FL Podiatrist Discusses Flip-Flops

Some call it flip-flop weather everyday in this island chain, but as summer heats up, nearly everyone can be seen wearing the open-toed sandal. With this, podiatrists say they expect many more visitors in their offices. A Tavernier podiatrist is advising her patients this summer to stay away from $2 flip-flops at drug and grocery stores. According to Dr. Brigette Smith, this footwear lacks stability and leads to ankle and foot problems. But many people in the Keys wear them too often, and convincing her patients to change can be a task.

Dr. Brigette Smith

"About 50 percent of people I can convince that the flip-flop is the problem," she said. Smith said the other half complain about the warm climate and refuse to give up flip-flops cold turkey. Smith said the patients who take her advice are usually motivated by their pain and are willing to try anything. But once the pain goes away, she said some of her patients fall off the wagon by reverting back to unhealthy habits. And although overweight people may be more prone to having foot problems, Smith said anyone can be susceptible.

AT THE COLLEGES

KSUCPM Ribbon Tying Ceremony to Be Held August 8th

Kent State University will host a symbolic ribbon-tying event on Wednesday, Aug. 8 to celebrate the establishment of the new Kent State University College of Podiatric Medicine, formerly known as the Ohio College of Podiatric Medicine.

Kent State University College of Podiatric Medicine

Faculty, staff, students, alumni, and friends of the university and the new college, as well as local officials and media representatives, are invited to attend the event, which will take place at the campus of the Kent State College of Podiatric Medicine. As opposed to a ribbon-cutting ceremony, the event is a ribbon tying, signifying the two institutions – Kent State and the Ohio College of Podiatric Medicine – coming together to become the Kent State University College of Podiatric Medicine.

INTERNATIONAL PODIATRISTS IN THE NEWS

Podiatrists Provide Full Spectrum of Care for Diabetes: Canadian DPM

"People with diabetes are individuals who share a common disease with wide-ranging effects," noted Dr. Timothy Kalla, an operative podiatrist in Vancouver, B.C. who specializes in preventing, diagnosing, and treating diabetes-related foot problems, one of the leading causes of hospitalization for people with diabetes. "Some people with diabetes are at a very high risk for foot complications where others are not."

Dr. Timothy Kalla

As a result, foot care needs to be tapered to the individual, based on his or her risk status and presentation. Foot care may range from regular preventative foot care visits to emergency operations. "Diabetes management necessitates a team approach, and podiatrists in Canada have the training and the ability to provide the full spectrum of foot care for people with diabetes."

NY Podiatrist Discusses Barefoot Running

Barefoot running is in vogue, but podiatrists caution that you have to watch your step. "You can't be a heel striker. You need to load up on the midfoot when you're a barefoot runner. You have to take a shorter stride," says Manhattan podiatrist Dr. Paul Greenberg.

Dr. Paul Greenberg

There are still obvious dangers to watch for. "They're coming in with a stress fracture, plantar fasciitis, or Achilles tendonitis. We'll explain that it's probably due to the barefoot running, but you can't prove it," says Greenberg.

PM JURY VERDICT REPORTER

RE: Delayed Union, Post-Bunionectomy (MI)
From: West's Jury Verdicts - Michigan Reports

Facts: Alice Collins said she presented to defendant podiatrists' employees at Michigan Foot and Ankle on May 2, 2006, regarding pain in her toes. She reportedly was diagnosed with hallux abductovalgus, tendoAchilles enthesiopathy, and contracted digits, and was provided Xylocaine and Decadron injections and physical therapy.

Collins said she returned to the clinic in June 2006 because of persistent pain, and Defendant 1 recommended surgery. Defendant 2 reportedly performed the surgery July 6, 2006, which included a bunionectomy, internal screw fixation, and a tenotomy. Collins allegedly received follow-up care at the clinic from podiatrist 3, but continued to experience pain.

Collins said she was diagnosed with delayed union of...

IN THE COURTS

MO Supreme Court Overturns Malpractice Caps

Overturning decades of prior legal rulings, the Missouri Supreme Court ruled this week that the state Legislature's $350,000 cap on non-economic medical malpractice damages is an illegal violation of residents' constitutional right to a trial by jury.

