NC Podiatrists Hold Discover Podiatry Dinner for College Students
APMA President Sylvia Virbulis, DPM and Gretchen Lawrence, DPM discussed what it is like to be a podiatrist to Summer interns Katherine Greene and Breanna Littlejohn from Gardner-Webb University in Boiling Springs, NC. Warren Joseph, DPM talked about the many opportunities podiatrists have in the various practices and subspecialties across the state and the nation.

The undergraduate students were able to ask questions about our eleven podiatry schools and what it might take to become a podiatrist. 5 Coding Mistakes That Cost You Revenue
Accurate coding is one of the most intricate and often frustrating tasks that doctors and staff must do. Erroneous coding can bring significant financial duress to a medical practice, either in the form of decreased revenue, audits, or even clawbacks from private insurers and revenue audit contractors (RACs).
5. Inaccurate Utilization of Modifiers
The largest errors are the improper use of modifiers -25 and -59 to expand treatment, which can lead to audits and clawbacks. Modifier -25 should be appended to an E/M code to report a significant but separately identifiable additional service rendered during the encounter, such as an injection. Modifier -59 is used to identify procedures/services other than E/M services that are not normally reported together but were appropriate to render under the circumstances.

Query: Medicare and L5000
Does Medicare cover forefoot filler inserts (L5000) for patients that are NOT diabetic? Here is the scenario I am thinking about: the long-term treatment of a patient with peripheral arterial disease that required a transmetatarsal amputation.

Response: L5000 is a prosthetic and does not fall under the diabetic shoe rules. If the patient has the transmetatarsal amputation and a filler insert will help him, then you should be able to bill it to Medicare DMERC. Be sure you use the -KX modifier along with laterality. Also be sure your notes indicate exactly why the patient needs the filler.

RE: Comprehensive Evaluation of Diabetic Patients

Podiatrists have long understood that diabetic foot pathology is typically multifactorial in etiology. I strongly believe that a substantial number of diabetic alterations, infections, and amputations could be prevented by the recognition of these factors and early proactive care. This involves dermatologic evaluation, vascular evaluation, neurologic evaluation, gait analysis, and biomechanical examination, evaluation for underlying osseous and articular abnormalities, and integrating and understanding of these factors into proactive efforts to reduce the incidence of alterations, infection, and amputation.
All too often, I believe that the “treatment” of the diabetic patient for so-called at-risk care is simply reducing nails for onychomycosis or calluses, and prescribing diabetic footwear. In my experience, the evaluation of the diabetic patient, particularly at the time of the initial visit, requires a detailed multi system, evaluation, and then education of the patient regarding therapies to prevent ulceration. Interdictive strategies range from actual aggressive and active treatment of onychomycosis to the treatment of deformities that...

RE: If Nurse Practitioners Can Increase Scope, Why Can't Podiatrists?

A podiatrist is licensed to treat any foot condition which is affected by other body parts. From a practical point of view, an increase in scope to treat a foot condition caused by a high blood sugar level would not automatically mean a podiatrist should do so. Podiatrists would need to acquire proper training and/or experience.
My point is if anatomic scope such as the ankle can be increased, why not have a more liberal interpretation of treating foot conditions to include increasing the podiatric medical scope by having a less restrictive legal interpretation by a state board of podiatry or a state medical board?
