TX Podiatrist Part of Team Saving Limbs With LimFlow Procedure
For too long, there was nothing Dr. Miguel Montero-Baker could do to keep some of his patients from losing a foot or leg. Now, though, a novel procedure is aiming to give those “no option” patients some hope. Baylor College of Medicine was one of 20 sites across the United States — and the only site in Texas — that was part of a clinical trial for a procedure known as trans-catheter arterialization of deep veins (TAVD). The procedure, which uses a technology called LimFlow, redirects blood flow from a blocked artery to a vein. Veins typically carry blood toward the heart, but the procedure re-directs it toward a patient’s foot.

The Houston team was particularly successful at helping patients avoid amputation because they also focused on caring for wounds after the procedure. Montero-Baker often worked in tandem with Brian Lepow, DPM ,a podiatrist who specializes in limb salvage. “We had much higher success rates of limb preservation,” said Dr. Lepow, a former Baylor assistant professor who now works with Montero-Baker as director of clinical operations and chief of podiatry at HOPE Vascular & Podiatry. “That was because of that combined effort.”

Local Marketing Tips for Small Practices
Facebook? Radio ads? Local SEO? Billboards? Email? There are so many options when it comes to marketing your small medical practice that it can be hard to know where to begin. With research confirming that 98% of consumers went online to research local businesses in 2022, focusing on activities that make it easier for potential patients to find you on the Internet is the most logical place to start. Here are some tips for small practices.
Tip 4: Create a modern, responsive website - If appropriate, include an online booking option so visitors can schedule an appointment directly from your website. Your website should be responsive and work well on mobile devices—the latter being crucial, with Google saying that sites that work well on mobile may rank higher in search results. Opting for a platform like WordPress will also make updates and maintenance easier. If you can't find a local web designer to work with, on-demand platforms like Upwork and Fiverr are a cost-effective and convenient way to match with a freelancer.
It's important to remember that your website will need regular updates. Just as your citations and Google Business Profile should be kept up-to-date, your website will also need to be accurate and reflect any changes to opening hours, services, contact information, and so on.

The patient presents with an ulcer at the left heel. He in a global period after resection of the right 5th ray 4 weeks ago. My questions involve using modifiers for the left heel ulcer when he shows up for follow-up visits:
1. How do I code and modify selective debridement of the wound on the other foot? Modifier -79?
2. What if the left heel wound does not require debridement at that visit. I plan to bill an E/M code. Would this be both Modifier -24?

FPMB publishes a wealth of general, contact, licensure, and regulatory information on its website. This data is reported to FPMB by its Member Boards, and 37 of the 53 classify themselves as “separate boards” that are podiatry-specific.

Dr. Jacobs, as usual, makes many important statements during his recent letter to the editor regarding the need for podiatrists and other healthcare providers to be afforded the opportunity to provide an annual comprehensive diabetic foot examination.
Unfortunately, the current system does not agree as it is not only not affordable but inaccessible to most. Let me explain. Screening examinations with rare exceptions are unfortunately non-covered services. In the case of diabetic foot screening, the only two screening examinations are coded under Loss of Protective Sensation, better known as LOPS (Initial G0245) or Subsequent (G0246).

Drs. Jacobs and Guiliana and both correct. There needs to be comprehensive podiatric evaluation of diabetic patients that can be performed annually and will be covered by all insurance companies. The problem is consensus on what constitutes a pedal diabetic foot examination. Several years ago, at a board meeting of the American Society of Podiatric Medicine, we tried to establish such an examination that would be accepted by all podiatrists and the insurance world.
To our chagrin, each of the board members who were involved in some aspect of podiatric academia, including one who was also dean of a medical college, all had different ideas as to what a comprehensive podiatric diabetic foot exam should consist of. Each doctor was firm that his protocol was the only correct one, and there was no consensus. There was no room for compromise. We dropped the project.
Perhaps Drs. Guiliana and Jacobs who have wide support from all aspects of our profession could get the ball rolling again and could help establish consensus for a comprehensive podiatric diabetic foot exam that will be accepted by all podiatrists as well as all insurance carriers. It may seem like a high mountain to climb but if this is successful, it could save the limbs and lives of many of our patients.
