Pedorthists & Podiatrists Should Work Together: PA Podiatrist
 
There has been some backlash to the decision by the American Podiatric Medical Association (APMA) to enter into an agreement to hold its 2015 annual symposium in conjunction with PFA’s symposium. “There was a controversy when I came out in favor of the meeting,” says James McGuire, DPM, CPed, director of the Leonard S. Abrams Center for Advanced Wound Healing at Temple University’s School of Podiatric Medicine. “I got negative feedback from a few podiatrists who felt that they didn’t need pedorthists and that they were competition for their practice.” That’s not the case though, he says. “A pedorthist working with you or as a close referral source can actually enhance your practice, and in many instances can do things you would rather not be bothered with in your office, freeing you up to concentrate on aspects of your practice that are more lucrative.”

The truth is that a good pedorthist-podiatrist relationship can benefit everyone, the experts say. The pedorthist and podiatrist can each increase their business revenue, get more referrals, and best of all, the patient has the benefit of a medical team working alongside one another for the best possible care. “Podiatrists don’t need to be threatened by pedorthists, and pedorthists don’t need to be threatened by podiatrists,” says McGuire. “The work they do is complementary.”

ICD-10 UPDATE

New ICD-10 Guidance - Part 7

Question 7: National Coverage Determinations (NCD) and Local Coverage Determinations (LCD) often indicate specific diagnosis codes are required. Does the recent Guidance mean the published NCDs and LCDs will be changed to include families of codes rather than specific codes?
 
Answer 7: No. As stated in the CMS’ Guidance, for 12 months after ICD-10 implementation, Medicare review contractors will not deny physician or other practitioner claims billed under the Part B physician fee schedule through either automated medical review or complex medical record review based solely on the specificity of the ICD-10 diagnosis code as long as the physician/practitioner used a valid code from the right family of codes. The Medicare review contractors include the Medicare Administrative Contractors, the Recovery Auditors, the Zone Program Integrity Contractors, and the Supplemental Medical Review Contractor.

As such, the recent Guidance does not change the coding specificity required by the NCDs and LCDs. Coverage policies that currently require a specific diagnosis under ICD-9 will continue to require a specific diagnosis under ICD-10. It is important to note that these policies will require no greater specificity in ICD-10 than was required in ICD-9, with the exception of laterality, which does not exist in ICD-9. LCDs and NCDs that contain ICD-10 codes for right side, left side, or bilateral do not allow for unspecified side. The NCDs and LCDs are publicly available and can be found at cms.gov/medicare-coverage-database/.

3. Adjustments
 
Also known as "write offs," adjustments are the detailed categories to which staff post the amount between an actual payment and your standard fee. These include contractual adjustments and multiple procedure discounts, of course, but also adjustments for denials (such as patient ineligible or a coding error), and A/R clean-up actions (such as bad debt and small balance adjustments).
 
The report should be several pages long; adjustments are supposed to be granular. If your report contains broad categories such as "miscellaneous" or "insurance write-off," that's a problem. Ask your manager to explain vague sounding categories, as well as large dollar amounts, and request the addition of more detailed adjustment categories, if necessary.

Reconstructive Foot and Ankle Surgery - This fellowship is a one-year experience during which the Fellow will develop a rational approach to revisional and reconstructive foot and ankle surgery. I fully agree with Dr. Kinberg's response on billing for the CPT 64632 service. However, where have you found a supplier for the dehydrated alcohol? The few U.S. dealers making this USP certified pharmaceutical available, have it priced so high that the cost of making a 25% solution + local + syringe and needle would force you to perform this service at a virtual LOSS! (There is affordable chemical grade USP dehydrated alcohol available, but it not for human/medical use). As a result, I no longer offer this effective treatment to my patients.
 
Whether it's our government or big pharma, many perfectly good medicinals have disappeared or been made too pricey to use (i.e.: polysporin powder --> gone! [no substitute], gentamycin cream went from $1.47/15 gm 1.5 years ago to almost $30/15 gm today! --> Neosporin and Silvadene cream are available, and cost-effective, but they have a higher allergy rate).
