NJ Podiatrists Save Woman's Life
Three members of ProActive Foot and Ankle Associates, Dino DiTrolio, DPM. Vincent Giacalone, DPM and Spero Demoleas, DPM were involved in saving the life of a 62 year old woman. They assisted in pulling the unresponsive woman out of Mastens Lake in New York on July 25, 2020 and quickly administered CPR upon assessing she was not breathing.

After several minutes, they were able to express fluid from her lungs and recover her pulse. She was responsive and breathing prior to the arrival of EMS. ProActive Foot and Ankle Associates recently held a Basic Life-Saving (BLS) course for its members and other doctors in the community, and this was instrumental in the saving of this woman's life!

Christopher J. Collazo, DPM recently joined the Foot & Ankle Center of Illinois as full-time associate serving Decatur, IL and surrounding areas. Dr. Collazo earned his Doctor of Podiatric Medicine degree from Des Moines University College of Podiatry Medicine and Surgery.

Dr. Collazo completed an advanced surgical fellowship at the Weil Foot, Ankle, and Orthopedic Institute in Des Plaines, IL. He was a three-year resident physician at Sanford Medical Center, Fargo, ND and served as Chief Resident. He is Board Qualified in foot and reconstructive rearfoot/ankle surgery by the American Board of Foot and Ankle Surgery.

FDA Approves Capsaicin for Treatment of DPN of the Feet
Capsaicin (8%), a topical, non-systemic, non-opioid pain treatment delivered as an office procedure in the form of a patch placed directly onto the skin, has been approved by the FDA. Its use is for treating neuropathic pain associated with diabetic peripheral neuropathy (DPN) of the feet in adults but should not be dispensed to patients for self-administration or handling. Capsaicin has no known drug-drug interactions. The most common adverse reactions include application site reactions such as erythema, pain, and pruritus, the majority of which are transient and self-limited. Pain associated with diabetic neuropathy is an extremely challenging condition to diagnose, treat, and manage effectively, having a significant quality of life impact.
DPN affects around 28% of all patients diagnosed with diabetes, and approximately half of those with DPN will experience the debilitating manifestations of painful DPN in their lifetime. In the U.S., one fourth of the health expenditure on diabetes is spent on diabetic peripheral neuropathy and is estimated to be more than $10 billion annually. Last week, CMS announced that audits would resume on August 3, 2020, in spite of the ongoing pandemic and cases of COVID-19 actually worsening in large parts of the country. Due to a significant backlash from providers and representative organizations last week, CMS has provided some further clarification on audits.
1) Targeted Probe and Educate (TPE): No time frame for resumption
2) Recovery Audit Contractor (RAC) and Supplemental Medical Review Contractor (SMRC) audits: No time frame for resumption
3) Post-payment reviews by the DME carriers will be restarted and that includes those that were previously in the cue. However, providers who respond by providing documentation of their inability to complete the audit will be reviewed by the DME MAC and likely will be dismissed.
To those who are not DME providers, similar audit resolution will likely follow on the physician side. This could not have been accomplished without the hard work of the Medicare Jurisdiction Councils of which I am the APMA representative. So to the recent question of why it pays to be an APMA member, well there you have it.

RE: New Tsunami of Toenail Fungus Advertising

What’s with the new tsunami of toenail fungus advertising? Last night in one hour, I saw four different advertisements for OTC onychomycosis products including, Nonyx, Kerasal, Fungi-Nail, and Lamisil AT. Is there something new? Or is it that just most people are working/learning at home and not wearing socks and noticing their feet?
If this is what’s happening, let’s all encourage APMA that NOW is the time to launch an advertising campaign about seeing your podiatrist, or entrepreneurial DPMs to advertise locally for these patients. Let’s remember the John F. Kennedy quote, “When written in Chinese, the word 'crisis' is composed of two characters. One represents danger and the other represents opportunity.”

Surely, there is a chemist in the profession or in one of the smaller Pharm labs that can make it cheaper. I had a bad neuroma in 1986 in my left foot, third interspace. I used the combo of .5% Marcaine and alcohol 7 times in my left foot. I never had surgery and never had a problem again. This combination has worked on thousands of my patients. I am sounding the alarm for the pharmacologists or chemists that are in our profession to make an affordable denatured alcohol.

Dr. Secord is absolutely correct that this is not really sclerosing. The term is used by podiatrists to describe the injection of 4% alcohol/local anesthetic. We bill this as a CPT 64455, injection of a local anesthetic. I do think this procedure is very effective but it is not sclerosing; the term differentiates this for clinical diagnosis of a neuroma.
