CO Podiatrist Refers Diabetic Patient For Lace-Up Gauntlet
 
During an appointment with Anna Weber, DPM, at the Diabetic Foot & Wound Center, Denver, she discussed the neuropathy and pain that Mike Woodring was experiencing in his feet. Dr. Weber noticed that his right foot rolls under when he walks. Weber referred him to Creative Technology Orthotic & Prosthetic Solutions, Denver, where he saw Lara Schrock, CPO, who casted and fitted Woodring with the ankle gauntlet. The lace-up gauntlet is made of cream-colored leather over a plastic inner shell; it extends part way up his calf and encases half of his foot. He wears it during his workday, about eight hours per day, three to four days per week, as prescribed, he says.

An added benefit of the gauntlet is that although it was prescribed to prevent Woodring's foot from rolling under due to his cerebral palsy, it also alleviates pain caused by the neuropathy and a bone fragment from a long-ago break—something Dr. Weber discovered when she examined an x-ray of his foot.

Millcreek Hospital Boot Camp Celebrates Another Successful Year

Boot camp week was another success this year! Thanks to the six remarkable medical students from Kent State University College of Podiatric Medicine and Des Moines University College of Podiatric Medicine and Surgery. All of the students are incoming second year students and spent the week in Erie learning from Millcreek’s podiatric team.

From a wound dressing workshop, to local anesthetic injections and even two sawbone labs, the team at Millcreek prepared an academic schedule to aid the students in their upcoming studies. Having a good time was not forgotten while at the local Erie Seawolves baseball game.

Soon, Brookings can boast its own full-time podiatrist, one who convinced Avera Medical Group, Brookings, a department of Brookings Health System, that a podiatrist was a necessary part of healthcare in the community.

IAI Holds Its Centennial Conference

The IAI (International Association for Identification) held its centennial conference August 3-7, 2015 in Sacramento, CA. The five-day educational conference included two days of forensic podiatry and gait presentations and workshops given by British and American forensic podiatrists and gait  specialists.

New ICD-10 Guidance - Part 8

Question 8: Are technical component (TC) only and global claims included in this same CMS/AMA guidance because they are paid under the Part B physician fee schedule?
 
Answer 8: Yes, all services paid under the Medicare Fee-for-Service Part B physician fee schedule are covered by the guidance.
 
Question 9: Do the ICD-10 audit and quality program flexibilities extend to Medicare fee-for-service prior authorization requests?
 
Answer 9: No, the audit and quality program flexibilities only pertain to post-payment reviews. ICD-10 codes with the correct level of specificity will be required for pre-payment reviews and prior authorization requests.

Five Financial Reports Physicians Need to See Monthly - Part 4

4. Credit balances
 
Credit balances are a financial liability. If they are accurate, you must pay them back to patients or insurers. A six-doctor group thought they could avoid this "unless someone asked," so they did not run a credit balance report for years. When they finally did, it was 65 pages long and had about $74,000 in credit balances — all of which had to be researched, verified, and refunded.
 
Review and approve the credit balances that staff have researched and verified every month. This report should be as close to zero dollars as possible.

QUERIES (MEDICAL-LEGAL)
Query: Obligation to Find Specialist Who Will Accept Insurance
 
A patient came into our office today with an ulceration. I want him to have a complete PVD work-up, but none of the vascular doctors in my area will take his particular insurance plan. We have called around without success. From a medical-legal point of view, how much does my office have to invest in time and effort to find that "pin in a haystack" doctor who will accept this particular patient's insurance. The insurance company won't help him either.

Dr. McGuire's comments are biased, and he is surely no more expert than podiatric colleagues who understand and apply biomechanics to podiatric conditions! Why would any podiatrist need the services of someone who could be only a high school graduate with 2-3 weeks of formal pedorthic education, unless the DPM does not feel competent? If the DPM can't get it right, how will someone with the most minimal of education, be able to do it better? I am not referring to just off-loading diabetic insoles, but the vast field of podiatric biomechanics applied to a solid podiatric sports medicine practice. Maybe a podiatrist who is not an athlete, him- or herself, has trouble applying functional orthotic therapy. But if that is the case, why not refer the patient to a podiatric colleague? Why not support our profession? 
 
What service would the CPed provide which would allow the DPM to pursue more "lucrative" services? I believe the scenario created by Dr. McGuire's rationalizations is going to lead to over-utilization, and is not in the best interest of professional podiatric patient care.
