NY Podiatrist Discusses Causes of Heel Fisures
Cracked heels can seemingly pop up out of nowhere, and they essentially suck during the summer when they're constantly exposed in sandals. And once they form, getting rid of them can prove tricky. Odds are your skin is quite literally cracking under pressure. "Our feet are responsible for holding up our body and therefore they withstand a tremendous amount of pressure," says Miguel Cunha, DPM, founder of Gotham Footcare. "When weight and pressure are applied to the heels of our feet, the skin expands outward. If the skin is dry, it becomes less elastic and rigid and therefore more prone to fissures and cracking."
|
Dr. Miguel Cunha |
There are quite a few factors that could be contributing to your cracked heels. Conditions like obesity, diabetes, eczema, hypothyroidism, Sjögren's syndrome, and juvenile plantar dermatosis have all been linked to cracked feet, says Dr. Cunha. Having flat feet, wearing ill-fitting shoes, and living in dry, cold weather can also play a part.

PODIATRISTS AND FOOTWEAR |
Look for Summer Shoes with Built-in Arch Supports: NY Podiatrist
“When it comes to summer shoes or sandals, a lot of them are hard to walk in because they are not connected to the foot,” NYC-based podiatrist, Emily Splichal, DPM, MS tells Yahoo Lifestyle. She recommends looking for sandals with straps that secure your foot and ankle for extra support, stability, and of course, comfort. But that’s just the beginning of her shopping checklist.
|
Dr. Emily Splichal |
It goes without saying, but it’s a reminder to pay close attention to a shoe’s footbed no matter what kind of style you’re shopping for. “A lot of summer sandals don't have built-in arch support, which you absolutely need, especially if you have flat feet,” says Dr. Splichal.

E-HEALTH NEWS |
Cost of Healthcare Data Breaches Reach Nearly $6.5M: Study
The financial impact of healthcare data breaches on organizations is nearly $6.5 million, according to a recent IBM study on data breach costs. Healthcare organizations had the highest cost of a breach, for the ninth year in a row––about 60% more than other industries in the study. Data breaches can result in losses of millions of dollars and customer turnover for companies across all industries, but the consequences can be even larger for healthcare organizations, which have sensitive patient information including medical records and personal information.
Over the last five years, the cost of a data breach across all industries has risen 12%, with an average cost of $3.92 million. Of that average, $1.42 million, or 36%, was the cost of lost business for organizations. Over the last 14 years of the study, the costs for data breaches in the U.S. have increased 130%. The costs represent the multiyear financial impact of breaches, increased regulation, and resolving criminal attacks.

PRACTICE MANAGEMENT TIP OF THE DAY |
5 Metrics to Improve Your Practice’s Finances- Part 3
Here are five key metrics that can shed light on your practice’s financial health:
3. Days in A/R - This represents the average number of days it takes for your practice to be reimbursed. Rejections and/or denials, incorrect coding, credentialing issues, and incorrect posting and appeals processes can stretch out the time it takes to be paid. Practices should break out days in A/R by insurance plan and patient so they can get a better sense of where problems exist, such as consistent coding errors, that they can address to speed up payment time. Any A/R that has surpassed 365 days from date of service should be considered for potential write-off.

CODINGLINE CORNER |
Query: The Billing of Custom Orthotics
A patient with a Medicaid plan is evaluated by our office and it is determined custom orthotics are medically necessary. The patient is cast for the orthotics. Can we mail the orthotics to the patient and bill for the orthotics on the date we place them in the mail (I believe it would be inappropriate to bill on the date the product was cast for). Of course, we would schedule a follow-up appointment for the patient to make sure the orthotics are working well. IF this is a possibility, should we just have the patient sign a form indicating the process?
Daniel Albertson, APRN, London, KY
Response: You are essentially asking if you can deliver a custom device without fitting it to the patient. This would depend on two matters with serious consequences for you:
1) Will your carrier allow you to do this?
2) Your comfort zone regarding the potential bad outcome(s) for your specific patient?
While surgical dressings and other types of DME can be shipped to a patient and most insurances companies and Medicare will permit this type of activity, custom fabricated devices typically require the patient to be fitted and dispensed prior to it being billed. Because this is for custom fabricated foot orthotics, one can assume safely this is not going to be billed to Medicare. However, because many non-Medicare carriers follow Medicare regulations, I suggest you do your due diligence.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1A |
From: Joseph Borreggine, DPM
I have been using VoIP for the last 4 years. There is a definite difference in phone voice quality since it is digital. There are also many extra features beyond the scope of this answer that will benefit your business. The only thing that is bad about VoIP is that when the Internet goes down, you lose your phone service. I use Consolidated Communications based in many states throughout the U.S.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1B |
From: Raymond Posa, MBA
VoIP is Voice over Internet Protocol; basically, your voice communications are transmitted over the Internet instead of the traditional phone lines. That service is called POTS which stands for plain old telephone service. In the early days of VoIP, there were issues with voice quality, but those days are long gone. The quality of VoIP is top notch and the systems have all the advanced features any practice could want.
One of the biggest differences between VoIP and traditional phone service is price. VoIP tends to be much less expensive. Another huge plus with VoIP is its portability. If you were to move your...
