Tweaking Your Footwear Can Prevent Many Foot Problems: NY Podiatrist

Your feet are two tough tootsies. They spend their days shoved into stilettos, pounding pavements, and supporting your weight while you bounce around on the treadmill or take a Zumba class. It's no wonder that one out of six people in this country complain of foot problems.

Canadian Podiatrist Discusses Athlete's Foot

This foot rash is the most common fungal skin infection. It's caused by a fungus that grows on or in the top layer of skin. Fungi grow best in warm wet places, such as the area between the toes. Athlete's foot spreads easily, and you can get it by touching the toes or feet of a person who has it. "People most often get it by walking barefoot on contaminated surfaces near swimming pools, on pool decks, or in locker or shower rooms," says Dr. Joseph Stern, a Vancouver-based podiatrist and the president of the Canadian Podiatric Medical Association. The fungi then grow in your shoes, especially if they're so tight that air can't circulate around your feet.

Treatment includes over-the-counter topical anti-fungal medication or a prescription from your doctor. The skin should clear up in two to six weeks. "To prevent getting Athlete's foot, wear something on your feet to and from the swimming pool," says Stern. It also helps to wash your feet with soap and water daily, dry them carefully (especially between the toes) and wear moisture-wicking socks in running shoes.

Malta Podiatrist Awarded Ph.D in Clinical Biomechanics

Podiatrist Alfred Gatt was recently awarded a Ph.D in Clinical Biomechanics from Staffordshire University, UK. His doctoral research, under the supervision of Prof. Nachiappan Chockalingam, was entitled ‘Biomechanical assessment of passive ankle joint complex dorsiflexion’.

Dr. Gatt has published articles in various international peer-reviewed journals, including the Journal of the American Podiatric Medical Association, The Foot, and Prosthetics and Orthotics International. A former head of the Podiatry Department at the Department of Health, he now lectures at the University’s Faculty of Health Sciences. He is a founding member of the Diabetes Foot Research Group, which was set up to research diabetic foot disease in Malta.

CA Podiatrist Holds 14th Annual Shoe Drive

Long Beach podiatrist Dr. S. Don Kim is taking steps to help the less fortunate. Kim Foot & Ankle Centers, Inc. is hosting its 14th annual Biggest Shoe Drive in Southern California — collections are being accepted now through Aug. 8.

Kim, 49, who said he first realized there was a need for shoes while doing free podiatry work in impoverished areas of Mexico, has collected more than 100,000 shoes in the past 14 years. The new and gently used shoes — for men and women of all ages — are given to local charities as well as organizations in Mexico (shoes also were sent to Japan after the tsunami last year).

Retired NJ Podiatrist is World-Class Photographer

In the 1970s, when Dr. Jonathan Singer was working as a podiatric surgeon in Arizona, he traveled to Yosemite National Park, where he met the photographer Ansel Adams at his gallery there. “I was inspired by his passion,” he said in a telephone interview recalling their meeting. “Looking into his eyes, it was an energy force. I always aspired to that.”

Now retired from podiatry, Dr. Singer, 64, began pursuing photography in earnest about 10 years ago. His photographs of rare flowers and plants are being shown in “Botanica Magnifica: Photographs by Jonathan Singer,” an exhibition running through Aug. 26 at the New Jersey State Museum in Trenton.

Query: Pruritic Lesions, Arm and Legs

My patient is an 84 year old, caucasian female with unilateral pruritic lesions left leg, which have been worsening for six months. The lesion is negative for fungal elements: KOH and two week culture. No Auspitz sign is elicited. Culture of some of the scale reveals 1+ Staph epidermidis.

There are similar lesions on the contralateral (right) arm. If anyone has seen this or knows what it is, I would appreciate your opinion. Most lesions are on the left leg, but there are lesions about the ankle cutis as well.

QUERIES (EMR)

Query: Sending Data for CMS Incentive

Last year, we switched from Medinotes to Lytec (McKessen). Problems arose when we tried to turn in our 3 months of records. While we are told that all our information is there, Dabb, the company handling our account is unable to get the information out of the computer to send it to CMS. They say it just happens to be a "glitch" in our software and we must start all over again. Our problem is that one of the docs who has been with us for almost 2 years is leaving. It doesn't seem fair to lose the incentive for him if he leaves. Is there an alternative way to turn in information, such as printing the charts and info and sending that to CMS instead of electronic sending?

Query: CME Courses In Hygiene and Sterility

Since I am now required by my Board of Registration to attend 30 hours of CME on "hygiene and sterility" because I did not sterilize my Dremel bit between patients (I do now), I am requesting your suggestions as to where such CME units can be found. The board is also requiring me to submit a list of proposed courses so that they may approve of the topics. I am having great difficulty finding courses that meet such limited criteria. Any suggestions would be appreciated.

RE: Classification of Diabetes

I realize this might seem petty, but as the foot experts and as the foot experts in diabetes, many podiatrists have trouble with the classification of diabetes. I read many times on PM News podiatrists who misuse classification of Type 1 and Type 2. The old terms were juvenile diabetes and adult diabetes. These were changed to IDDM, insulin dependent diabetes mellitus and NIIDM, non-insulin dependent diabetes mellitus.

This was confusing as well, so the terms were changed to Type 1 and Type 2. You do not change types once diagnosed. Think of the old types as well as the word "dependent". Type 1 are dependent on insulin. Without insulin, they would die. Type 2 may take insulin, but this does not make them a Type 1 just because they take insulin. They are NOT dependent on insulin to live. They use insulin to help control their diabetes.

Remember that 90 % of those with diabetes have Type 2 and 10% have Type 1. Type 1 is mostly diagnosed in children (average age 9-12), while Type 2 is normally found in adults who are overweight. Type 1 is a quick onset while Type 2 is a gradual onset. If we are going to be the experts in the diabetic foot, let's use acceptable terms like our fellow healthcare workers.  I do see a lot of misuse by MDs as well.

The query was how to find out if a patient is deceased without using the IVR. Personally, for the expense, time, hassle and other aggravations, I would simply mail the announcement to the information that my office has on file. That said, the most current free site is the Death Master File edition of November 30, 2011. This site can be found at ssdmf.info/.  Paid sites include ancestry.com, genealogy.com, etc. 

One can also use the paid site CheckMedicare.com (the parent company is strongly supportive of podiatry) that provides information directly from CMS. From this site, you also get the current address of your patient, plus the patient’s zipcode+4. The CheckMedicare.com site can be copy/pasted into other computer programs (e.g., Microsoft Word). My office prints the CheckMedicare.com into a readable pdf file for future easy access. I am not aware of any site that provides this information for free anymore, so the November 30, 2011 is the most current free information. Though tedious and not always accurate, you can perform a search using the terms: obituary death “patient name” which will give you paid obituary information. The advantage of CheckMedicare.com is that the site is HIPAA compliant.

RE:  UK Podiatrist Discusses Treatment of Neuromas (Peter Bregman, DPM)

I am in full agreement with Dr. Bregman. Interspace nerve conditions are, in my experience, almost exclusively entrapment disorders. As such, I have been performing decompression procedures for the past 20 years with very good results. Neuroma (as defined as a tumor of the nerve) is extremely rare. What we see in “clinical neuromas” is an inflammatory, not a neoplastic process.

For the most part, I have been using the IsoGard system of decompression. I do this in my office using local anesthesia. I have not removed an interspace “neuroma” in years. The Association of Extremity Nerve Specialists is an excellent source of information and training to learn these advanced nerve techniques.
