Wet Footprint is Arch Type Indicator: IL Podiatrist
According to podiatrist, Dr. Brennan Reed, we have two arches in our feet. "Most people know about the longitudinal part of the foot, which we talk about either being high or low. But there's also another arch called the transverse arch that goes across the foot from side to side," Reed said.
|
|
Dr. Brennan Reed |
Reed said you can see for yourself by looking at your wet footprint when you walk out of a pool. "If you have a footprint that is kind of C shaped, you can see your toe pressures and you can see kind of a spot where your arch is, you probably have a normal or maybe a high arch. If you step out and your foot is wet and it's flat because every part of your foot is touching the ground like a pancake, then you know that your foot is flexible or has a low arch," Reed said. Reed said depending on your job and activity type, you may be okay with a flexible or low arch.

Retired MI Podiatrist Now Hosts CCTV Show
Dr. Fred Bernstein, a former podiatrist, is exploring his long-standing interest in broadcasting by hosting Let’s Go Clubbing, a new show that airs on Fox Run retirement community’s closed circuit TV station. He profiles Fox Run’s dozens of resident-run clubs and committees on his show.

IL Podiatrist Keynotes Address to Family Physicians and Internists
Sev Hrywnak, DPM, MD delivered a keynote address on the topic of healthcare economics to over 100 family physicians and internists during a day-long symposium on “The Changes in Healthcare”. Hrywnak said, “there are so many changes that have been occurring and these changes will have an impact on healthcare providers as well as patients.”
|
|
Dr. Sev Hrywnak |
During the day-long seminar, speakers discussed trends in medical care and the direction payors are taking. Value-based reimbursement, metrics, and analytics were also discussed. “It is very important now to begin to gather data about your patient care, the expenses involved, and the treatment results,” added Hrywnak.

FL Podiatrist Comments on Zika Virus
At the 2016 Annual Education Conference of the Florida Public Health Association (FPHA), newly appointed Florida Surgeon General and Secretary of Health, Celeste Philip, MD, MPH presented her “State of the State” address. A significant component of her report dealt with Florida’s preparations and strategies for dealing with the potential public health scourge of the Zika virus.
|
|
Drs. Celeste Philip and Chet Evans |
An FPHA board member and conference participant, Dr. Chet Evans said, "With the identification of two more possible non-travel related cases of Zika virus, it is more critical than ever that podiatric physicians, especially in the state of Florida, be aware of the epidemiology, signs, symptoms, therapeutic measures, and sequellae of this virus. I would like to see more podiatrists pursue an active interest and role in public health.” Dr. Evans is the Chief Health Innovation Officer for Rollins College and former Vice President for Medical Education and Program Development at Lake Erie College of Osteopathic Medicine.

RE: The Passing of Sherwin Levy, DPM
Sherwin Levy passed away this week. He was another icon in our profession. Sherwin was one of the first podiatrists to lecture and educate our profession on practice management. He later took on the role as a national leader at the VA. Sherwin was my neighbor and friend. He will be well remembered and sadly missed.

| RESPONSES/COMMENTS (NON-CLINICAL) - PART 1A | |
From: James Koon, DPM
Like many, I am tired of this complaint. It is really, really simple: YOU are the doctor interpreting the LCD. YOU know what will and will not get paid by insurance. YOU know what is and is not medically necessary. YOU are responsible for the repayments if Medicare deems the service non-covered and you submitted a claim for covered services.
If the patient is unhappy that their routine foot care is not covered then ask them the following question: "Mrs. Jones, are you asking me to commit fraud?”
“But my other doctor gets it covered!” Response: "Then...
Editor's note: Dr. Koon's extended-length letter can be read here.

| RESPONSES/COMMENTS (NON-CLINICAL) - PART 1B | |
From: James Lucarelli, DPM
What I have done to solve this problem is copy the LCD from my Medicare carrier and highlighted certain parts. If patients complain about wanting to be seen sooner, I show them the guidelines regulating the 60-day global period. They can pay cash if they want to come sooner. Just like your situation, I will show them what the Medicare guidelines consider RFC - i.e. corns and calluses. If I have a diabetic patient who does not meet the guidelines, I will show the patient the guideline in print, as well as show the covered diagnoses and class/clinical findings required.
If the patient tells me "they" said it would be covered, I tell them to get that person whom they spoke with on the phone to send it to me in writing that they now cover this service and it now supersedes the previous long-standing published LCD. If that doesn't work, I wish them well and tell them they can shop around for another doctor who will do what they want but I am not going to lose my license, be fined, or end up in jail just to make them happy.
James Lucarelli, DPM, New Bedford, MA

| RESPONSES/COMMENTS (NEWS STORIES) | |
From: Brian Fullem, DPM
In response to the quotes in the Minnesota paper, it is important to note that radial shockwave therapy does not create micro-fractures in the heel. Focused shockwave therapy can help to heal fractures but radial shockwave therapy or EPAT does not penetrate that deep. Also, what does 10% better than surgery even mean?
Radial shockwave therapy works by blocking substance P, the fibers that cause pain, and by inducing the production of new blood vessels to the area - neovascularization. An excellent free review article can be found here:
