MPMA Donates 200 Pairs of Shoes to Needy Children
For plenty of kids, a new pair of shoes is a given for “back to school” time. Thanks to the Michigan Podiatric Medical Association (MPMA), nearly 200 youngsters in Wellspring Lutheran Services programs will be among those sporting new “kicks” when classes resume next month. Recently, the MPMA dropped off dozens upon dozens of new athletic shoes at the New Directions Campus of Wellspring in Farmington Hills, which supports the foster care system and kids in residential care with emotional and behavioral issues.

“Children’s feet change quickly as they age,” said Dr. Jodie Sengstock, MPMA director of professional relations. “Improper fitting shoes can aggravate the feet and create sores and other problems. Our goal is to educate the public on how and why to have children in proper fitting footwear and to help some in-need children start the year off on the ‘right foot’.”

IN Podiatrist Joins Medical Staff at Jasper Memorial Hospital
Dr. Tyler Kelly, a podiatrist, was recently appointed to the active medical staff at Memorial Hospital in Jasper. Kelly received his medical degree from Kent State University College of Podiatric Medicine in Independence, Ohio, and completed his residency at St. Mary’s Medical Center in Evansville.

Most recently, he completed the Stryker Reconstructive Rearfoot and Ankle Fellowship Course in Las Vegas. He is a member of the American Academy of Podiatric Practice Management and the American Podiatric Medical Association.

Wearing Flip-Flops Can Create Hammertoes: WA Podiatrist
Spokane podiatrist Dr. Nicholas Tanner believes flip-flops do not provide the foot with enough support, which can cause long-term damage. “Flip-flops that don’t have any arch support are going to allow the arch to completely collapse,” Tanner said. He warned that a lack of arch support can lead to general foot pain, along with tendonitis.

Tanner said flip-flops can alter how the foot is held as toes grip the shoe while the foot lifts from the ground. Tanner said, this habit can create hammertoes or bent toes that will not fit into regular shoes and can require surgery to fix if left untreated. “When one is exclusively in the flip-flops, they are more susceptible to these problems,” Tanner said.

CA Podiatrist Charged with Embezzling Nearly $1 Million from Elderly Woman
A 71-year-old retired podiatrist who allegedly swindled an elderly client by promising to invest nearly $1 million of her life savings has been charged with embezzlement. Dr. Roger P. Hertzberg, a licensed financial advisor at one point, convinced the San Jose woman that he had invested her $900,000 in money market funds and real estate nationwide. But investigators believe the fugitive spent the money on himself.

Pioneering Limb-lengthening Technology Offers Painless Correction for Teen
Thirteen–year–old Carlos Echevarria is in the midst of a pioneering medical procedure. Inside his left leg is a titanium rod attached to his femur that – with the power of a tiny magnetic motor – gradually increases in length. The technology will make Carlos’ leg eight centimeters longer and correct a length discrepancy he has had since birth. Performed recently by Jonathan Schiller, MD, a pediatric orthopedist at Hasbro Children’s Hospital, this new procedure eliminates the need for a large external frame fixed with wires and pins, often a cause of irritation and pain, and virtually always a source of infection. Schiller is the first in Rhode Island to perform the procedure with the internal rod, called PRECICE Nail.
Schiller says the process takes some maturity and commitment from the patient, who must take a few minutes four times each day to activate the motor inside the rod using an external remote control, similar to a video game controller. Each of the four sessions stretches the rod just .25 mm. The process is painless. In Carlos’s case, the full course of treatment will take about 80 days. Once the bone has completely healed, the rod will be surgically removed approximately one year after it was placed.

I have used this modality for fibromas with limited success. It does not eradicate the fibroma but appears to soften the prominent tissue and allow it to be less prominent. Patients have found that it improves foot loading pain and avoids surgery. I have only done this in the last two years, so I am unable to provide any guidance about long-term outcomes.

RE: Time to Change Professions?

I was in a men's clothing store this past weekend. When I went up to the register to pay for my items, there was a man at the register checking me out. I noticed he was wearing a caduceus around his neck so I asked him about it. He told me he was an internist who could not bear to get up and go to work anymore. Battles with insurance companies and Medicare had worn him down. He now owns two shops selling men's clothing. He said he has never been happier. When someone wants something that he sells, they MUST PAY for it BEFORE they leave the store.
In essence, doctors who accept insurance are selling their wares and then letting the buyer walk out the door without paying. What's worse is that the buyers feel entitled to walk out the door (with armfuls of care) and not pay.
Perhaps it's time to think about...

I have found that the real answer from CMS regarding nail and callus care is the following: "You are covered for treatment/services that are medically necessary." They do not actually come out and say it is covered but talk "medically necessary". Patients tend to think that toenail and callus care are always medically necessary. I also ask, "Why did you leave the previous doctor when it was a covered service by the previous doctor?" Not only should you have a copy of the LCD from your regional carrier but also obtain a patient Medicare Booklet that describes what is covered and what is not covered.
Finally, I agree with Dr. Lucarelli, "I wish them well and tell them they can shop around for another doctor who will do what they want, but I am not going to lose my license, be fined, or end up in jail just to make them happy."

We live in a three-dimensional world that is ready for a three-dimensional representation of the space that lives under the bones of the foot. 3-D weight-bearing CAT Scans, 3-D software, videos, and graphics are the future and cannot be represented by footprints. Kapanji’s “Vault of The Foot” is a better, 3-D representation. The wet test that Dr. Reed uses needs to be made vestigial as it has led to little EBM. While it may be good for marketing gym shoes, it has little scientific justification.
Conclusions like “depending on your job and activity type, you may be okay with a flexible or low arch” reflect the vacuum that exists in podiatry when it comes to biomechanics, and deserves re-visiting.
