EPAT Beats Surgery for Heel Pain by 10 Percent: MN Podiatrist
Dr. Walt Ellis said St. Cloud Foot & Ankle Center was an early adopter of the original EPAT technology. He said the clinic was part of a handful in the Upper Midwest rotating a large OssaTron unit around to help treat heel spur syndrome and other orthopedic injuries. However, the clunkiness of the machine, the large out-of-pocket expense on the patient ($600-$750 per session), and the infrequency of availability made the OssaTron system impractical.

Three months ago, Ellis decided to revamp his use of EPAT. "Basically we are creating tiny stress fractures and signaling to the body that it needs to repair itself," Ellis said. Each treatment lasts about 10 minutes and is completed weekly for a period of three to five weeks. Ellis said the success rate for EPAT is about 80 percent, 10 percentage points better than traditional surgery.

TX and DC Podiatrists Lecture at 1st Congress of Diabetic Limb Salvage in Asia
Dr. Lawrence Lavery, from the University of Texas Southwestern Medical Center in Dallas, and Dr. John Steinberg, from Georgetown University in Washington, DC, lectured and moderated scientific sessions at the 1st Congress of Diabetic Limb Salvage in Asia, which took place last month in Seoul, South Korea.

The meeting was attended by physicians from 23 countries. Many countries in Asia have the fastest growing rates of diabetes in the world. And diabetic foot complications are a growing public health concern.

Lawrence Kobak, DPM, JD : Charting should allow another physician to know what you did, and why you did it. A better chart that gets in to your reasons for what you do makes it much more difficult for attorneys and investigators to criticize. Most lawyers will move on to a doctor whose records are not so well done; thus, you need to chart today as much for compliance as to accurately explain what and why. Great records are worth the the investment of an extra 30 minutes each day.

10 Keys to Keeping Text Messages Safe - Part 5
8. Procedure: Screenshots - Create a procedure to prohibit screenshots of secure text messages. Most secure texting solutions natively prevent copy/paste, but they cannot prevent screenshots without a mobile device management solution.
9. Procedure: Dictation - Create a procedure to use keyboard dictation of any messages containing protected health information.
10. Procedure: Attachments -Create a procedure to prevent the ability to attach images or video that contain PHI from the device's camera roll. Instead, devise a procedure whereby users have to add attachments from the camera.

RE: The Passing of Sherwin Levy, DPM
Sherwin Levy, DPM of Santa Monica California has passed away. Although generally remembered as a successful practitioner, many may not realize that Dr. Levy was the first appointed director of podiatry services in the central office of the Veterans Administration. I had the pleasure of assisting Dr. Levy in the central office. He was instrumental in establishing many of the current foot and ankle services within the Veterans Administration and the growth of this service over the subsequent years.

Query: Ultrasound for Plantar Fibromas
I have a patient with a growing plantar fibroma, confirmed by MRI. My patient stated that he had read an article that ultrasound is now being used to destroy plantar fibromas. I am aware of ultrasound being used for plantar fasciitis, have used it in the past, but I am unable to find any reference to its use for fibromas. Does anyone have any information on this?

Dr. Moglia expresses understandable irritation when CMS gives "contrary advice" to patients who contact them regarding coverage of routine foot care, inevitably making him appear incorrect when he says it is ONLY covered under (highly limited) specific conditions.
I am reminded that the very same thing occurs when taxpayers contact the IRS and ask specific questions. The answers to those questions vary enormously and are almost always at "odds" with what your CPA stated. Your CPA says a circumstance is "black;" and the IRS says it is "white." There have been some studies on this very issue, and the replies from the IRS more resemble a dart-board in a bar where drinks have been free for the last 4 hours.
No matter what CMS says, if a DPM attempts to vary from the RFC rules excessively, there will be big trouble awaiting, no matter what CMS says or does not say. When patients bring this up, I think I would use the IRS metaphor as a direct comparison. No matter what CMS tells your patient(s), you do NOT "owe" them a personal professional healthcare prosecution to bend the rules so they can avoid paying you.

This problem goes way beyond having CMS employees telling Medicare patients wrong information about coverage policy. The IRS is famous for giving wrong information on tax rules to the public. I am sure most gov’t agencies have similar deficiencies in transmitting accurate information to the public who are simply trying to understand and comply with the rules.
The routine foot care issue, however, is plagued by so many other confounding problems. For example, the right for regional Medicare carriers to interpret federal CMS guidelines independently. While being on warfarin is a recognized risk factor and acceptable for routine foot care in one part of the country, the same patient’s care is medically unnecessary (and potentially fraud and abuse) in...
