WesternU Team Publishes Previously Unseen Discovery of Obturator Nerve
Two separate teams of WesternU students discovered cases where the cutaneous branch of the obturator nerve (CBO), which usually remains in the thigh, ran into the foot. A few previous reports in the literature describe CBOs passing the knee to innervate portions of the calf, but none show CBOs extending to the ankle and foot. “Cutaneous Branch of the Obturator Nerve Extending to the Medial Ankle and Foot: A Report of Two Cadaveric Cases,” by Brittany Staples, DPM, Edward Ennedy, DO, Tae Kim DO, Steven Nguyen, DO, Andrew Shore, DO, Thomas Vu, DO, Jonathan Labovitz, DPM, and Mathew Wedel, PhD, was published July 23, 2019 in the Journal of Foot & Ankle Surgery.

“Students and faculty from podiatric and osteopathic medicine collaborated interprofessionally on this research, which is contributing new knowledge to the medical community,” said WesternU College of Podiatric Medicine Associate Dean Jonathan Labovitz, DPM, one of the paper’s co-authors. “We are proud of the initiative and skill shown by our students in the anatomy lab and we hope they carry this sense of discovery and teamwork forward as they complete their residencies and begin their careers.”

NY Podiatrist Recommends Good Supportive Sandal for Summer
“Sandal season signifies pedicures, foot beauty, and freedom to toes and feet. However, with that freedom comes a few cautions,” says Doug Tumen, DPM, a board-certified podiatrist based in New York’s Hudson Valley. According to Dr. Tumen, podiatrists’ offices usually receive an influx of patients in the months leading up to Labor Day—and most of those cases are due to either sandals or summer beauty faux pas.

“Not all summer sandals offer good support. Many are thin-soled, flat, and too flexible. Flip-flops lead the list as the most common cause of summer foot pain. A good, supportive sandal, should have a thick firm sole and incorporate arch support and cushioning,” says Dr. Tumen.

OH Podiatrist Questions Robert Mueller
Former special counsel Robert Mueller appeared unwilling or unable to answer a basic question about the scope of his investigation into potential conspiracy between the Trump campaign and Russia, striking another odd note in testimony Wednesday that appeared at times off-kilter and confused. Rep. Brad Wenstrup, a Republican from Ohio, asked Mueller whether it was “accurate to say that your investigation found no evidence that members of the Trump campaign were involved in the theft or publication of Clinton campaign-related emails?” Mueller first paused and then asked Wenstrup to repeat the question, which the congressman did. Mueller paused again, and then answered, “I don't know. I don't know. Uh, uh, what they've, well, uh — ”

Dr. Wenstrup got specific, quoting from Mueller's report. “On page five, it says your report ‘did not establish that any members of the Trump campaign conspired or coordinated with the Russian government in its election interference activities.’ So therefore it would be inaccurate, based on this, to describe that finding as open to doubt, that finding being that the Trump campaign was involved in the theft or publication of the Clinton campaign emails," he said “Are you following that, sir?” Wenstrup asked. “I do believe I am following it,” Mueller replied. “But it is, um, that portion of that matter does not fall within our jurisdiction, or fall within our investigation.”

Video of OH Podiatrist Dancing with Dog Goes Viral
Laura Testrake started an Instagram account dedicated to her family’s dog Hank just last week and he’s already hit celebrity status. Testrake saw ABC’s “Good Morning America” highlight other cute dogs and decided to tag the morning news program as well as anchor Ginger Zee in a video of her husband Dr. Matthew Testrake, a podiatrist at Cleveland Clinic, holding their dog Hank, a 7-year-old Goldendoodle, and dancing to Ed Sheeran’s “Thinking Out Loud.”

A representative for GMA reached out on Monday morning and right after, Testrake’s video was being played on television. “The kids always think it’s funny when my husband picks Hank up because he is so large. We were watching my sister’s wedding video and singing when Matt came around the corner with Hank dancing. I just thought it was too funny not to share.” Testrake said. She added, “I would say Hank is not partial to Ed Sheeran as he enjoys many genres.”

Evaluation of the NNB Metric for the Diagnosis of Cutaneous Melanoma
An article in JAMA Dermatology showed that among 46 studies, which included 3681 clinicians, 455,496 biopsied tumors, and 29,257 melanomas, the number needed to biopsy (NNB) to diagnose cutaneous melanoma ranged from 2.2 to 287. A weighted mean for NNB was 18 for all clinicians, 7.5 for dermatologists, 14.6 for Australian primary care providers, and 13.2 for all U.S.-based dermatological practitioners (which included dermatologists and advanced practice professionals).
The NNB for diagnosis of cutaneous melanoma varies widely, and standardization of reporting this metric warrants further exploration. Clinician-directed educational opportunities may improve the NNB, particularly among non-dermatologist clinicians.

5 Metrics to Improve Your Practice’s Finances
Here are five key metrics that can shed light on your practice’s financial health:
1. Net collections - This measures the effectiveness in collecting reimbursement dollars. However, this metric is not relevant unless two months have passed since the reporting period, as this feeds providers with a clearer picture of collections after the claim cycle. Net collections can be impacted by high deductibles that increase patient liability, procedures that are not covered, incorrect posting processes, or unworked A/R. Practices must know the difference between gross and net collections and assure that all adjustments were made correctly.

Query: NDC Codes on HCFA Forms
We have not been getting paid for injections because insurance companies are telling us that the NDC code (on the injectable) has to be on the HCFA form and some carriers do not want it on the form, at all. Our clearinghouse tells us that we cannot have it both ways. If we put the NDC code on one form, it has to be on all the forms (computerized.) How are others handling this problem?

Response: This seems to be an issue for both your EHR and the clearinghouse to resolve. If the carrier doesn’t want this info, then the clearinghouse shouldn’t send it and your EHR should not prompt your biller to input it into the claim. Conversely, if the carrier wants it, your EHR should prompt you to input this info and should not allow the claim to process without it. These are no doubt complex issues to resolve, but your clearinghouse needs an attitude adjustment on this one.

Dr. Armand R. Gasbarro, DPM, age 83, passed away at Northwestern Memorial Hospital on July 23, 2019. He had been in medical practice for 60 years at Lansing Foot and Ankle Center in Lansing, IL.

He was a U.S. Army Veteran and graduated magna cum laude from the Dr. William Scholl College of Podiatric Medicine in 1957. He participated in training podiatric interns and residents at Northlake Hospital from 1965 to 1982.

Reconstructive Foot and Ankle Surgery - This fellowship is a one-year experience during which the Fellow will develop a rational approach to revisional and reconstructive foot and ankle surgery. The Fellow will function as a Specialist and participate in the teaching of podiatric medical students and residents, and have ample opportunity to be involved in clinical and basic research projects. The fellowship will provide the podiatric surgeon further expertise in Charcot reconstruction, trauma and deformity correction.
