On July 23, Paul Langer, DPM of Twin City Orthopedics and Josh White, DPM, CPed of OrthoFeet, completed the Ironman Lake Placid. For Langer, age 56, it was his second full-length triathlon and for White, 60, his fourth.

5 Coding Mistakes That Cost You Revenue
Accurate coding is one of the most intricate and often frustrating tasks that doctors and staff must do. Erroneous coding can bring significant financial duress to a medical practice, either in the form of decreased revenue, audits, or even clawbacks from private insurers and revenue audit contractors (RACs).
3. Inaccurate Capturing of Patient Status
The confusion between whether a patient is new or established, which should usually be established at the front desk, can lead to lower payments if the patient’s status is not properly captured.

For the record, Dr. Gordon is partly correct. Benzodiazepines, Ambien, and Tramadol are considered controlled substances but gabapentin is only considered a controlled drug in a handful of states and not by the federal government. This is as of 2022 and could change.
The bottom line on whether an individual should register for a DEA number depends on whether his or her practice warrants it. I know medical doctors who do not have DEA numbers and they are well-respected members of the healthcare community. They are however in specialties where prescribing opioids or the drugs listed by Dr. Gordon is rare for them. Because their practices are very large and have many PAs and nurse practitioners working for them, they deemed the expense of paying for DEA licenses for all of their practitioners to not be worth it.

Regarding the question on proper coding for the re-insertion of a loose and fixation device, my concern would not be the proper code. The real question would be why did the fixation device loosen? The author states there was no infection but those such infections can be quite subtle. If there is no infection, why would you think that the device would hold a second time when it failed to maintain purchase initially? I’d give it a little bit of thought before I'd introduce the same implant.

I read this post asking how to potentially bill for adjusting a backed out screw for a hammertoe just weeks after surgery. The doctor wanted to take the patient back to the operating room to “adjust the screw”. I really think something is wrong here. Taking the patient back to the operating room to adjust a simple twist of a screwdriver with a superficial percutaneous incision? This is a huge waste of insurance dollars spent when the doctor can simply do this in the office with local to the toe. Common sense should prevail.
Also, why would the doctor even want to charge for this? This should be a simple courtesy procedure since it is so close to the surgical intervention. Billing patients and insurance companies for this kind of situation, in my opinion, does not bode well from a PR point of view. Looks greedy. Why should the patient be presented with another bill for a nothing burger situation? What do my colleagues think?

I have always thought of Allen Jacobs, DPM as the Kurt Vonnegut of podiatry. His sharp witted writing and lectures also include similar thoughts on the trajectory of where things are going; in this case CME meetings. I agree that it is not a valuable use of time to have a series of infomercials. I do think that vendors need to have value to support meetings. I would like to see more open discussion and debate within a CME program so it is not just a highlight reel of best cases and scenarios. Industry and technology they represent do have a place on the podium, but it should be done in a thoughtful manner. Science has to be at the core but it is okay to have case studies that have some relevance as part of the discussion.
To think that many solutions that work will be able to come up with funding for meaningful studies is not living in reality. If it were that easy, there would be more double-blinded randomized controlled studies. That is not to say that these trials are not an important piece, but everything is relative in the practice of medicine.
