CA Podiatrist Uses High-Frequency Ultrasound to Treat Plantar Fasciitis
Millions of people suffer from heel pain, and it can be due to tight calf muscles, long-distance running, or shoes with poor support. Treatment for this painful condition can be just as excruciating. But now, there's a new device that local doctors are testing that doesn't involve needles or surgery. It's a high-frequency ultrasound that helps repair musculo-skeletal injuries. "This ultrasound penetrates through the skin through the fat, and actually directly focuses on a beam of energy into the plantar fascia that helps break up the scar tissue in the fascia," said Dr. Bob Baravarian, director of the University Foot & Ankle Institute in Santa Monica.

Using this technology, Baravarian can target exactly where the scar tissue is. The clinical trial involves two 5-minute treatments that are about a month apart. Then patients will have to wear a boot for about two weeks. "The microscopic injury that we cause needs to be protected so that it heals without scar tissue formation," he said.

As part of the podiatry team, Bartel will provide treatment for a wide range of foot and ankle conditions, from diagnosing and treating ingrown toenails and bunions to flat foot correction, Achilles tendon repair, and orthotics management.

Aussie Podiatrist Joins Liverpool Diabetes Research Group
To combat the alarming epidemic of diabetic amputations, the Ingham Institute Liverpool Diabetes research group is leading a number of research projects. This includes the world-first clinical trial led by Dr. Namson Lau and Professor Hugh Dickson, who are using the drug denosumab to treat charcot foot, a condition that causes the weakening of the bones in the foot that is a side-effect of diabetic foot disease.

Additionally, podiatrist Matt Malone recently joined with the Institute’s Infectious Diseases research team to investigate diabetes foot infections as a whole. Together, they are using next generation DNA techniques to better understand the bacteria causing diabetic foot infections (we call this the microbiome), their resistance to antibiotics, and the presence of unusual pathogens.

Query: Getting CMS to Provide the correct Advice on RFC
Patients get angry when I inform them that their foot pain caused by "callus" formation or trimming toenails is considered routine in the absence of qualifying criteria (i.e., PAD or at-risk DM). When they call CMS to complain, the helpful agent is happy to inform them that my advisory is incorrect. To whom can I complain to get CMS agents to get the policy straight?

Query: Getting Paid for an E/M and Strapping
When billing for an office E/M visit and strapping (CPT 29540), we are finding that payers are frequently bundling the two together, and paying for the strapping (which is of course the lesser charge). How do we bill correctly to get paid for both?

Response: The established E/M service is a component service included in the "comprehensive" strapping application according to the CCI edits [Medicare]. If, however, you use modifier "-25", you override the CCI edit. You can only use the "-25" modifier when the E/M is "a significant, separately identifiable evaluation and management service". In those case, you should get paid for both the strapping and the E/M visit. Non-Medicare payers have similar, but their own versions of bundling edits.
For example, if you are treating a patient with an established plantar fasciitis diagnosis by the application of a subsequent strapping, the E/M service would be included in the strapping allowance.

I would like to know more about the surgeon's closure technique and offer a suggestion. We close with one or two 4-0 Monocryl, horizontally-oriented subcutaneous sutures, not pulled so tight so as to create a lump of the loose adipose. Skin is closed with 5-0 Prolene in a horizontal mattress manner. Steristrips may be used in lieu of the Prolene if there is very good hemostasis. The skin and subcutaneous tissues in that area are very pliable and freely movable. All you're trying to do is re-approximate the skin and subcutaneous tissues. We do not try to "close" the deeper tissues.

Dr. Smith asks Dr. Siegel, “what makes him so special” because Dr. Siegel’s health is doing well because of ACA. My answer is a simple word: civilization. We live in a civilized society. I do not complain that I have to pay taxes for the parks in my city that I do not use; I do not complain about paying taxes for schools for children I did not have; I do not complain that I supplement a transit system that allows your patients access to your office; I do not complain that I pay taxes to build the roads that your patients use to travel to your office. These are the prices we pay for civilization. There are many who have that independent attitude that “I have mine; screw you!”
For you, I say, there is plenty of land available where many can go to live self-sufficient: you can sink your own wells, handle your own sewage, farm your own land. Have at it. That civilization can provide medical treatment and care for Dr. Siegal is a win/win situation for all of us: 1) he is working member of society who I am sure pays taxes, etc., and 2) we have a very intelligent man who is able to contribute to the well-being of citizens in his community by being a podiatrist. You see, those children that I pay taxes for their education, one day, they will take care of me. This is what we call civilization.

RE: What overall grade would you give the Affordable Care Act?

I agree with the doctors who think the ACA has gone way beyond what was needed and has in fact cut into our ability to keep up with overhead increases. It has forced many young practitioners out of practice and will continue to do just that. We experience patients who deny themselves the care they need or postpone it as they see deductibles and co-pays rise considerably.
All that was needed was to force insurance companies to accept people with pre-existing conditions. All the rest was icing on the cake to make insurance companies more profits.
