FL Podiatrist Recommends Using OTC Antifungal Spray in Gyms
Brian Fullem, DPM, a sports podiatrist in Clearwater, FL says bacterial infection via feet is unlikely unless you have an opening in the skin, such as a blister. In addition, a 2015 study on bacterial variability in gyms found that “shoe” surfaces (floors, mats) are much less likely than “hand” surfaces (machine rails, free weights) to host rapidly evolving bacteria, which are more likely to cause infections.

Fungal infections, however, are another matter. "Fungus needs three things to grow - darkness, warmth, and moisture," he says, making both your workout shoes and a typical gym locker room perfect environments. Appropriately enough, tinea pedis, or athlete's foot, is the most common fungal infection picked up in gyms, according to Fullem. "It's a good idea to wear some sort of footgear at a public pool or locker room," Fullem says. As an extra precaution, apply an over-the-counter antifungal spray to your shoes and feet after gym workouts.

OH Podiatrist Sentenced to Prison for Healthcare Fraud
A Mentor podiatrist was sentenced Monday to 33 months in federal prison for defrauding Medicare and Medicaid by submitting claims worth more than $1.1 million for treatment he provided, even though he was prohibited from doing so. Dr. Robert Rosenstein executed a scheme through Community Foot & Ankle of Mentor to get money for claims submitted to both programs, according to charging documents filed in March.

Cluster of Septic Arthritis Cases Tied to Careless Injections
Violations of sterile technique caused an outbreak of septic arthritis, according to a recent study discussed in Science Daily. “This large, costly outbreak highlights the serious consequences that can occur when healthcare providers do not follow infection prevention recommendations,” said Kathleen Ross, an epidemiologist with the New Jersey Department of Health.
The outbreak affected patients who underwent knee injections for osteoarthritis at a private New Jersey practice. An investigation began in March 2017 following complaints to the facility and reports to local and state departments of health. Inspections revealed poor handwashing, unsafe injection practices, and subpar cleaning and disinfection practices. Two such practices included using pharmacy bulk packaged products as multiple-dose containers and preparing syringes days before use.

PRACTICE MANAGEMENT TIP OF THE DAY |
5 Metrics to Improve Your Practice’s Finances- Part 4
Here are five key metrics that can shed light on your practice’s financial health:
4. Unexpected denials - These are wholly preventable and can be remedied with a change in process, policy, or behavior. Problems that lead to these unexpected denials include inconsistent benefits and eligibility verification, lack of review of medical policy changes, incorrect coding processes, and inconsistent maintenance of system tables. It’s possible to recoup some of this money, but resubmitting denied claims comes at a cost — and that takes away from the bottom line. The Medical Group Management Association (MGMA) estimates that it costs a practice an average of $25 for each denial that must be reworked and resubmitted. Overall, industry standards indicate unexpected denials should comprise less than 5 percent of total denials.

RESPONSES/COMMENTS (NON-CLINICAL) |
From: George Jacobson, DPM
It was mentioned that with VoIP when the Internet goes out, you lose service. I don't want to advertise for Comcast, but when our Internet goes out, their system automatically call forwards our office phone to our cell phone. Perhaps other companies do the same thing, but you have to set it up. This has been helpful and also informative when we didn't know the service was down. It can also be confusing when it isn't expected.
We have an office message on a designated cell phone so a patient will leave a message. At that point, we can usually figure out that something was wrong and test the phone lines. We then begin answering the cell phone as if it were the main line. If not corrected by end of day, we let it go to the message after hours. It was particularly helpful after hurricane Irma, as our office electricity was out for two weeks.
George Jacobson, DPM, Hollywood, FL
