PODIATRISTS IN THE NEWS - PART 1

Forensic Testimony by OH Podiatrist Clears Accused Murderer

One certainly does not associate the work of a podiatrist with helping to free a man accused in a murder case from life in prison. But, if that podiatrist also doubles as a forensic podiatrist, such an occurrence can come about. Dr. Michael Forman, a Pepper Pike resident who practices in Lyndhurst, decided about four years ago that he wanted to add a little spice to his practice and pursued a podiatric forensics degree.

Dr. Michael Forman

In the end, all of Forman’s work paid off in a big way for a New Zealand defendant. While most forensic podiatrists are called in to give supplemental testimony as a defense or prosecuting lawyer builds a case, Forman’s work in the Scott Guy murder case in far-off New Zealand proved to be decisive in the finding of Ewan Macdonald not guilty of a murder charge. “It was the most famous criminal case in the history of New Zealand jurisprudence, similar to the O.J. case in the U.S.,” Forman noted.

PA Podiatrist Recommends Fitflops as an Alternative to Flip-Flops

Flip-flops are a big flop with podiatrists. “There’s no support system, very poor shock absorption and because of the way they’re constructed, your foot and ankle and leg all have to strain a lot more to walk and keep the flip-flop on,” said Dr. Peter S. Holtz of Holtgate Podiatry in Lemoyne.

Dr. Peter S. Holtz

Holtz recommends investing in Fitflops instead, which provide a support system not seen in the typical flip-flop. “You get what you pay for,” he said.

PODIATRIC PRODUCTS IN THE NEWS

CMT Ass'n Announces Partnership with Aetrex Worldwide

The Charcot-Marie-Tooth Association, a national non-profit devoted to Charcot-Marie-Tooth (CMT) disorder, has announced a partnership with Aetrex Worldwide, Inc., a global leader in comfort and wellness footwear products based in Teaneck, New Jersey. The partnership will focus on two critical areas: the delivery of comfortable, well-designed shoe solutions to people with CMT while creating awareness about CMT and the CMTA to the public and the trade through Aetrex’s broad reach of customers and professionals.

Larry Schwartz, CEO of Aetrex

“We’re very excited to partner with the CMTA,” said Larry Schwartz, CEO of Aetrex. “Their vision of seeing a world without CMT is something we hope to help them accomplish.” Together the CMTA and Aetrex Worldwide, Inc. will increase awareness about CMT while meeting the fundamental but unmet need of a well-fit shoe for those living with CMT.

QUERIES (NON-CLINICAL)

Query: Alternate Source for Oasis Grafts?

I have been using Oasis grafts for a few years, with great success. Now, Oasis grafts have become very expensive, almost prohibitively. Is there another source? How about a foreign source?

RE: Intermetatarsal Nerve Regeneration (Art Korbel, DPM)
From: Tip Sullivan, DPM

Having some experience in microsurgery, I can tell you that you: 1) just increased the surgical time from 15 minutes to at least an hour. 2) just put a foreign material in the foot - bovine collagen 3) just increased the size of your incision and dissection to at least twice the size of a typical neurectomy 4) your post op-care will require a much more guarded approach. As much as I enjoy new and innovative mousetraps, I advise leaving this idea alone.

RE: Affordable Care Act (Obamacare) Perk
From: Richard Rettig, DPM

My personal health insurance is with a major health insurance company through my wife's employment. Today, we received a letter titled "Notice of Health Insurance Premium Rebate."

It states they are REQUIRED by the Affordable Care Act to apply at least 85% of the health care premiums received towards actual medical care (and not 'profit', advertising, salaries, etc.). Even knowing they would be held to this number, they still missed it by 1.5% and have to refund the excess premium either as cash to us or towards reducing next year's premium. It is called the "Medical Loss Ratio Rule." In previous years, they would have kept this. 1.5% isn't that much, but I bet that in previous years, when there was no mandate to hit the 85% mark, that the number would have been much higher.

You may be for or against the ACA concept, but there are some goodies in those 2801 pages! You can be assured I never got a letter like this in the past. I am glad that the insurance companies are being held accountable for their excess premiums.

RE: Fotona Laser for Onychomycosis (Dennis Shavelson, DPM)
From: Chris Milkie, DPM, Bela Pandit, DPM

I disagree with Dr. Shavelson's remarks regarding the Fotona laser. We have had very good results with it and excellent customer service. It has also been minimally uncomfortable for most patients. We are also pleased that Fotona has been FDA-approved. I recommend it without hesitation!

Chris Milkie, DPM, Milwaukee, WI, cmilkie@wi.rr.com

I have been using the Fotona Laser for almost two years now and have had great results. I have only positive things to say about the Fotona laser and its representatives that Dr. Shavelson mentions. I personally receive emails regularly for follow-up, and they are always available on the phone. In fact, the representative even sent me a 17 year old patient who drove two hours for laser treatment for a nail that was misdiagnosed as cancerous when, it fact, it was a fungal and yeast infection. The Fotona laser cleared up the nail immediately.

Never once was the laser promised to be pain-free, and to be honest, the pain is very minimal. In fact, during training, it was taught that the patient would have some discomfort. All the other reputable lasers I have had a chance to test all caused pain as well. I am not sure what personally happened in Dr. Shavelson's experience, but don't rule out a great laser due to one review. And, as with any treatment for onychomycosis, the patient needs to initiate and maintain  lifestyle changes (i.e. not walking barefoot, replacing shoes, maintaining hygiene, and treating the tinea pedis, etc.) to see the best results.
