A Changing of the Guard at CPMA
 
After dedicating over two decades of his life in service to the California Podiatric Medical Association (CPMA) as Executive Director, Jon A. Hultman, DPM, MBA, CVA, will step down from that position at the end of December 2024. Dr. Hultman’s tenure as CPMA's Executive Director is the longest in the Association's 112-year history. He was honored for his decades of service to CPMA's members, their patients, the Association, and the practice of podiatric medicine as a healing art during the California Podiatric Medical Association's Annual House of Delegates this past June.

Though stepping away from the role of Executive Director and the organization's day-to-day operations, Dr. Hultman will remain actively engaged with CPMA as the Association pursues its primary objective – PARITY! Cornelison, DPM will take over the Executive Director reigns at CPMA.

Christus Ochsner St. Patrick Residency Welcomes LA Podiatrist
 
The Christus Ochsner St. Patrick Hospital Podiatric Medicine and Surgical Residency Program welcomes foot and ankle specialist Sydney Rudy, DPM, as their newest resident. The program is a partnership with the Center for Orthopaedics. Rudy began the three-year program in July. She earned her Doctor of Podiatric Medicine degree from the Kent State University College of Podiatric Medicine in Independence, Ohio.

Day 1 of the lerEXPO Rearfoot/Forefoot MIS Surgical Virtual event gathered hundreds of clinicians for a weekend of learning. This session My MIS HAV Approach brought together Patrick DeHeer, DPM, Noman Siddiqui, DPM and Brain Loder, DPM to discuss their approach utilizing the MIS technique in treating hallux valgus. The session included an open dialogue that stimulated many questions form the audience looking to learn more about MIS.

Seven Communication Sins Every Practice Should Avoid
 
Avoiding seven common communication errors can boost the productivity of the average medical group by 80 percent. So claims Skip Weisman of Weisman Success Resources, a Poughkeepsie, NY-based consultancy.
 
3. Lack of Immediacy, Urgency, and Promptness 
 
Too often, Weisman said, we procrastinate in having difficult conversations. There are legitimate reasons for delaying a conversation, he acknowledged. "Sometimes right in the moment, immediately, is not the right time to communicate. There are other things going on. There may be other people in the environment."
 
If you are waiting to gather more information pertinent to the conversation, then you are not really procrastinating, he added. However, if you are avoiding a conversation because you are afraid, busy or just think it will be difficult, you may be causing harm.

Query: CMS/Noridian Chart Review
 
What are the actual  consequences of not responding to a post-payment  chart review request from CMS/Noridian? Noridian paperwork states just automatic recoupment of fees paid. Anything more serious? Ciox and Episource Medicare risk adjustment chart review requests? Ciox and Episource do not publish any consequences. Their risk adjustment oppressive chart requests (65 and 85 at a time) take  staff time away from my regular duties.

It saddens me to read your post. The agenda of the AMA is to limit those who can be called physicians and surgeons to only those who are MDs or DOs, so it does not surprise me. I couldn't imagine that prejudice against me when my daughter was going through medical school. In fact, during a case on an orthopedic rotation, the resident said something to that effect to her. The attending had him call her to apologize. She graduates from orthopedic residency next year and her attendings have asked me to present grand rounds. 
 
I think one option is to have your daughter transfer as this bias will not change. I discuss these biases in my lecture that is on PRESENT. This a good example of why we need our DPM schools to evolve to granting the MD degree. Sending empathy to you and your family from CA.

It is sad to read about this unnecessary discrimination against a DO student family. I know the Des Moines University DO college did a similar thing in 1984 when a son of a local DPM was not allowed to hood his son at DO graduation. The DPM had provided a free podiatry clinic to the DO school for 20 years leading up to this slap in the face. At the time, a nurse was allowed to hood her son at graduation, just to make the hurt even more. The College of Podiatric Medicine & Surgery at DMU was just starting at that time so the CPME was interested to hear about this and apparently did express concerns. If your daughter's school is at a university having a DPM program perhaps this could be something CPME might have some interest in.
 
The event was emotionally traumatic to the involved family. The DPM refused to teach podiatry students and residents for many years because of this snub.

In 2009, I introduced the Moore Balance Brace, which was initially manufactured by Arizona AFO. Following OHI's acquisition of Arizona AFO, I began a long-standing collaboration with OHI, primarily through the leadership of Jason Kraus, OHI’s former CEO. Under Jason’s guidance, numerous studies were conducted using my design, including biomechanical, clinical, and peer-reviewed research published in journals such as Geriatrics and JAPMA.
 
From the beginning, I advocated for a multidisciplinary fall prevention program. As the AFO gained widespread clinical use and demonstrated significant success, many DME companies started copying my basic design to replicate its clinical benefits and financial rewards. To differentiate the Moore Balance Brace from these imitations, I withdrew my intellectual property and involvement from OHI and once again collaborated with Orthotica’s CEO, Jason Kraus. Dr. Jacobs’ post is unfortunately true, despite similar posts, from many esteemed colleagues, many who have called for a re-emphasis of biomechanics. In the three or more years I personally have been posting on this issue, nothing in a positive direction has changed. Dr. Jacobs alludes to my review of charts not having sufficient biomechanical or basic gait data. That alone is bad enough! What he failed to mention is it’s far worse than that. In my role as advisor for a myriad of orthotic labs, the sheer volume of improperly completed order forms is staggering. And it’s not just limited to podiatrists. 
 
Chiropractors, orthotists and pedorthists also somehow feel the orthotic lab can fix a bad scan or cast, or somehow figure out the right prescription for what ails the patient. That’s like leaving it to the pharmacist to know which hypertensive medication a patient should take. No ill will towards the pharmacist, but that’s not their responsibility. Just as it’s not the labs issue to repair a cast or predict how much posting or shell rigidity your patient can tolerate. Fabrication labs can do just so much. It’s up to the clinician to do a proper work-up!
 
Our residents need more time in clinical rotations in biomechanics and less time doing reconstructive trauma surgery. That should be left to post-residency fellowships. We need to spend more time educating the next generation of DPMs on in-office, everyday routine matters and less time-on the less frequent. If that means CPME or other acronyms make changes, so be it. I’m not involved in academia but sometimes it takes an outsider to see what’s wrong. I believe that Dr. Jacobs, I, and many others of our generation see the problems too well!

As a practice consultant, I advise against no-show fees and getting new patient "deposits". Some of my clients follow my advice, some don't. One guess as to which clients have more successful practices.

I find this topic an interesting one. Since the pandemic, the no-show rate in my practice has increased significantly despite all kinds of text, email, and voice reminders being sent, and I have heard the same complaint from many of my colleagues. Many of my MD colleagues in my area are charging for no-shows and even eyebrow threading shops and manicure/pedicure shops are charging no-show fees too. Incredibly, some restaurants are now requiring a DEPOSIT to even reserve a table!  Yet many podiatrists seem scared when it comes to collecting money from patients for our fees and services when they shouldn't be. It's the way of the world - deductibles are high and insurance covers less every year. Have you been to the dentist or the dermatologist lately? They have no problem collecting cash and in large sums at that.  
 
What is the logic behind not charging patients who don't show up a fee for fear that you may scare them away? They're not showing up anyway! And if they decide they aren't going to pay it, that's ok. If they want to reschedule at some point in the future, they'll have to pay it before another appointment is given and in the meantime you just got rid of a patient who doesn't pay. I think that in this day and age, a reasonable no-show policy that includes a penalty for repeat offenders or blatant disregard of the appointment lets the patient know the importance of keeping the appointment and is an effective way to reduce no-shows. It certainly has had that effect in my practice.
