PRACTICE MANAGEMENT TIP OF THE DAY
Seven Communication Sins Every Practice Should Avoid

Avoiding seven common communication errors can boost the productivity of the average medical group by 80 percent. So claims Skip Weisman of Weisman Success Resources, a Poughkeepsie, NY-based consultancy.

5. Lack of Appropriate Tone and Body Language

Negative expressions, tone of voice and gestures, such as eye rolling, often get in the way of helpful communication, Weisman said.

3. An understanding of the impact that stereotyping can have on medical decision-making.
4. Strategies for recognizing patterns of healthcare disparities and eliminating factors influencing them.
5. Approaches to enhance cross-cultural skills, such as those relating to history-taking, problem solving and promoting patient compliance.
6. Techniques to deal with language barriers and other communication needs, including working with interpreters.

RESPONSES/COMMENTS (APMA STATE COMPONENT NEWS)
From: Lloyd Smith, DPM

Seeing 3 icons of our profession mentioned in one blog deserves a few comments. Having them all come from California is not a surprise. Having them all as friends has been my privilege. Jon Hultman has been a determined and committed leader in California. He solidified a state with much controversy and has kept the state focused on our collective key priorities. He was always a wise counsel to me through the years.

Jeff DeSantis was introduced to me at the APMA House of Delegates years ago as the best leader to come from California in years. I was privileged to give his nominating speech as he became an APMA Trustee and eventually our President. He has become a dear friend and colleague.

Lyman Wilson was always a quiet leader and great sounding board on many controversial issues of each House. I will never forget his warmth and wisdom. We should all salute these three gentlemen.

RESPONSES/COMMENTS (MEDICAL-LEGAL)
From: Kristin Happel

As far as requests from CIOX and other companies doing "risk adjustment reviews/audits" on charts, it is just like Dr. Kesselman says: these companies are hired by Medicare Advantage plans to look for DXs that means they can charge CMS/Medicare more to insure patients who opt for these plans. When we get these requests, we send an invoice to the company in question (often CIOX) requesting payment of $100 per chart. For us, this is usually around a $3500-$4000 invoice. In the past, this has been paid, and then we send what they want, of course.

However, in 2023, CIOX decided to "argue" with us. They said it was in our "contract" with UHC to send the chart notes free of charge. When we asked them to send us the portion of our contract that said that, what we got back from them was...crickets. We know what our contract says, and it does not say that, and they knew it too, but they thought we would fall for their BS. Nope. They never sent us the money we requested (although they said several times a check was in the mail), and we never sent them the charts. And they did not boot us from UHC, because they can't. I have said it before in this forum and I will say it again, regarding risk adjustment reviews/audits: "Show me the money." End of discussion.

As far as the "post-payment chart review" request from Noridian MAC, I defer to Dr. Kesselman on this, because I have never seen this happen unless it was a refund request where something was accidentally billed incorrectly that resulted in a refund being owed, and in all circumstances, once the initial billing was corrected, the claim then paid again.

RESPONSES/COMMENTS (DME)
From: Elliot Udell, DPM

A multi-faceted approach to fall prevention is something that we all should be involved with. The larger the practice is, the more opportunity there is to potentially save lives. As to the Moore Balance Brace, Dr. Richie, the inventor of the Richie Brace cites research that it does not prevent falls.

Dr. Moore and Jason Kraus, cite university-based research that their devices do help prevent falls. Dr. Moore has lectured extensively on the benefits of balance bracing and cites hundreds if not thousands of patients he has treated with his brace. This, combined with the fact that so many major AFO labs make and dispense balance braces, makes it seem unfair to take one expert’s opinion over another. Perhaps PM News  can do a survey on whether practitioners who prescribe balance braces have seen any benefit in the realm of fall prevention. This might help resolve this controversy.

RESPONSES/COMMENTS (NON-CLINICAL)
From: Paul Kesselman, DPM

I would sit down with the administration and point out rather graphically how the DO profession did find themselves in the exact same position years ago. And in fact, they are still facing the exact same discrimination. I have several close friends who went through the same sort of "hazing" many podiatrists went through, until it eventually stopped because we were able to prove our knowledge base in our area of expertise was at least equal if not superior to those "real doctors”.

You might even have some conversations with the AOA who has had a more cordial relationship with APMA than AMA. Certainly as was mentioned, the DO schools which have relationships with DPM schools might also help and be willing to intercede. But I have a further comment that goes beyond the professional issues. It is the parent who has given so much to get their child into the position of being "coated". To me, taking that opportunity away is a far greater form of disrespect to the parent than this professional prejudice.

I remember my WCC at ICPM more than forty years ago and it was the parent not some stranger who was given the honor to White Coat their child. This is the ultimate form of disrespect to a parent.

RESPONSES/COMMENTS (OBITUARIES) - PART 1A

Dr. Lyman Wilson was an extraordinary individual—humble yet brilliantly insightful. He loved our profession! A kind and remarkable mentor, he was a pillar in guiding many outstanding podiatrists in Southern California. His dedication to patient care and extensive political, educational, and administrative contributions have profoundly shaped our profession here in California. Dr. Wilson's engaging personality and persuasive advocacy efforts left an indelible mark on all fortunate to cross his path. Rest in peace, Lyman. Your legacy endures.

RESPONSES/COMMENTS (OBITUARIES) - PART 1B

RE: The Passing of Lyman Harold Wilson, DPM

It hurts my heart to say that my friend, colleague, and mentor, Lyman Wilson, has passed. I was a shot/discus track coach at El Modena high school when his son Michael entered the ring. I told him I was interested in feet, and I just told him how to throw the discus, and he said, do you want to talk to my dad?

I was researching podiatry at the time, and he convinced me that it was the only way to go. He would have convinced you to change your profession and become a podiatrist. He was an ambassador for our profession. A wonderful man!
