5 Common Billing Mistakes to Avoid

It’s easy to make small mistakes when billing. Unfortunately, small mistakes can sometimes cost you big money. Fortunately, it might not be as difficult as you think to avoid them. Here are five ways to prevent some of the most common - and costly - billing mistakes.

5. Explain Your decisions. - Not getting prior authorizations when payers require them for procedures and medications is another common mistake. They’re also a hassle. “Pre-authorizations are a big administrative burden. It’s one of the biggest issues for our members,” Brennan Cantrell, commercial health insurance strategist for the AAFP says. “If you have enough staff, it’s ideal to have one person in charge of this.” If that’s not possible, you may need to consider reviewing and adjusting workloads. You also need to make sure the diagnosis code and the procedure code match in order to demonstrate medical necessity to payers. If not, the claim will be refused. It’s important that your staff be well trained on coding if you want to avoid this common error.

QUERIES (CLINICAL)
Query: Balance Braces
 
Almost every DME company has their own version of a  "balance brace".  I was wondering if anyone can answer a few questions for me. The brace is often touted at seminars as part of a fall prevention protocol. Question 1) Is there any proof at all in the medical literature that wearing of this brace prevents falls? (I am not asking if it decreases postural sway). 
 
My second question is from a biomechanical standpoint, the brace cuts down on sagittal plane motion of the ankle as there is a stationary plate under the device - is this truly beneficial over allowing sagittal plane motion? In other words, is there any proof that this configuration of the brace is more beneficial for fall prevention than a typical split upright AFO?

In a recent publication entitled, "What Your Gait Says About Your Health", the author(s) went into great detail about some issues every person experiences during their lifetime. As podiatrists, we should be on the forefront of monitoring these issues, documenting them, and referring patients to a variety of other practitioners as deemed medically necessary. A question to ask is why wasn't a podiatrist lead team or institution performing this study?

Stan was my co-undergrad at NYU, and classmate in the NYCPM (then the MJ Lewy College of Podiatry) Class of 1960. We were the smallest graduating class ever, just 17 of us, and naturally were all very close. “Schtendle” was the tallest (around 6’), and one of the most fun guys in the class. He was also a really sweet, gentle, nice guy, and it was great just keeping in touch on and off over the years. I know that his patients loved and respected him greatly. Our class has unfortunately dwindled to a very few remaining, and like the others who have passed, he’ll be sorely missed. My sincere condolences to his kids, grandkids, and great granddaughter. R.I.P. Stan.

RE: The Passing of Stanley Schwartz, DPM

I'm truly sad to learn of the passing of our dear classmate, Stanley Schwartz, lovingly referred to as “shtendel”. As I, Stanley commuted from Brighton Beach to the M.J. Lewi College of Podiatry, a 2 hour trip to 125th Street. 

Stan was loved by all his classmates, and I’m sure related well to his patients. Our class of 1960 has unfortunately dwindled, but those of us still lucky to survive have very fond memories of the four years we spent together. Shtendel will not be forgotten.

After 35 years of practice, I made the decision to charge patients who do not cancel at least 24 hours in advance. The fee has gradually risen over the past 10 years from $40 to $75. I wish I had done it 25 years earlier. This was never done to get income for the missed appointments; it was always to try to get patients not to cancel last minute. Every new patient receives and signs a form stating our cancellation policy at the same time as they fill out their new patient intake forms. 99% of patients sign the cancellation forms. When patients call to cancel the same day, we remind them of the cancellation policy and about 1/3 of the patients say "OK, I'll make it." That alone makes the policy worthwhile.  
 
Unfortunately, we also get people who just don't show up for their appointment and don't call. We call those patients and depending on their reason for not showing and not calling, we frequently forgive the first time. If a patient has a history of no-shows, we will not let them schedule another appointment until the $75 no-show fee for the last visit has been paid rather than let them pay it at the time of their first visit.  
 
I tell patients we instituted this rather than overbook to make up for cancellations. I also think that most patients respect you for it and frequently patients say, "I'm fine paying it. I realize it was my fault." We receive thousands of dollars in "no-show" payments a year.