A divided court on Tuesday overturned the court's own 20-year-old decision to uphold caps on non-economic damages, ruling that "while this court always is hesitant to overturn precedent, it nonetheless has followed its obligation to do so where necessary to protect the constitutional rights of Missouri's citizens." writing for the majority on Tuesday, Chief Justice Richard Teitelman said Adams was wrong and overruled it, citing similar court decisions to overturn legislative malpractice damage caps in states such as Washington (1989), Oregon (1999), Alabama (1991) and Florida (1987).

RESPONSES / COMMENTS (CLINICAL)

RE: Chronic Diabetic Ulcer (Kel Sherkin, DPM)
From: Robert Phillips, DPM

From the picture, it looks like the patient has a hyperextension deformity of the IPJ. Also, it looks like the hallux is much longer than the 2nd toe. What is the ROM of the 1st MPJ? I’m not sure that your orthotic Rx gives me an accurate picture of the ROM and alignment factors. Does the FFVg-5 deg in the orthotic mean that the patient has a forefoot valgus? Any possibility of a forefoot valgus with a metatarsus primus elevatus? Is the length of the hallux due to an excessively long first metatarsal, or due to a long phalanx?

I’m not sure what a rigid polypro device does for a 255 lb. man. What thickness are you using? Is it milled or is it thermoformed? What is the length of the device from...

RE: Affordable Care Act (Obamacare) Perk (Jon Purdy, DPM)
From: Jason Kraus

Dr. Purdy hit the nail on the head in identifying why there is such a deep ideological divide in our country today. Dr. Purdy's hope that he still lived in a world that was founded in 1776 and his narrow view of capitalism, juxtaposed with those who believe that we don't really live in that world any longer, and that unbridled capitalism cannot solve all of today's very real challenges, has created an ever-widening abyss.

While I think most Americans still place their faith in the foundational pillars established by our founding fathers, many have come to realize that the unimaginable changes that have taken place in the past 230 years require us to reflect on the needs of our modern world and adapt in ways that preserve our democracy and advance society. Those who insist on harkening back to the days of yore will always be at odds with those who believe that our nation is best served by constantly evolving in a changing world in a manner that preserves our core beliefs as a nation.

RESPONSES / COMMENTS (NON-CLINICAL) - PART 1B

RE: Affordable Care Act (Obamacare) Perk (Jon Purdy, DPM)
From: Bryan C. Markinson, DPM

Dr. Lang clearly supports the notion of universal coverage (so do I) and clearly supports it to be run by the government. My personal preference is cradle to grave Medicare for everyone, financed by premiums through payroll deduction. This way, younger people who will use it less can finance the costs of the traditional Medicare we have now for the elderly and disabled. However, I would also like to see an "in" or "out" option for both doctor and patient.

Dr. Lang is also well versed in the provisions of the ACA. However, in present form, the ACA is dependent on the "exchange" to provide healthcare insurance to those now forced to purchase it. I find it incredulous to believe that 10 to 30 million people (no one knows how many for sure) are going to find the exchange premiums affordable, especially when they are used to having it painlessly withdrawn from their paychecks. Employers also have incentives to NOT provide coverage under ACA.

I foresee a groundswell of people, maybe millions, who will find themselves protected by ACA provisions, but unable to afford to participate. I am not talking about poor people here; I am talking about working middle class whose employers will take advantage of paying a fine rather than providing coverage.

RE: CA Podiatrist Says Team Approach Saves Limbs and Lives (Lee Rogers, DPM)
From: Allen Jacobs, DPM

No one suggested that multidisciplinary teams do not help reduce amputations. Dr. Rogers' suggestion that I did is beyond insulting. Dr. Rogers was asked to justify his suggestion in the interview that the sedentary lifestyle AS A RESULT of amputations was a cause of 68% mortality over 5 years. He did not do so.

Dr. Rogers was asked to support his suggestion of 68% mortality over 5 years post-amputation. He mixes studies which include those amputations resulting from PAD, presumably knowing that such patients also suffer from concurrent coronary, cerebral, and renal vascular disease.

Finally, it appears that Dr. Rogers has fallen back into the "I was misquoted, the statements were taken out of context" world of accountability, or lack thereof, of what we say. If Dr. Rogers wishes to take "credit" for interviews suggesting that he is saving ulcer patients at a 90% plus rate from amputation, he must also take credit for the "facts" which he offers to an interviewer.
