arthritic area if the arthritic area was the cause. I told him to compress and elevate the foot. Any other thoughts?

Dr. Herbert's point is well taken. The term "entitlement" refers to a right granted to an entity by law or custom. In most instances, it is taken for granted by law or custom that an individual is entitled to something that he has actually EARNED, as you and the rest of us have. However, there are numerous examples of rights granted by law or custom that have truly been granted and not actually earned. Some of these are rooted in good, such as taking care of widows, orphans, the blind, or crippled. As we know, many of these grants have evolved into giveaways, usually to curry political favor. Our esteemed political class, followed by a complicit press, has purposely blurred the lines between what has been earned compared to what has been given, just to keep stirring the pot.

Dr. LaFata is correct when he says that "unsightly bunions all have something in common: They're a pain." They are not only a pain physically, but also emotionally and cosmetically as well which should not be discounted. A new report from the American Society of Plastic Surgeons reveals that Americans spent more than $16.5 billion on cosmetic "plastic" surgery, and minimally invasive procedures in 2018. Why should podiatry be any different?
 
Many of us have had countless patients who present to our offices with bunion concerns that are not always painful. These are mostly young women and some men who are socially embarrassed by the appearance of their feet including both bunions and hammertoes, not to mention nail infections. There is a place for cosmetic foot procedures in this profession and to make a blanket statement to a patient that s/he should not have surgery due to the lack of pain is short sighted. Explain the risks of such a procedure including pain, swelling, and scarring, and let the patient be the ultimate arbiter. Refer them to a surgical colleague and allow them to be treated.
