Should certifying boards grant emeritus status to retired podiatrists?

PODIATRISTS IN THE NEWS - PART 1
Barbie-Like Cavus Feet Not Ideal: TX Podiatrist

Walking and living with an extremely high arch isn't as picture perfect as it's made to look. For starters, "no one actually has feet perfectly shaped to fit the form of a high heel," Chanel Perkins, DPM, a Texas-based, double-board-certified podiatrist, tells POPSUGAR. And for those who come close to it, life can be pretty uncomfortable. In fact, there's a condition known as cavus foot, which is essentially a high-arch foot, even when standing, that "somewhat mimics Barbie's famous foot type," according to Dr. Perkins.

Dr. Chanel Perkins

Many people are born with high arches, so genetics can definitely play a role in whether you develop cavus foot, per the Cleveland Clinic. For others, cavus foot may be a symptom of another condition such as stroke, cerebral palsy, spina bifida, and more. Cavus feet can cause other issues, from hammertoes to instability when walking and calluses on the balls, heels, or sides of the foot, says Dr. Perkins.

PODIATRISTS IN THE NEWS - PART 2
NC Podiatrist Discusses Risks of Sun Damage and Skin Cancer

Don’t overlook your feet during your sun protection routine. Feet get sunburned too, and melanoma on the foot or ankle is more likely to be misdiagnosed than on any other part of the body. A study published in The Journal of Foot & Ankle Surgery reported the overall survival rate for melanoma of the foot or ankle is just 52%, in sharp contrast to the 85% survival rate for melanomas on other areas of the body.

Dr. Gretchen Lawrence

Apply sunscreen to the tops and bottoms of feet and limit sun exposure. Gretchen Lawrence, DPM notes, “If you spot abnormal moles or pigmented skin, including under toenails, visit a foot and ankle surgeon. Early detection and treatment could save your life.”

PRACTICE MANAGEMENT TIP OF THE DAY
5 Coding Mistakes That Cost You Revenue

Accurate coding is one of the most intricate and often frustrating tasks that doctors and staff must do. Erroneous coding can bring significant financial duress to a medical practice, either in the form of decreased revenue, audits, or even clawbacks from private insurers and revenue audit contractors (RACs).

2. Missing E/M Codes

Oftentimes, this is the result of incomplete charting, typically due to provider distraction. Charts without follow-up typically result in the claim being sent late or unbilled.

CODINGLINE CORNER
Query: Adjusting Hardware

A colleague of mine posed an interesting scenario, and what I thought was an easy answer wasn’t so clear. What do you think would be the best way to code this scenario?

A patient underwent hammertoe correction using a screw beamed through the toe. Everything was fine, but at week 6, the screw loosened and was backing out, tenting the skin. There is no infection just irritation. The plan is to return to the operating room and “adjust” the screw by tightening it and thereby re-seating it into the toe. It is within the global period. It isn’t a removal and replacement, just re-tightening the existing screw. Thoughts?

R. Alex Dellinger, DPM, Little Rock, AR

Response: This is a challenge. I’m not aware of any CPT for the procedure you’re describing. I did research this and found one suggestion from a professional coder. They advised using the code CPT 20680, introduction or removal hardware, deep, with modifier -52, signifying reduced service.

My only other suggestion is to use CPT 28899, submit with an operative report and a letter explaining medical necessity and your rationale for performing this procedure. Request a peer review.

RESPONSES/COMMENTS (MEDICAL-LEGAL)
From: Sloan Gordon, DPM

Unfortunately, I don’t agree with my esteemed colleague. Many medications that we prescribe in a normal podiatric practice are actually controlled substances (scheduled drugs) without being opioids. Often, I find people grouping opioids with other controlled substances that are unrelated. Some examples:

Gabapentin (neuropathy), Ultram (pain), Ambien (post-op for sleep), and certain benzodiazepines to relax a patient undergoing an office procedure.

I think to be a legitimate provider and be able to have a complete armamentarium to take care of your patients, one should be able to have all of the appropriate licenses and credentials to do so.

Sloan Gordon, DPM, Houston, TX

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1A
From: Robert Scott Steinberg, DPM

The Midwest Podiatry Conference (MPC) used to have huge attendance, but no more. The conference is held in downtown Chicago, where the cost of everything is sky-high. It's a killer for exhibitors.

The MPC needs to poll past attendees.
• Is downtown Chicago the draw?
• Do attendees make it a family vacation?
• Would the conference be more attractive for attendees and exhibitors if moved to the suburbs where costs would plummet?

Robert Scott Steinberg, DPM, Schaumburg, IL

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1 B
From:  Allen Jacobs, DPM

The recent commentary regarding the state of post-graduate education for CME is noted. His participation in seminar programs and his products for the betterment of patient health are appreciated. Like Mr. Cioe, I miss the days of the large scientific gatherings such as the great Hershey seminars, the old Connecticut state meetings, and do enjoy the few remaining similar programs such as the NYSPMS annual meeting.

Peter, Paul, and Mary popularized the Bob Dylan song The times they are a changin. So too has podiatry post-graduate education. CHEC is first and foremost to serve the doctor, not the exhibitor. My own seminar, the St. Louis Podiatry Seminar, has increased in registration every year since going...

Editor's note: Dr. Jacobs' extended-length letter can be read here.
