High Heels Can Lead to Pinched Nerves and Inflammation: Toronto Podiatrist

"In high heels of 3 or 4 inches, the big toes get pushed towards the second toes which creates an increased angulation at the big toe joint, and this can lead to bunions," according to Toronto podiatrist Lloyd Nesbitt, DPM.

FL Podiatist Receives Certificate of Recognition Award from FBPM

Dr. Joseph Sindone was part of a medical mission team to Haiti after the earthquake. The Florida Board of Podiatric Medicine has recognized his selfless efforts in taking care of adults and children in Haiti.  Beth Pearce, DPM, Chair of the Florida Board of Podiatric Medicine recently presented this prestigious award to Dr. Sindone at a ceremony in Tampa, Florida.

Dr. Sindone is Assistant Professor, Orthopaedic Surgery and Rehabilitation, University of Florida College of Medicine, Shands Jacksonville. Sindone is a Past President of the Florida Podiatric Medical Association, and is the Past Chair of the Florida Board of Podiatric Medicine.

NY Podiatrist to Be Lead Investigator in ActiPatchtm Clinical Trial

A 6-week pilot study evaluating ActiPatchtm as an effective adjunct treatment for the healing of recalcitrant wounds yielded positive results. By week 3, 50% percent of the subjects experienced complete recovery, and by week 6, subjects experienced an average of 93% reduction in ulcer size.

Research - The primary purpose of this fellowship is to provide the Podiatric Surgeon who has completed a minimum of a three year residency, and who is committed to a part-time/full-time academic career in Podiatry, further education on research of the Diabetic Foot. The fellow is expected to complete several clinical or basic research projects during the term. This fellowship is a two-year experience during which the Fellow will develop a rational approach to research of the Diabetic Foot and have the opportunity to earn a masters degree in Clinical Investigation. The Fellow will function as an Instructor/clinical.

Reconstructive Foot and Ankle Surgery - This fellowship is a one-year experience during which the Fellow will develop a rational approach to revisional and reconstructive foot and ankle surgery. The Fellow will function as an Instructor/clinical  and participate in the teaching of Podiatric medical students and residents, and have ample opportunity to be involved in clinical and basic research projects. The fellowship will provide the Podiatric Surgeon, further expertise in Charcot reconstruction, plastic surgery (diabetic soft tissue reconstruction), trauma and deformity correction.

Duration: 2 years (7/1/11 – 6/30/13) and 1 year (7/1/11 – 6/30/12) Application Deadline: 12/1/2010 Interviews: December 9–12, 2010 Stipend: $44,100/Year.  Requirements: Completion of 3-year residency; a commitment to a part-time/full-time academic career in Podiatric Medicine and Surgery. Texas License eligible & ABPS Board Qualification eligible in Foot & Rearfoot/Ankle Surgery (Test dates & Application Deadlines TBA).

FL Podiatrist Cycles to Raise Funds for ADA

Podiatrist Dr. Gregory Stirneman traveled over 100 miles by bicycle near the end of July, all in a single day. He said he rode for the American Diabetes Association in an effort to raise funds at the Tour De Cure in Fort Worth. “I rode 102 miles and raised $1,825 for the American Diabetes Association,” Stirneman said. “All the money goes to the ADA for research.”

Reform Law Includes Large New Fraud Enforcement Powers

Some of the least publicized but perhaps most far-reaching provisions of the new health reform law relate to fraud-and-abuse enforcement. The law grants the federal government dramatic new powers — many of them not clearly defined or in partial conflict with other statutes — to pursue both providers and health plans.

For example, one provision in the law states that Medicare and Medicaid payments may be suspended “pending an investigation of a credible allegation of fraud against the provider of services or supplier” unless the HHS secretary determines there is a good reason not to suspend the payments. The statute directs the HHS secretary to consult with the department’s Office of Inspector General to determine if there is such a credible allegation of fraud, but there is no definition of “credible allegation” in the law, notes Carrie Valiant, a partner in EpsteinBeckerGreen.  .

Cure Procrastination

Many hard-core procrastinators recognize that they have a problem; they just are not willing or able to force themselves to do the things that need doing. Find the resolve you need by making two lists.

First, list the workplace tasks that you enjoy doing and usually make time for every day, even when they are not important or urgent. In large bold letters, label the list “Do not do these.”

Next, identify important tasks that you often delay doing. Those may include such things as projects that you dread, housekeeping tasks you don’t complete, or patients you avoid contacting. Label that list “Do these.”

Hang the lists side by side in plain sight, as visual reminders of what you should be doing.

Query: Nail Surgery for Chemo Patient?

My patient is a 45 y.o. female who is currently undergoing chemotherapy for liver CA. I have treated her for an onychia which was easily managed via excision of a portion of the offending nail. She has indicated a desire for a "permanent" solution. I would like the thoughts of my esteemed colleagues regarding if and when this could be done. My sense is that this is NOT a necessary procedure as the nail can most likely be managed via regular podiatric care (her insurance will not pay for "routine care").

For many years, I have used candida injections for verrucae. Candida is a yeast derivative that is injected subdermally into several of the verrucae monthly until resolution. The dosage is .3 cc candida mixed with .3 cc Xylocaine. Basically, you create a wheal under several verrucae and follow up in four weeks, if not resolving. Candida can usually be purchased through the hospital pharmacies.

Most doctors have been involved with various types of IPAs and understand the concept. “Groups” tend to have more integration yielding the opportunity to garner more savings and improvements.  Through “grouping”, our members hope to achieve improved profits through management efficiencies, standardization, leveraging advanced technology, improving purchasing costs, sharing common expenses, leveraging ancillaries, and utilizing our EMR data to prove to payors that we have improved patient outcomes and superior utilization. Size without the evidence of quality isn’t expected to cut it in the future. In the case of the Dallas “supergroup”, Podiatric Medical Partners of Texas (PMPT), we spent three years reviewing various types of merger models settling with a structure that met our vision, goals and objectives. 

To optimize the value of the group, we wanted to standardize and integrate as many functions and items as possible, within certain parameters, prior to go-live. We have standardized and streamlined the majority of our operational functions to maximize economies and efficiencies. In some cases, practices were able to eliminate or reduce administrative staff by outsourcing these functions to our MSO, which “went live” March 29 of this year, just five months after signing an MSO service agreement with IPS. It took a lot of courage for our members to take this step. We anticipate membership to grow significantly in the coming months.

Every group will look a little different due to the members' philosophies and geographic region. Hopefully, we will see more of our colleagues nationally grouping together to solve future challenges. I am more than happy to share my experiences with anyone interested in taking this exciting journey.
