Non-Painful Bunions Don't Need Surgery: PA Podiatrist
Unsightly bunions all have something in common: They're a pain. People often tolerate bunions, but calling a doctor is the next step to a healthier foot. How will you know if your bunion requires surgical correction? “I always tell my patients, you can have a big, ugly bunion and it might not bother you, so until it affects your daily life, that's when an attempt should be made to surgically correct it,” said Paul LaFata, DPM, a board certified doctor of podiatric medicine at West Lawn Podiatry, Spring Township.

“I have patients who are not happy with the way their foot looks, but if they're not having pain, I wouldn't want to operate because we're not doing cosmetic bunion surgery,” Dr. LaFata said. Any type of surgery has the potential for risks, such as infection or bleeding, so unnecessary intervention is not a good idea, he said.

PA Podiatrist Provides Running Shoe Buying Advice
If you're a runner, the wrong running shoe could sideline you, a foot expert says. Choose carefully, or you risk discomfort, pain, and injuries such as shin splints, plantar fasciitis, stress fractures, irritated nerves or the loss of a toenail, warned Christina Rowe-Bauer, DPM, a podiatrist with Penn State Health Sports Medicine. When buying shoes, do so in-person at a store, preferably one with knowledgeable staff who encourage trial runs in the shoes at or outside the store.

Shop in the late afternoon because feet swell as the day progresses and as a run progresses, Dr. Rowe-Bauer said. When trying on shoes, wear the socks you wear while running. Choose shoes that fit well, regardless of size. Foot size changes over the years, and shoe sizes vary by brand and even by model within the same brand. Select shoes with a wide toe box so that your toes aren't constricted during running, she added.

Irish Podiatrist Rows Across the Atlantic for Charity
What takes 32 days, 22 hours, and 5,500 kilometers to complete? If you’re the four young men who set a new record when they arrived in Antigua, the answer is rowing across the Atlantic. “We had calluses on our hands and our calf muscles had wasted away so much that it took a couple of days before we could put one foot in front of the other without looking as though we had drunk six pints,” says Eoin O’Farrell, one of the four.

The foursome consisted of junior doctors Seán Underwood (25) and Patrick O’Connor (28), podiatrist Eoin O’Farrell (26), and entrepreneur Thomas Browne (27). To make the challenge even more worthwhile, they decided to use it to raise funds for Cork University Hospital. “They say charity begins at home, and Seán, Patrick, and I are from Cork,” says Eoin. “We’ve also worked in the hospital. They are building a new children’s unit and we decided to try to raise €20,000 for the dedicated cystic fibrosis ward.” They have raised over €29,000 to date.

VTE: Long-Term Recurrence Risk Stays High for Years
A systematic review meta-analysis was made of 18 randomized controlled trials and prospective cohort studies; 7,515 patients. After an initial unprovoked venous thromboembolism (VTE), patients who complete at least 3 months of anticoagulant therapy still face a high recurrence risk up to 10 years. The results of this meta-analysis emphasize the need for close long-term management.
Follow-up in this study outpaces that in most previous analyses, which found an overall recurrence risk of about 10% yearly in the first couple of years. This analysis finds triple that risk at 10 years.

Off-Label Use: Patient Safety Implications - Part 1
The patient safety implications for physicians in prescribing a medication or using a medical device in an off-label manner include the following:
-
Ensure that the medication is not an experimental drug. It must have FDA approval for some use and not be for “investigational use.”

Become knowledgeable about the medication or device, its originally approved use, and the new “off-label” use—including the potential risks and complications, side-effects, and contraindications with its use. For example, if the medication literature or package insert states that it is “not for” a particular use, avoid that off-label use.

Investigate the acceptance of the proposed off-label use in the medical community or your specialty. Determine whether the proposed off-label use meets the national standards in the specialty.

Conduct a thorough history and physical to determine whether the patient has any underlying medical conditions that would prohibit the off-label use and whether the patient is taking other medications that might adversely interact with the proposed off-label use.

Although most practices have little control over patient coverage types, they can better mitigate financial risk by understanding all the components that impact revenue. Here are six ways to improve your practice’s RCM reimbursement tactics.
2. Build a payer matrix - Reduce each contract to the most basic components that affect day-to-day operations: effective and term dates, notice periods, and default fee schedules. Update the matrix regularly to reflect any amendments, addendums, or fee schedule changes to the original agreement.

Podiatrist, musician, WWII Veteran, golfer, individualist, conservative, husband, and father. John “Jack” Reyer, 98, of Hermitage went to be with the Lord on August 1, 2019 surrounded by his loved ones. Dr. Reyer studied podiatry at Temple University from 1941-1943.

After serving in WWII, he graduated from the Ohio College of Podiatric Medicine in 1947. His mentor was Dr. Ralph Dye; Dr. Reyer specialized in the Dye technique of foot correction.

For many years, I was a Medicare provider. For the last ten years, I have been on Medicare and Tricare as a patient. I pay nothing for Tricare and somewhat over a hundred dollars a month for my Medicare coverage. What I pay a month can increase a little bit when my income goes over a certain amount. What I pay for Medicare and what I have paid by Medicare taxes covers only about one third of what Medicare really costs. Of course, those on Medicaid pay none of their own costs. In the old days, we used to call Medicaid welfare. In the old days, we would have to call at least 70 percent of what we get from Medicare also welfare. Today, all these programs are referred to as entitlements! I resent that my military retirement is also referred to as an entitlement!
We must remember that in this brave new socialistic economy we will have to get used to it. Our younger podiatrists will have to get used to working for the government because I can assure you that the private practice of medicine will be forbidden.
